Struck against a moving object

Employee was riding in the buddy seat of an articulating truck when the truck hit a rock, jarring the employee's back.

Miner was rolling crusher rotor when left hand pinky and ring finger got caught between crusher rotor and side wall of crusher housing.

On April 12, 2021, employee reported back was sore from helping change crusher liners on April 10, 2021. First Aid method RICE (rest, ice, compress, elevation) was initiated at that time.

Employee was loading roller racks from pallet into pick-up truck. As they turned to step off pallet a foot got caught on a pallet band. Employee fell and hit right arm on the roller rack.

A customer truck driver exited his truck at the scale house window to retrieve his ticket when he fell to the ground gasping for air. 911 was called and CPR performed until EMS arrived. Later died of full cardiac arrest at the emergency room.

Struck by falling object

Employee was replacing seals in tilt cly. Pin of the 990 loader slid off the frame and landed on employee's right edge of foot. Causing a fracture in the right little toe.

Two employees were installing a new idler rack on C1 conveyor, the rack was already positioned onto the conveyor when the two EE's attempted to upright the rack to its final position. The injured EE strained his back muscle at this time.

Employee was loading haul truck with shot rock. As employee returned to get second bucket of rock, a boulder rolled off the pile and struck the bucket of the loader. Employee was jarred forward and seat belt caused him to be pushed back into his seat.

EE was re installing the seat on a D8 dozer. He was lining up the seat and it slipped from the support position and pulled his left shoulder resulting in a strained rotator cuff.

THE EE WAS REMOVING THE ROOF FROM A GENERATOR HOUSING. AS HE REMOVED THE LAST BOLT, TENSION CAUSED THE ROOF SUPPORT TO SPRING UP & HIT HIM IN THE RT. SHOULDER.

Fall from machine

THE INJURED EE WAS TIGHTENING HOUSING BOLTS ON TOP OF THE PRIMARY CRUSHED. HE WAS USING IS FOOT TO PUSH THE WRENCH AND HIS FOOT SLIPPED OFF THE WRENCH AND THE OTHER FOOT SLIPPED OFF THE FRAM. HE FALL 5' & HIT THE DRIVE WHEEL AND THEN FELL 4 1/2' MOR & HIT CATWALK. AS HE FELL HIS LEFT ANKLE HIT THE WHEEL RSULTING IN A FRACTURE, ALSO POSSIBLE HAIRLINE FRACTURE TO RIGHT KNEE.
