Employee was assisting repair/replace toggle plate and toggle plate bearings on the jaw crusher when they suffered a pinning type injury.

Struck against a moving object

Employee was in a haul truck when a rock fell and jarred the truck. Employee complained of back pain and utilized the RICE treatment until the January 24th. Up until the 24th, the employee stated multiple times they were feeling better each day. Employee went to ER with back pain and was released with prescriptions.

Over-exertion in lifting objects

Employee was assisting loading VSI hammers from a pallet to a loader bucket. Employee moved 1 hammer (118 lbs.) and complained of back and leg pain.

Fall onto or against objects

Employee slipped descending a dozer and fell on the tilt cylinder, breaking two ribs. EE had surgery on 4/11/2025 due to ribs not healing properly. Was released to full duty on 8/5/2025.

Employee had arrived at work and pulled into the scale house parking lot and was exiting EE's vehicle to throw away personal trash from home when EE fell exiting EE's vehicle.

Service technician was using our weight cart to move weights to calibrate their truck scale. The cart flipped and the service technician hit his head on the weights that were in the cart causing a gash on his forehead requiring nine stitches.

Injured employee was assisting with the replacement of a 42' conveyor belt. The conveyor belt slipped, striking employee in the ribs. The results in this action of the conveyor belt caused the employee to be knocked over the handrails of the cat walk he was standing on.

Working in the cab of a D9R, when employee opened the door, the wind blew dirt into his eye.

Injured employee was using a chain wrench to correct brushing alignment. His right foot got caught between the wrench and drill tower which resulted in a contusion and laceration.

Injured employee was stepping backwards while directing the loader to pick up a pallet, and felt his right knee pop. Employee was not carrying anything, did not trip on anything or was not on uneven ground. Received medical care on 12/06/07 and put on restriction starting 12/06/07.

Struck against stationary object

The injured employee was troubleshooting the drill. As he accessed the area, his left ring finger came in contact with the pop off valve of air compressor resulting in cut.

The employee was using a bar to dislodge material that was packed in a chute. He was pulling back on the bar and mashed his left thumb between the bar and the handrail on the catwalk. The injury resulted in 6 stitches.
