While working on the 17001 Pipe Truck, the employee was cutting zip ties leading to the alternator when the knife slipped, causing a laceration to the left hand on the end of their thumb. The employee later received 5 stitches at the ER.

The employee was tightening a come along while doing utility work when the come along broke and the hook struck the employees finger. The employees' finger was later on determined to have a fracture at local hospital.
The employee was working on disassembling the temporary hoist for an upcoming job after testing. A steel piece was being hoisted to be removed from the platform when it shifted and struck the employees hand against a handrail. The incident resulted in a broken finger.
Employee was exiting a truck when the slipped and caught themselves with their right arm. The employee initially reported it as a Report Only injury. Since the injury occurred the employee has felt more pain and the injury has turned into a Restricted Work on 10/16/25.
The driller was rigging out of 1482 taking a cable down from the cable hangers when they lost balance causing them to fall and cut left elbow. The employee was taken to the ER and received 3 stitches. They went back to work the same night with no restrictions.

The employee was drilling out a pilot hole in the back of a haul truck for a light bracket. The drill bit employee was using broke and slung a piece of metal in employee's left eye. The employee wasn't wearing the safety glasses properly.
An employee was struck by a lid from a haul truck A/C unit in the left hand after it had fell causing a laceration. The employee received 5 stitches from the local hospital.
While attempting to plug a light vehicle tire, employee inserted the plug and while extracting the plugging tool enough force was applied causing the tool to slip and lacerate employee's left hand. Employee was taken for medical eval at nearest hospital where EE received 7 stitches. Employee was released to work with no restrictions.
Employee was tightening down a ratchet strap while hauling a bundle of bolts when their hand slipped and made contact with the mass of the telehandler. At the time of the incident the employee reported it as a report only incident. Then days later turned into a reportable injury after employer was notified.
Employee tripped over a rock which caused EE's other knee to make contact with another rock causing a laceration.

Employee reported that EE had twisted EE's left ankle while delivering grader tires to the shop for work to be completed.
Employee was checking hydraulic hoses on extra swing of rig when EE attempted to turn around EE's foot stayed planted, and EE's right knee twisted.
Employees were changing a sump pump. The pump was attached to the telehandler basket. While repositioning the pump, this caused the pump to fall, bouncing the employee around in the basket jarring EE.
At approximately 2:30 am blasting crew was loading heading in 1887 Upper. While handler was loading left side of cut rock came loose from top left corner of cut and fell and struck employee in left arm
Employee (mine supply hauler) was unloading grader tires off west side cage. While unloading the 2nd tire it spun around and pinned employees left leg against cage.

Operator was fueling up pipe rig. When the tank filled up it didn't shut off right away and diesel splashed up in operators face and on EE's arms. Diesel ran down behind operators safety glasses getting fuel into EE's eyes. Operator not knowing EE had diesel on arms, wiped eyes with sleeve.
The employee was getting off a tractor to move a piece of pipe out of the way for a telehandler. As the employee was getting off of the tractor when employee stepped off into some water, employees foot was half on rock and when EE put weight on it, EE's foot rolled outward injuring their ankle.
Employee states that they were stepping out of an anfo-rig when they stepped on a rock. Employee fell and caught themselves with their right hand, injuring right hand.
Employee states a rock fell onto the boom of the truck, employee jumped out of the way of the falling rock. When employee jumped attempting to move employee twisted right hip and lower back area.
Employee was pulling on telehandler injuring left shoulder.
The employee was operating a loader in which they were having trouble reaching the foot pedals, which irritated their back. The employee was then placed on a mechanical scaler in which the operation there of, irritated their back even more. The employee notified their supervisor and was taken to seek medical attention.
Employee was using torch cutting bolts off and burned through glove in turn burning left index and middle fingers.

The employee was going to put a bolt on the driver when EE tripped on a rock, falling to the ground. EE stuck hands out to catch self and noticed afterward that the right hand was starting to swell.
Employee was working on mobile equipment and the engine compartment door fell on the employee's left hand
The employee had been installing air and water in the 1092 stope and while attempting to turn the air header back on, the employees feet became tangled causing the miner to fall. The employee placed left arm out to brace the fall and struck the ground.
Operator was traveling down a roadway in a haul truck when struck a pot hole. Operator felt discomfort in back and the incident cause lower back pain and spasms. Incident did not become restricted or reportable till 5/4/2021 after further evaluation from healthcare provider.
Employee was tightening a centerlizer and the wrench slipped cause a laceration to the left hand.

