Employee stepped on the bottom end of a frozen berm behind the shop causing EE to fall injuring EE's right leg.

Injured employee was assisting another employee move a metal plate. In the process of moving the plate, the injured employee lost his grip and the plate fell and struck him on top of the left foot directly behind the safety toe protection of his boot.

Employee was cleaning around tail pulley under plant and a rock fell and struck him on his lower back. Employee was sent for medical treatment that consisted of prescription medication for bruising and was released to work to perform normal job duties.
