Employee was trimming the bottom tab off a poly-deck screen so it would fit flush against the side of the screen. EE was utilizing a retractable razor knife and as EE cut away from self the blade slipped off catching the tip of EE's left ring finger.

The employee in reference has allegedly stated that his lower back & right leg has been injured while working at the facility crusher run tunnel area. The employee was operating a D8-R cat dozer in & around the crusher run tunnel stockpile area at the time of the incident. The employee allegedly has stopped the dozer & stepped down onto the stockpile area to relieve his self.

The injured EE was changing a screen at the plant and was sliding the replacement screen over another screen to get it into place. There was slack in the come-a-long and his right middle finger was caught between the screens requiring stitches.

THE EMPLOYEE WAS PULLING ELECTRICAL WIRE FROM POLE TO POLE AND SLIPPED ON UNLEVEL GROUND. HE TRIED TO BRACE HIMSELF FROM THE FALL AND PULLED A MUSCLE IN HIS NECK.

THE EE WAS WORKING ON THE CONE CRUSHER CONTROLS & UPON COMPLETION HE CLOSED THE DOOR WITH HIS FOOT. WHILE PUSHING HE FELT A POP & PAIN IN HIS RT. FOOT. IT RESULTED IN A CRACKED BONE AT LITTLE TOE ON RT. FOOT.
