Over-exertion in pulling or pushing objects

Employee was changing an air line out on a water truck when EE heard a pop and felt a tear in the left forearm.

Struck by flying object

Employee was using water hose to wash tail pulley and during this task it splashed back at them and got in an eye causing a scratch. Employee was wearing safety glasses.

Employee was exhibiting symptoms of a heart attack. It was later determined by a medical professional the symptoms were non-work stress related.

Struck by falling object

Injured ee was changing 4 hydraulic hoses on a loader. While removing the plate below the air filter cleaner, the plate became dislodged. With a pry bar the employee worked to get the plate loose when it finally dislodged the access door dropped onto the employee's hand. He pulled his hand back out of reaction causing a laceration to his left thumb which resulted in stitches

Injured employee was trying to reposition the suction hose on water pump. While the injured employee was pushing other employee was pulling. The hose went through the upright and the hose sprang back and caught his finger between the foot valve and the upright of boom truck. Resulting in a fracture right index finger.

The employee was driving a truck to measure a conveyor belt. He looked up at the belt and struck a concrete block. The injury resulted in a laceration to the forehead requiring stitches.

The ee was operating a haul truck and stated the seat bottomed out an djarred his back. Incident was not reported until 1/9/06 and medical attention was rendered on 1/10/06.

Employee had periodic screening chest x-ray read by two readers. X-ray was characterized as pneumoconiosis with scores of 1/1 and 1/0. Damage or impairment cannot be determined by x-rays.

THE EE WAS RUNNING A DOZER AND WHEN HE EXITED THE CAB AND STEPPED ONTO TRACK, HE STATED HIS LEG POPPED.

THE EE WAS USING A HAMMER TO CLEAR CLOG FROM CHUTE IN SURGE TUNNEL. WHEN CLOG WAS FREED, HIS LEFT HAND WAS CAUGHT BETWEEN A PIECE OF ANGLE IRON 7 FREED MATERIAL. THIS RESULTED IN STITCHES TO M IDDLE & INDEX FINGERS.
