Employee was bending parts and he was repositioning the part with a 4 lb hammer. He delivered a glancing blow with the hammer causing him to strike his left ring finger.

Over-exertion in pulling or pushing objects

Injured was in the process of moving a barrel of chain. He stated he was rolling it to the fork lift and heard a popping sound in his right shoulder. He went to the Dr. He did not receive a diagnosis & was released to return to work. On 5/13/10 injured went for a follow up & was placed on restricted duty & referred to therapy. Injured was taken off work 8-17-10 for surgery.

Caught in, under or between a moving and a stationary object

Injured was using a portable dumpster to empty trash and while dumping the hopper, he got his finger caught in a pinch point.

Struck by flying object

EE was cleaning check valve with pneumatic needle scaler, when a piece of foreign material entered right eye, past safety glasses with side shield and a face shield. EE had material removed and was given antibiotic ointment for the eye.

Over-exertion in lifting objects

EE was helping to lift 120 pound grease barrel into the back of a pickup, when he had lower back pain.(EE went back on restrictions on 5/14/08.)

Employee had beam lying on saw table rollers. He lifted the end to align it with the saw and strained his back. Doctor placed man on temporary restricted duty, not to lift over10 lbs., and was given medications.
