While mounting drill, employee alleges they lost footing, slipping but catching themselves and felt pain in their back.

Struck against stationary object

Injured employee was assisting on the repairs of the # 6 conveyor. The conveyor belt had broken & was lying on the ground at the take-up pulley area. The injured was in the process of hooking a chain to the gravity take-up & conveyor belt when the belt started to shift and he moved back striking his left arm on piece of metal angle brace causing a deep laceration

Over-exertion in lifting objects

Injured employee along with another employee were installing a chute at the secondary crusher, they had installed a 20' x 30' of 3/8th plated steel in the chute. Once the plate was in place the injured employee had to make a minor adjustment and was repositioning the plate when he felt a sharp pain in his left arm. Resulting in a strain to the bicep muscle

The ee was troubleshooting the electrical switch gear and the door was open on the box. He turned his head and as he looked back around, the wind blew the door into his mouth, loosening an implanted tooth.

THE EMPLOYEE WAS DESCENDING A TRUCK MOUNTED LADDER WHEN HE SLIPPED SPRAINING HIS LEFT KNEE AND ANKLE.