A mobile maintenance mechanic was repairing a hose in a haul truck's engine compartment in the underground diesel shop when the hood of the engine compartment accidentally shut, landing on the mechanic's left ankle.
On 11/23/19 around 4:05am an employee was loading powder in 1292 when a rock fell off of the face striking them in the left forearm. While reaching out to prime a hole a 1.5 x 2ft x 4 thick rock slid off about 6-7ft from the face striking the employee in the left forearm causing a 7cmm laceration and deep tissue contusion requiring 14 sutures.
An employee was helping load a heading in 668 lower when struck by a rock in the lower back area. The rock, approximately 10in x 8in and 1in thick, fell from the face, struck the basket of the ANFO rig, then fell striking the employee on the ground in the shoulder and back. The impact caused a small puncture wound on the employees lower back requiring 4 stitches.

tripped over valve block resulting in 3"" laceration to rt knee
EE alleges slipped and felt knee pop. Did not report until the next day. Dx. with a knee strain and referred to ortho. Placed on restrictions until that consult is completed.
Hit pot hole while riding in back of tractor and caused EE to hit back on the bar. Was not placed on restricted duty until 10/21/2016 because of continued complaints of pain.
Tripped when dismounting and turned ankle resulting in a sprain.
Putting hangars on finger for chute repair. 8 ft piece of metal square stocks cut index finger on sharp edging at top. Heavier than expected. Stitches / possible hairline crack in tip of index finger bone.

When climbing into cab of truck felt pull in abdomen.
Rock fell from back hitting ee on the rt arm right above elbow.
A loader operator was cleaning up the mill hole when he heard a loud noise and had dust and smoke come up around the loader. The supervisor was notified and while coming to investigate noted that the smoke and smell was consistent with blasting fumes.
Stood up from squatting position, slipped and fell backwards, lacerating rt elbow either on rig or rock.
Felt burning and stinging in abdomen while lifting a heavy pump
EE was taking the kelly rod off. Due to the drill mast being rolled to one side, the kelly rod fell off the rear mast roller, smashing his middle finger on his right hand between the kelly rod and the side of the mast. Even though gloves were used it still split his finger on the inside first knuckle. First aid was given. Hand was washed and bandaged.
Hit pothole while driving truck causing teeth to snap together. Chipped #8 tooth. Saw dentist on 7/9/14 and a cap was recommended for tooth

Employees was removing loose finger from the raise. The finger had dust on it and slipped out of his hand mashing left middle finger. This area is a limited space area and does not provide a means of utilizing additional hoisting equipment, meaning the process of this repair requires manual handling.

The Coy Hoist lost power, the General Mine Foreman was notified and evacuation began. The electrical department was called in to restore power while the evacuation was in process at the Hicks Shaft, the power was restored at 5:01 P.M. evacuation was stopped and work was resumed.
Cut finger, sent to orthopedic Dr who did surgery and place on restricted duty which we just got the medical for. He was placed on restricted duty.
At approx. 6:00 A.M. on November 6, 2011 there was a hoist malfunction at the Coy mine, MSHA hot line was called and the incident was reported. Evacuation of the mine commenced through the Hicks Shaft, the problem was found and repaired. Employees were allowed to renter the mine and proceed to work by 9:00 A.M.
Crews were evacuated through the Hicks Shaft due to a contactor being down on the hoist. The problem was found and repaired at 8:15 am. MSHA was contacted.
The Man hoist was taken out of service to perform unscheduled repairs to a cracked brake shoe. All miners were evacuated out of the mine prior to taking the hoist out of service no emergency evacuation needed, MSHA was contacted as a courtesy, to report the repair, the OEM was brought in to make the repairs.
A power outage caused an evacuation of the Coy Mine, the cause of the outage was a thunder storm. MSHA was notified and the mine was inspected prior to returning to work.

