Struck by falling object

Operator was conducting remoting activities in an area known as the 2025 area when a large rock contacted the cab. Operator was found in the cab out past the designated no-go line and sustained fatal injuries.

Caught in, under or between a moving and a stationary object

A group solely comprised of RWS contractors [MSHA Contractor ID B5045] was working in the mine. A pump sitting on a trailer was moved by RWS contractors by a telehandler, without notification to Nyrstar. RWS hitched the trailer to their telehandler. RWS pulled the trailer up a ramp. The trailer hitch broke. An individual RWS EE traveling on foot behind the trailer was struck

Struck by falling object

An employee was loading a face with explosives when a rock fell from the face and caused a laceration to the employee's leg. The employee received 7 stitches to the leg laceration at the Hospital.

The employee was walking across the roadway underground to a power center to turn off a fan when EE rolled EE's ankle and felt a pop.

A rock fell down the shaft and struck the employee working in the shaft on the back of the neck. A contusion was incurred to the impact site at the back of the neck.

Employee was cutting the metal band on a bundle of wire when the band came back from stored energy and slapped the back of the employee's hand causing a laceration on left hand.

Struck against stationary object

The employee was making repairs to #33010 Sandvik TH540 Haul Truck Air Conditioning system. During live testing, the employee's thumb came into contact with the condenser fan resulting in a fracture/laceration.

Around 7:00 am employee was dressed in street clothes and getting ready to walk to parking lot to go home after the night shift rotation had ended. As employee was walking out of change house and up the steps they felt a pop in their left knee. Could not bear any weight on left leg.

Struck against a moving object

Employees were leaving work area in 2250, when the driver of the tractor was getting out of the way for the 460 TH to pass. When re-entering the road, the tractor was in loose material. Upon breaking free, the tractor swung to the right contacting the rib. The employee seated on the same side, hit hard hat on the railing of the man carrier upon hitting the rib.

At approximately 8:30AM on 8/28/23, a mechanic was attempting to repair a blown turbo boot on the engine of the 37605 supply hauler truck. EE stood up on the offside fender stepping backwards and tripped over the exhaust guard bracket causing EE to fall backwards into the oscillating section of the supply hauler hitting face and ribs on the truck.

Crusher operator fell after EE's feet became entangled in crusher wash down hose. Employee felt a sharp pain and heard a pop in EE's ankle when EE fell.

Employee was riding the man cage and dropped lunch box. When the employee went to pick it up, the handle was at an angle on the cage floor. As the employee went to grab the handle, EE's finger got cut from a sharp piece of metal from the cage floor.

Employee states while cleaning the mill hole off, the loader hit a large rock and came to a sudden stop jarring EE's back. Employee complaining of lower back pain.

Employee was cleaning safety glasses. When the employee went to put them back on their face they accidently scratched the left eye with the earpiece on the safety glasses.

At 8:30 am a mechanic was removing old pressure washer in wash bay to install new unit. During removal the wash unit tipped from the telehandler forks and guard on washer unit caught the mechanics glove cutting EE's right middle finger and scraping stomach. Mechanic was taken to the hospital and received 4 stitches in EE's right middle finger.

Employee was moving lumber outside the training room. They bent down to move a 2x4, as employee was getting up said they felt sudden pain in lower back.

Employee was stacking screen on rib, when EE stepped back EE didn't realize EE's footing was on a piece of screen. The screen slid out from underneath EE, causing the employee to slip and fall.

Employee was assisting with a pump when a rock fell from the rib striking the employee in the hard hat. As a result, the employee sustained soreness and a neck strain. Incident classified as MTI.

At approximately 11:50am at Young Mine a miner had entered the 7690 heading after mucking activities, checking the area, when an approximately 100 pound rock fell striking EE on the head and arm. The rock fall occurred on the left-hand rib, where the rib meets the face, about 10 feet up the rib line.

Employee was changing a lift cylinder on top of a loader when the incident occurred. The employee was trying to move the crane in a better position with a strap when the employee lost balance falling approx. 5ft to the ground below.

