Struck by... (Not Elsewhere Classified)

EE was using a 1/2-ton chain hoist to loosen a 24""x36"" screen panel from the center deck of screen deck #1. EE raised the corner of the panel a couple inches, while releasing the tension to reposition it, EE said lost their balance and EE's right-hand slid toward the opening and the panel came back down on EE's right thumb, splitting it open.

Caught in, under or between a moving and a stationary object

The injured employee was adjusting the belt scraper on C-6 conveyor. The belt was still in motion as EE attempted to adjust the scraper and this caused the spud wrench to forcefully be spun back causing it to contact EE's finger against a rail. The employee was taken to the ER and given stitches and a splint.

Technician inspecting mobile crane and unaware of boom position in regards to positioning of technician's hand. Boom was retracted and sliding section of boom injured technician's hand. Cuts to fingers requiring stiches and fractured bone in right ring finger requiring 2 pins.

Employee was performing maintenance on a heater fan & motor. The heater started to tip over and he reached to grab it and his left little finger was caught between the blade & the sheet metal housing and sliced the finger requiring 5 stitches.

On Thursday 4/27/06 employee was taken to doctor and it was determined that he had a sprain of his left ankle. He stated his left foot slipped off a rock and turned his ankle.

Employee was using a 5 foot pry bar to pry the rocks loose from the chute when a rock came down and hit his finger against the pry bar, fracturing his left middle finger.

Employee was cutting a bolt with a torch. Torch brushed across the employee's finger and burned through glove, causing a second degree burn to the employee's left middle finger.

Supervisor was holding a box wrench on the bolt heads of the tube assembly of a Telsmith Screen while another employee was tightening the nuts on the opposite side with a ratchet when the Supervisor's right little finger got pinched causing a laceration that required five stitches.

EE WAS INSTALLING A NEW TAIL PULLEY WHEN HIS LEFT INDEX FINGER WAS CAUGHT BETWEEN THE PULLEY SHAFT & CONVEYOR FRAME. MEDICAL ATTENTION REQUIRED STERI-STRIPS & A FINGER SPLINT. EE RETURNED TO W ORK, PER TREATING PHYSICIAN, BUT IS TO WEAR THE FINGER SPLINT FOR 7-DAYS. NO LOST TIME.

Struck by flying object

THE EE WAS CHANGING A TOOTH ON HIS LOADER. HE USED A HAMMER TO HIT THE TOOTH AND LOSING IT UP. WHEN HE HIT THE TOOTH ON THE POINT END A CHIP CAME OFF AND HIT HIM IN HIS LEFT UPPER ARM BICEP.
