After EE repaired a loose guard on a dozer, as EE was preparing to enter the cab while still on the tracks, EE slipped and fell injuring their left shoulder.

Employee was working on a screen tower when dust was blown into left eye during strong winds. Employee was diagnosed with a scratch on eye and an ulcer due to the inflammation.

Employee removed plastic trim to try and fix an intermittently working windshield wiper on a dozer. In removing the plastic, a section broke off. Employee was going to set the piece outside of the dozer, and while exiting the machinery EE hit the dashboard with the trim and the sharp edge penetrated through EE's shirt and stuck EE in EE's right arm just below EE's bicep.

Employee was changing a rear tire on a water truck. The area was wet. When the employee used a lifting bar to move the tire the bar slipped on the wet ground causing the tire to fall on the employee's foot.

EE was operating D8N dozer on stripping dump area, pushing stripping material. While leveling dump area he encountered a slab of rock est. to be 6'x8'x15"". His plan was to remove it from topside of dump. He positioned dozer at or about 45 deg angle to the top of dump site. Dozer traveled over berm & traveled a distance down the slope then slid to left side, came to rest at bottom
