Caught in, under or between a moving and a stationary object

Utility operator was preparing to wash out the rail scale. The loader operator was weighing the loader on the scale. The loader operator announced on the radio that he was moving the loader off the scale. He slowly moved forward when he observed the utility operator on the ground. The utility operator apparently fell and was crushed by the front left tire of the loader.

Bobcat operator struck a fixed object. Reported to company on 5/29 when occurred on 5/5.

Fall to the walkway or working surface

While walking across a snow-covered, icy parking lot at the mine, an employee was taking EE's time and shuffling EE's feet due to the slippery conditions. Unfortunately, EE lost EE's footing and fell, landing heavily on EE's left elbow.

Struck by flying object

EE had a 1-1/4"" hose in a bumper mounted vice removing a quick disconnect. The hose was not connected to any equipment. It had residual pressure though & once the QD was at the end of its threads it shot off the hose & struck EE in the mouth splitting EE's lower lip, damaging upper tooth & breaking 2 lower teeth on EE's side.

Employee was assisting cutting plastic ANFO loading hose to length to replace worn hoses on powder truck. While cutting hose, employee-trainee lost control of knife being used to cut pipe, which caused knife to close onto employee's right middle finger, lacerating the skin at/around the first knuckle.

Maintenance employee was putting a belt on the pulley moving it forward to align it with both hands. The tip of the leather glove initially got caught between the belt and pulley, the forward motion caused fingertip to be pulled in approximately 1"". The left index fingertip had the fingernail ripped partially required 4 stitches.

An operator was exiting the railcar mover via the fixed ladder. When stepping from the bottom step to the ground roughly 12 inches below, the employee felt a pop in knee. Ground conditions were favorable with no mud or loose material.

Fall from machine

Operator began to maneuver drill from the low-boy. The spotter signaled operator to stop, operator engaged the parking brake, unbuckled seat belt, stood up to access the situation. At this point the cribbing rolled, shifting the front tire off the trailer. At this same time the operator exited the cab and sustained an injury to heel.

Employee was cleaning the windows of the front door to the main office. When the employee was coming back into the building the employee grabbed the side of the door when a wind gust suddenly blew the exterior door which swings outward shut, pinching finger and causing the skin to be removed from the tip of the right hand ring finger.

Employee was attempting to attach a sign to a red cone. As employee was putting a hole in the cone the blade slipped and the knife closed on employee's right index finger.

The employee was working inside the 2nd Floor MCC Electrical Room running conduit for new lighting. The employee committed an unsafe act by choosing to ride the trolley box to gain access to the ladder across the room. The employee fell approximately 20 feet landing on back side. The JSA stated employee should have completed the work from the ladder.

While replacing a section of conveyor belting, the belt became stuck on the conveyor structure. The injured employee reached into the structure to free the belt and it pinched left ring finger against the belt frame.

Miner was attempting to re-position the forks on a bobcat. The fork bounced and when it did his right hand got pinched between the fork and the support behind the fork causing a laceration which required stitches at the local medical facility.

Accident type, without injuries

At 0736 08 March 2013 the Interim Mine Manager checked the shot that the 3rd shift supervisor had conveyed interest in the mine manager reviewing and noticed some bolts on the ground that had come down with the shot. It was later determined that this was not reportable by the mine however the field office supervisor stated that he thinks it is and directed us to submit a 7000-1.

EE alleges a chipped tooth while connecting an air line on the rail car. EE went to the dentist 12/05/11. The company accepted charges for the dentist visit.

Fall from machine

Employee decided to step off the end of the crawlers to the ground when he right heel momentarily caught. This momentary catch caused him to loose his balance and fall to the ground with his right leg bent under his body.

EE was exiting small elevated ramp when his foot slipped due to rain, his knee popped tearing his ACL in his right knee.

Working on screw conveyor. He lost his balance and fell back on his out stretched arm to catch himself. He felt a slight burning pain in his right shoulder.

EE, leadman maintence, smashed the end of his left index finger while replacing the bolts in 66"" rollec mill journal. Seven stitches was required.

Employee came around a curve in a loader, miss-judged distance and struck pillar.

A claim for short term disability disclosed that a fall at work several months earlier resulted in knee surgery. There is no record that the injury was reported as required.

Employee was installing bucket elevator's equipment when the bar he was holding slipped causing the bucket to jerk his arm & shoulder. *Note: Employee was released to return to work with restr ictions. He never returned to work and therefore was terminated.

HIS FAILURE TO PROPERLY POSITION HIE FEET WHILE TURNING THE DRUM CAUSED HIS FOOT TO SLIDE BETWEEN THE DRUM AND THE CRUSHER WALL.
