Injured employee climbed into the back of his service truck to get a ladder. Getting out of the back up of the truck he misplaced his footing, causing him to lose his grip causing him to fall and land on his hip.

The injured employee was cleaning up spilled material on the concrete slope and pit of the conveyor area. The employee was injured when the shovel he was using accidentally struck him in the chin while he was performing his task. The occurrence caused a laceration to his chin resulting in nine stitches.

Injured EE was closing door on grader and had right hand in door jam on hinged side. EE pulled the door closed with left hand and mashed his right index finger.

TRE injured employee was holding a chain for the mechanic using a punch and hammer to drive out pins in chain link. A piece of metal broke off the chain pin and went into employee left forearm.

At the time of the injury, the employee was removing a pump discharge hose from the tooth on an excavator bucket when the boom of the excavator accidentally moved striking the employee's right foot. The injury resulted in a fracture to his foot.

THE INJURED EMPLOYEE WAS PULLING ON A TORCH HOSE THAT HAD BECOME TANGLED WITH WELDING LEADS AND GOT OFF BALANCE AND FELL TO THE GROUND, RESULTING IN A SPRAINED WRIST.

THE EMPLOYEE WAS PUMPING OUT THE CONTAINMENT AREA AND THE PUMP CAME LOOSE FROM THE STAND. HE TRIED TO CATCH THE PUMP & HIS RIGHT RING FINGER WAS CAUGHT BETWEEN THE PUMP AND CONCRETE FLOOR. IT RESULTED IN A LACERATION REQUIRING STITCHES.

WHILE ASSISTING LOADING SHOT AT VULCAN CHATTANOOGA, EE FELT A PAIN IN HIS STOMACH. IT WAS INITIALLY ASSUMED TO BE INDIGESTION AND APPROPRIATE MEDICATION TAKEN. THE PAIN WORSENED DURING THE NIGHT AND HE WENT TO THE ER FOR TREATMENT. IT WAS DETERMINED THAT HE HAD A PULLED STOMACH MUSCLE.

THE EE WAS WORKING ON THE #6 CONVEYOR FEEDER BELT FOR THE #3 CRUSHER. CLAMPS WERE ON THE BELT & HE WAS USING A SLEDGE HAMMER TO LOOSEN THEM, & THE HAMMER SLIPPED & HIT HIS LEFT INDEX FINGER.

THE EMPLOYEE HAD COLLECTED A SAMPLE FROM THE HEAD PULLEY AND WAS DESCENDING THE CATWALK. HIS FOOT SLIPPED AND HE STRUCK HIS LEFT LEG ON THE CATWALK RESULTING IN A LACERATION.

THE INJURED EMPLOYEE WAS IN THE PROCESS OF PERFORMING MAINTENANCE ON THE #1 BELT LINE WHEN THE INJURY OCCURRED. HIS RIGHT HAND WAS CAUGHT BETWEEN TWO PIECES OF METAL CAUSING THE INJURY.

THE EMPLOYEE WAS BEHIND SHOP DOING CLEAN UP WORK AND STEPPED ON A ROCK AND FELL. TRYING TO BREAK FALL, HE TWISTED HIS LEFT WRIST.