Injured was operating a mechanical scaler. He stated while sitting on the equipment, that he noticed a tingling in his left foot. As he continued working the pain progressed into a burning sensation. Injured has past neck problems & was scheduled for surgery in Dec. (non work related) He has had a fussion done previously for a similar type occurance. More medical info will follow.
On 9/24/10 injured slipped, stating he pulled a muscle. He was transported to E/R, x-rays were neg., released to return to work. On 9/28/10 injured returned to Co. Dr. for follow up and was placed on restricted duty.
While scaling the 668 Drift, he was double checking area sounding roof, he saw a piece of rock that looked loose, when he struck the rock with his bar, rock fell to the ground, he stated the rock hit him in the arm and on the foot causing a contus. to his foot. He cont. After shift went to e/r and released then followed up with co. DR. on 9-28-10 and was placed on rest. duty.
The liner on the skip door had come loose fouling the door. The men were evacuated from the mine and repairs were made, men were allowed to reenter the mine after all working places were examined and cleared. M.S.H.A. was notified on the hot line at approximetly 7:00 a.m. to notify them of the problem.
Electrical power to the mine was interrupted at 4:58 A.M. There were 12 men working underground, the evacuation notice was communicated and employees assembled at the Hick's Road air shaft for removal. The electrical issue was checked out (breaker at the main substation) and employees given permission to return to work after all corrective actions were completed at 5:50 A.M.
The hoist went down at 8:35 AM 23 men were under ground, the hotline was called at 8:53am. The men assembled at the escape shaft, the escape hoist was activated and escape hoist lowered to retrieve men. The hoist was repaired by 9:34 am and the crew returned to their work stations.
Employee was walking and slipped on the shop floor, he ceased work and went to Hospital
While moving a water hose out of a heading, employee stated he had a pain in his left calf. He worked 3 more shifts then request to seek medical attention. He was transported to the Dr. and was diagnosed with a ruptured calf. On 7-13-10 he was placed on restricted duty.
Injured was washing down the skip pocket using a water hose. He lost control of the hose and it hit him in the face, causing a laceration and a fracture to the nose.
Injured was getting a tool when a piece of metal fell striking him on the finger.
The main hoist park brake set and would not release after loading a skip. Initial inspection indicated the problem was mechanical in nature and the mine was ordered to begin evacuation procedures.

A power outage occurred thereby diabling the hoist. The mine was evacuated due to the lack of two escapeways.
Employee was helping clear dump chute. Rock came loose, mashing right thumb. Partial amputation to first joint.
Employee was helping pinch grizzley beam in place and beam rolled, mashing finger, left hand, index finger. Required 14 stitches.
EE was in a basket about to install a new airline when some ground came down on him. He had hand scaled the area prior to beginning work. EE received stitches to the elbow and bruised ribs.
EE was walking across powder magazine. On one of his steps his right ankle rolled and he heard a pop. There were no tripping hazards. EE suffered a distal fibula fracture.

Hoist failed to operate and electricians were called to investigate. 'Park brake' coupling had failed setting the brake. All personnel were withdrawn. MSHA was notified. Coupling was repaired and hoist placed back in service 12/23/08.
Power was lost to shaft and hoisting equipment due to an area wide power outage. The employee's were gathered in one area until power was restored around 12:00 PM.
While cage was taking crew down shaft a loose handrail clipped the bottom of the cage door. Door pulled off of its tracks toward shaft, hit next landing and forced door into cage striking 3 EE's. 3 EE's recieved first aid only. After getting employees off cage it was down for more than 30 minutes to investigate.
EE was driving a haul truck when he ran over a rock in the road that caused truck to bounce, EE hit head and shoulder on canopy post. EE did not seek medical treatment until 12/04/08 at which time he received no restrictions but was sent for physical therapy.
Power was lost to all of mine (surface and underground) rendering hoist unusable. Evacuated employees out escape shaft. Electric company restored power to site at 7:45 AM.
Employee was loosening bolt with wrench. Dislocated shoulder
East skip door would not close after loading skip to haul muck. It was discovered that the kicker rail had broken loose and become hung in skip. This caused the hoist to be unusable for more than 30 minutes.
EE was using ANFO hose to charge a lifter hole against the rib. The hose plugged up. When it dislodged, it blew back, causing EE's hand to hit rib and lacerate his knuckle on pinky and ring fingers. 6 stitches were required.
EE was moving loader with door open. Door swung shut on EE's thumb breaking it.
