Employee was loading pallets on a telehandler and tripped and fell, cutting EE's right hand.

Operator was walking and tripped on a rock on dry ground and tried to catch self with left hand and broke wrist.

Employee was working on lifting spring bolts and hydraulic cylinders on 5 1/2 ft. crusher or moving toggle ends for hydraulic toggle. Sought medical treatment on 8/23/2024. Returned to work with no restrictions. Surgery scheduled for 9/9/2024.

OTR technician was repairing chains on a Loader. EE was cutting off a chain link and the cutting wheel shattered and a piece of the wheel struck the technician on the shoulder causing a laceration.

Wet, muddy. Worker had rubber liners over work boots. Buckle of liner got caught on pull rope for wire pull and tripped. Braced fall with left arm. Experienced soreness recently and wanted to get MRI.

Employee was trying to lift a piece of plate metal into the bed of a service truck. The plate slipped, catching the employee's left little finger between the plate and the bumper, resulting in a broken little finger and 11 stitches.

Injured employee felt pain in EE's left shoulder as EE worked to maneuver the ansul rig hose over rock next to the left rib while loading and underground shot

Struck by flying object

A piece of metal became embedded in the injured employee's right eye sometime after performing welding operations on the D9 dozer blade. It is believed the metal entered the eye when the employee wiped face.

The employee had finished his shift and had parked his haul truck in the designated parking area. He was noticed by another employee sitting in the haul truck seat slumped over the steering wheel. The employee called 911 and other employees performed first aid & CPR (using the onsite AED). The employee was pronounced deceased at the scene by the Medical Examiner.

The injured employee alleged that he was stepping off the haul truck ladder and his left foot went into a washed out rut. He then stated that he felt a pain in his left knee.

Struck against stationary object

Injured employee was accessing the Komastu PC 270 trackhoe. He used his right foot on the crawler step and placed his left foot on top of the track. When he placed weight on his left foot, his left knee extended outward to the left resulting in a strain.

Miner was repairing CAT D-8R dozer at customer mine site. Jack stand slipped. Caused contusion to left index finger. Miner went to clinic for evaluation. Diagnosis was contusion. Miner was given splint to protect finger while working. Instructed to take OTC meds for pain. Returned to regular duty 12/10/2013.

Injured employee was assisting another employee to change finger door on the chute. When the weight of the door began to slip the injured employee grabbed the door, and when he did he felt a twitch in his back. Employee did not seek medical attention until Wednesday, January 2, 2012. Injury resulted in a strain to the back.

Injured employee was removing the grating in order to access a turn buckle to make necessary adjustments, when the grating hung on adjacent beam. When the grating was freed the employee lost his balance which caused the grating to strike his shin. Resulting in a laceration.

After hours, the small out building (Approximately 10 ft. X 15 ft.) caught fire and was extinguished by the local fire department. The cause of the fire is under investigation.

The alleged injury occurred when the EE was installing a new cable for the quarry pump. The EE was pulling on the cable, when he felt a strain, resulting in a hernia.

The injured employee was preparing to start washing platform and catwalks around finishing. When the employee was pulling wash hose, he felt a pain in his right arm.

The injured EE and another EE were disassembling a platform in the plant. The injured EE was removing bolts and part of the platform fell, striking him on the right hand, resulting in six stitches.

The injured employee was working with another employee putting belt clamps on a conveyor. The clamp slipped from the other employees grasp and caught the injured employees right hand resulting in a laceration.

THE EMPLOYEE WAS HOOKING A SPREADER BAR TO THE MAIN HOOK ON A CRANE. HIS RIGHT HAND GLOVE CAUGHT UNDER THE CLEVIS AND HOOK, RESULTING IN A FRACTURE IN TWO PLACES ON HIS RIGHT THUMB.

EE AND OTHER EE HAD A NUT FROM A CANE CRUSHER IN A BUCKET WASHING IT OFF. THE BUCKET WAS ON THE GROUND BUT AFTER SOME TIME IT LEVELED AND THE NUT SLIDE OUT THE BUCKET AND HIT HIS RIGHT FOOT.
