Employee stepped off the stairs of a clay shredder and turned/hurt EE's right ankle.

The guide chain on the forklift the employee was operating kinked at the pulley, the employee stuck EE's hand between the pulley and chain. The chain smashed the tips of the 2 middle fingers on EE's right hand.

Inspector walked over to railroad spur to talk to KWT employee. After finishing the conversation, the inspector turned to walk away and fell before resuming the inspection. Inspector fell on the left wrist causing the injury.

Employee was working on the engine of a haul truck & was leaning on his right side stomach to reach across the engine. He felt discomfort in his right side stomach. Employee was not seen by a doctor until 6/18/2013.

The employee has had biennial chest x-rays since 1999 which have historically been read negative for indications of occupational lung disease by a NIOSH certified B-reader. In 2012, Unimin changed B-reader's and the new b-reader has read the employees most current & historical Unimin images as positive for occupational lung disease.

Employee was changing a tire on the 3rd axle of his dump truck. When he began to re-install the tire to the hub he felt a pop in his right shoulder.

Employee was changing a light bulb using a scissor lift. A forklift operator was moving bulk bags in the area around him. The employee stepped off of the scissor lift and was struck from behind by the bag on the forklift. It pushed his left side into the scissor lift. He received a contusion to his left arm resulting in modified duty on 6/21/2011.

Reportedly the employee was operating a shredder when the flipper kicked off and quit running. Reportedly the employee was shoveling the packed clay from the flipper when he felt pain in his left shoulder. Employee was placed on restricted duty by doctor on 8/19/2010.

Employee was cleaning up at the end of his shift. He was on the platform at the collection tank. He was lifting the lid on the tank with his left hand when he felt pain in his left shoulder area. On 6/29/09 he was diagnosed with a bicep tendon rupture. He was placed on restricted duty.

Employee was shoveling clay material from under a blending hopper. As he was dumping the clay from the shovel he hit his elbow on a support beam. On 3/5/09 he was taken to doctor for fluid retention on his elbow. The doctor removed the fluid from the elbow.

Employee was putting a bag on the bagger discharge spout. The bag got caught on a burr on the spout. He reached around the spout at an awkward angle to pull the bag off and felt pain in his right shoulder. He was taken to a doctor and placed on light duty.

EE was walking from dryer control room to activate a control at a blending hopper. As he was stepping onto the concrete pad he tripped and fell. On 10/10/07 he was taken to a dr and was diagnosed with a strained calf muscle in his left leg. He is currently on restricted duty as of 10/10/07.

EE was turning crank wheel to close a boxcar door. EE was facing the handle, which was about head high. The handle stuck, EE applied more force, handle came loose and his upper body weight went forward. EE felt pain in his right shoulder. Given medical treatment on 8/31/07 and placed on restricted duty for a muscle stain on 9/4/07.

EE was hammering on the side of a feed hopper with a small hammer. He felt some pain in his lower back. He was taken to the doctor on 2/5/07 and diagnosed with a strained muscle. He was placed on restricted duty.

Employee was tightening the flipper bolts with channel lock pliers when he felt pain in his left elbow. He continued to work for several weeks with some discomfort. On 12/09/06 he was taken to the doctor for medical treatment and place on restricted duty.

While adjusting the flipper on the shredder, the adjusting lock nut broke loose. The wrench he was using slipped forward and his momemtum carried him into a guard on the machine. This caused a cut above his left eye, resulting in 3 stitches.

Employee was laying out pallets in the baghouse to put bags on. As he was sliding a pallet in place, he felt something pop in his back. On 6/5 he was taken to the doctor and placed on modified duty.

HE REPORTED ALLEGED ACCIDENT ON 8/15/05. HE WAS TAKEN TO DOCTOR ON 8/30/05 AT WHICH TIME HE WAS PLACED ON RESTRICTED DUTY

EMPLOYEE WAS USING A ""RAKE"" TO CLEAN UNDER A HOPPER. AS HE WAS PULLING MATERIAL (CLAY) TOWARDS HIMSELF HE FELT A PAIN IN HIS RIGHT RIB CAGE AREA. HE WAS TAKEN TO DOCTOR ON 5/9/05 AND WAS DIAGNOSED AS A MINOR MUSCLE STRAIN.

