Employee closed the door to the primary switch house on EE's thumb.

EE slipped and broke their fall with right arm while repairing a feeder pan. EE had surgery for torn rotator cuff on 4/29.

Struck by flying object

Employee was removing toad stool from parts crusher. Employee struck an easy-out to remove a wedged bolt from a socket. A shard of metal came off the punch at impact and entered the employees left hand through the glove.

Struck by falling object

Injured employee was removing guard to remove a return roller. As the employee removed the nuts holding the guard he turned to the right to set down the impact gun and nuts and the guard fell to the ground striking employee in his left arm.

Injured employee was adjusting water on a cone crusher and stepped on side of concrete pad that supports the bottom of steps with right foot, resulting in a sprain.

Employee was cutting plastic tie wrap with pocket knife when the knife slipped and cut his left hand on the side of his forefinger. Cut required sutures and employee was able to return to work with no restrictions.

Struck by flying object

Employee was using high pressure water hose for clean up when an unknown object flew up and struck him on his cheek bone just under the right eye that caused a small cut. Employee was taken for medical observation and recieved 3 stitches and a tetnus shot. Employee returned to work with no restrictions.

Struck against stationary object

Employee walking across yard and took his safety glasses off to clean them while still walking and walked into a set of detachable pallet forks that were lying on the ground. Employee's left leg struck the forks in the shin area and caused a laceration that required sutures. He was able to return to work with no restrictions.

Employee strained back while changing out return roller on conveyor belt.

Fall to the walkway or working surface

Employee was cleaning up under a conveyor with a water hose. When water was turned on his footing was not sufficient to hold the pressure and caused him to slip and fall. DELAYED MEDICAL TREATMENT.