At 1:25 AM the blasting crew had finished hand scaling the back in 9664 and were in the process of loading first couple holes when a rock fell from the back and struck one of the blasters in the left forearm and wrist causing a laceration that required stitches.

Employee was marking up the face in preparation for drilling when a rock fell from the face striking the employee in the head, then brushing down the right side of the employee's body. The heading and face was checked prior to work beginning in the area.

Employee mounted a tractor on the surface prior to going underground to start shift. After mounting the tractor, employee thought that the safety latch/gate was closed on the rear of the tractor and went to lean back against it when employee fell out approx. 3ft landing on left side.

A blaster was using a box cutter to cut the debt cord on a round loaded in 9664 and lacerated left hand below the thumb area. Safety was notified and transported the employee to the local ER for evaluation and had 14 stitches put in the laceration.

Operator operating a telehandler on a decline when machine rolled down decline hitting a parked pickup truck. Operator was complaining of lower back pain and soreness.

Employee reported that employee had a Allergic reaction to the muck boots. Employee knew employee was allergic to neoprene and wore the muck boots

Employee was trying to get a piece of steel out of striking bar when a piece flew off and struck him in the arm. Employee initially declined medical treatment. Employee later went to the dr. and had the metal removed and was prescribed antibiotics.

Employee placed left hand on machine to brace himself while walking around equipment on the right side of engine compartment. He did not see the broken piece of mirror under the gasket laying on the cowl of the cab. When employee placed hand on gasket, the piece of mirror punctured his left hand between his two middle fingers.

Fall from machine

Employee was stepping off of his bolter rig and slipped on the bottom step. As a result of the slip, he hit the ground landing on a rock. The force of breaking his fall resulted in an anterior dislocation on his left side shoulder. At the time of the injury, the employee had 3 points of contact established. The step of the bolter was wet, as is common in underground mining.

At 12:35p.m. a fire was reported on the 34014 Oldenburg Drill hdg. in the 95-17 Hdg. in the 85-15 stope.Foreman called for an evac. of the mine. MSHA,mine rescue,911,G.M. was contacted at 12:50 p.m. At 1:35p.m. all accounted for except 3 util. EE's.They could not evac. w/o passing thru smoke, they comm. to the surf. by phone, rescue arrived and trans.men to the srf.at 3:50 p.m.

A haulage truck inadvertantly raised the bed into dump position in a low back area mashing the high voltage cable, thus causing a power loss to the 85-15 area of the mine and ths resulted in loss of power to the man cage. MSHA was notified and the mine was evacuated until power was restored to the man cage approximatley two hours later.

EE was rolling a tractor tire out from machine with another EE and the IE was walking back and tripped over a rock pile and landed on the rock pile. This was an underground mine and the only light was the light on the helmets.

using jacks to slew a 15 ton rail car back onto the tracks when one jack became stuck beneath the car. Miner stood on jack handle trying to get it to release when the handle released and struck him in the rt leg and pinned him against the coupling. Placed on restricted duty until MRI completed

Electrical Contact burnt out making the hoist inoperable, the mine was evacuated and MSHA was contacted. The hoist was down for a total of 27 hours. Repairs were made and the mine was examined prior to reentering the mine.

Employee was in the process of servicing his equipment in the 412 shop area, he was using a grease hose to service his truck and as he kneeled down he felt a pop in his right knee.

The injured was tossing a rock the size of a football into the car he was pulling, he tripped about the time he done this action and caught his hand between the rock and car causing a fracture to the meta carpal.

The employee was walking up the belt line tracking the belt. As he was walking behind the pillar at the midsection. The employee's right knee buckled and the employee fell to the ground as he fell he struck his Thigh on a metal bar to the belt line causing him to bruise his right thigh.

While boosting a truck at the 71 Mill Hole injured fell from the truck causing a fracture to his right fore arm, and right hip.