EE STEPPED ONTO THE TOP OF SUMP TANK WHICH WAS 15"" ABOVE THE FLOOR. HE HAD JUST FINISHED WASHING OFF THE TANK AND IT WAS WET. HE SLIPPED AND FELL BUMPING HIS RIGHT KNEE. HE WAS DIAGNOSED WITH A BRUISE AND GIVEN NO MEDICAL TREATMENT. ON 7/1305 HE WAS PLACED ON RESTRICTED DUTY WITH FOLLOW UP MEDICAL TREATMENT

IT WAS A RAINY AND WET DAY. EE WAS WALKING ON CONCRETE AREA NEAR THE RAILROAD TRACK WHEN EE TRUNED TO GO BACK THE WAY EE CAME, EE FOOT SLIPPED OFF INTO THE DRAINAGE AREA. EE SLIPPED AND FELL TRYING TO CATCH SELF WITH RIGHT HAND, SUSTAINING A FRACTURE IN ARM.

A PORTION OF THE LEG OF THE TANK WAS BEING CUT OFF. EE WAS PUSHING ON THE TANK ROCKING IT BACK AND FORTH TO DISLODGE THE PART OF LEG BEING CUT. AS EE WAS DOING THIS, EE LEFT HAND WAS CAUGHT BETWEEN THE TANK AND CAT WALK.

Employer was walking near the railroad track. He slipped down on extremely slick ice and snow, landing on his left leg and foot. He broke a bone in his left ankle.

EE had pipe wrench on an airlock shaft trying to turn it. he squated underneath the pipe wrench/cheater bar and pushed up, putting pressure on the top of his shoulde3r. this resulted in an apparent shoulder strain.

Ee was working with another ee, moving metal flat bar from one storage rack to another. One piece that was under the piece he was moving fell, striking him on the left leg. Ee received contusions on his leg.

AS EE WAS GETTING OUT OF THE TRUCK, HE HIT HIS ELBOW ON THE ""DOOR SEAL"". ALTHOUGH THIS INJURY OCCURRED ON 1/13/04, EE DID NOT IMMEDIATELY HAVE PROBLEM ENOUGH TO WARRANT MEDICAL ATTENTION. EE SUBSEQUENTLY WAS TAKEN TO THE DOCTOR ON 02/19/04 AT WHICH TIME IT BACAME A MSHA REPORTABLE ACCIDENT.

EMPLOYEE WAS STANDING ON THE GROUND BEHIND RAILCAR LOOSENING THE HANDBRAKE. HE HAD TO REACH ABOVE HIS HEAD TO LOOSEN THE HAND BRAKE WHEEL. WHEN HE PULLED ON THE HAND BRAKE WHEEL HE FELT PAIN I N HIS RIGHT SHOULDER.

EE WAS BAGGING, PULLING BAG OFF OF BAGGER SPOUT. BAGS WERE FLUFFY AND HARD TO PULL OFF. STRAINED WRISTS PULLING BAGS.

EE WAS GOING TO MOVE SHREDDER AFTER SHREDDING CLAY. HE FORGOT TO PULL MAIN DISCONNECT BEFORE ATTEMPTING TO PULL THE TRAIL CABLE MALE PLUG FROM TH STATIONARY FEMALE SOCKET. AS PLUG WAS PULLING OUT, THE WIRES WERE LOOSE ENOUGH TO CAUSE IT TO SHORT OUT AND THE LEADS ARC'D, PRODUCING INTENSE HEAT THAT CAME OUT OF THE SIDE OF THE MALE PLUG BURNING FINGERS AND LEFT HAND.

EMPLOYEE WAS LOOSENING A NUT AT THE NOODLE EXTRUDER WITH A TORQUE MULTIPLIER WRENCH. HE WAS ALSO USING A 5' CHEATER BAR. THE NUT CAME LOOSE AND HE HIT HIS FINGER, MASHING IT BETWEEN THE BAR AND THE GUARD RAIL NEARBY.

EE WAS GOING DOWN STEPS AND HEARD AN AIR VALVE OPEN. HE LOST CONCENTRATION WHEN HE LOOKED BACK AND MISSED THE LAST TWO STEPS. WHEN HIS RIGHT FOOT CAME DOWN ON THE LANDING HIS RIGHT KNEE HYPER EXTENDED. HE FELL AND BUMPED HIS RIGHT KNEE AGAINST THE WALL. KNEE WAS BRUISED AND HYPER EXTENDED.

EMPLOYEE WAS MOVING A 50# BAG OF BARIUM CARBONATE FROM THE PALLET TO THE TABLE WHERE IT IS STORED, A DISTENCE OF ABOUT 12 FT. WHEN HE PICKED UP BAG AND TURNED, HE FELT A PAIN IN HIS BACK. EMPL OYEE SAID THAT HE POSSIBLY TURNED HIS UPPER BODYBEFORE MOVING HIS FEET.