While re- railing a wrecked ore car the injured was using a bar 5 feet long to pry the car. The car slewed over to set down on the track pushing the bar in a downward motion, striking the injured on the left leg, he also held on to the bar causing strain to his right shoulder. Injured was placed on (restricted duty on 7-7-10 and is estimated to be on light duty for 4 days.)

While scaling a rib, injured stated he felt sharp pain in his left arm. The injured worked two more shifts then went to the Dr. on 6/28/10. Was examined and released to return to work with no restrictions. On 7/2/10 injured had follow up and was placed on restricted duty.

While installing a new hoist rope for the new man hoist, injured was observing the rope being pulled through the sheive wheel. When the Crosby clamp fouled on the hand rail, it broke the metal loose causing the metal to strike injured on the left side of the knee.

Power to the main shaft and hoisting equipment was lost due to an area wide power outage. The secondary shaft and adit were still usable. Power was restored around 12:00 PM.

EE was hanging pipe back up when the chain slipped off the pipe. The pipe struck the EE and bent him backwards causing pain in his stomach and shoulder. EE has a hernia and a strained shoulder.

Due to a power failure on the property, the secondary hoist was out of commission for more than 30 minutes. The main shaft and adit were still usable. Full power was restored after 4 PM.

EE had grease on boots and slipped while walking down stairs. EE bruised/strained back and hip. EE will receive physical therapy.

EE was driving a truck when he ran over a hump in the road and a pot hole on the other side. This caused his back to start hurting. He went to the doctor and was put on restrictions.

EE stopped to roll a rock out of the way of drilling a lifter hole. While rolling the rock he felt a pain in his back. Pain worsened over next 24 hours. He is now on restricted duty.

EE was helping bolt a hydraulic hose manifold on a boom. While EE has handing manifold up, the other EE slipped off drill and landed on first EE. EE recieved a shoulder strain and lumbar strain. EE received restrictions in which he could not do his job fully on 4/8/08.

Broken guide in shaft caused east skip to stop. Shaft is shut down until repairs are made. Shaft was being used for production only, it is not the primary or secondary escapeway from the mine.

Employee was walking up a back stope and slipped on the muck. Employee fell to the ground and struck his forehead.

Employee was removing a rock jam from the crusher chute. The ground bar being used slipped and the employees arm struck the edge of the opening.

The employee was driving Haul Truck #302 a Toro 40D. He had pulled over and stopped to allow another truck to pass coming the opposite direction. The other truck passed and he pulled out crossing to the other side of the roadway and struck the pipelines on the opposite side.

Cleaning tail gate of dump truck, his right thumb caught in tail gate. Reportable fracture, right thumb; lost time.

Employee stepped off of powder truck, twisted knee and fell. Treated for pulled muscle. Given prescription medicine.

Employee was cleaning up around the tracks. He was lifting a rock to place it back on the car when another rock rolled off and crushed the tip of his left thumb. The injury was a fracture of the distal phalanx of the left thumb. Thumb was placed in a splint and medications were given.

While lifting a case of powder, twisted the wrong way, pulled groin.

Employee was working on transmission. He had his foot propped up on the frame of the equipment. His foot slipped off and hit a piece of metal attached to the frame.The incident was reported three days later after it was infected. Employee was taken to doctor, given antibiotics and cleaned the scrape.

Employee was unloading a powder truck. Apparently, he was twisting while picking up boxes of powder. When employee complained of back pain, we took him to the doctor for evaluation. Doctors evaluation was Lumbar Strain. Placed on temporary restriction to not lift over 10 lbs. given Presctiption Medication.

Employee was working in the shop. Turned to pick up bolt and felt a pain in his back. Refused medical treatment at the time but came in the next day with catch in his back. Referred to doctor and diagnosed as back strain. No lost time, placed or limited duty not to pick up over 10 lbs.

EE was leaving the mine office by the back steps. There were three employees standing on the steps talking. Rather than disturbing their discussion he decided to walk down the grass area and hold the handrail.When he got to the bottom he stepped on the sidewalk and turned his left ankle.
