Fall to the walkway or working surface

After retrieving a voided ticket from a customer truck at the ticket box, the employee started down a small gravel incline on the back side of the ticket box. The employee's left foot slid out from under them on the gravel as they were stepping forward, which caused them to trip and fall, resulting in dislocation and multiple fractures of the ankle.

Struck against a moving object

A Komatsu 605 haul truck backed into a Cat 775G haul truck. The right rear corner of the bed of the 605 contacted the left side handrail and front left cab support of the 775G. EE in the 775G was transported by ambulance to ER claiming pains in their back.

Over-exertion in lifting objects

Employee was reaching into the back of a pick up truck to remove an object, when doing so employee felt a pain in right shoulder.

On Friday June 4th, an employee felt a pop in the elbow when EE picked up a bucket of crusher backing material. After still experiencing pain on June 24, EE decided to seek medical attention, and was given work restrictions due to inflammation in EE's elbow.

Injured employee was handing a power tool to another employee when the other employee lost grip of the tool causing the tool to fall and striking the injured employee in the mouth causing a chipped tooth.

The employee was dismounting a haul truck when they stepped wrong, causing an ankle to roll. Employee was diagnosed with a closed torus fracture of distal end of right fibula.

The employee was injured when a section of pipe shifted when a track hoe caught the pipe. The pipe struck employee's leg causing a tibia plateau fracture as well as a small laceration next to the left eye which required stitches.

The employee was using a 5 foot bar to pry the stationary jaw die into place. The bar slipped off and the employee fell back onto the conveyor and struck the back of his head on the bottom of the chute.

Over-exertion in lifting objects

The employee was moving boxes of copy paper in her office and felt pain in the lower back left side. She advised her supervisor, but did not seek medical attention. On 9/1/15, the employee was still in pain so she went to the doctor and was advised she sprained her L-26 lower lumbar.

Employee was connecting a 24""x8' long concrete pipe, when he had the excavator operator raise pipe to set it in the ditch. At this time the cable twisted, causing pipe to turn. This pinned his left leg between a second pipe setting behind him.

TWO EMPLOYEES WERE ATTEMPTING TO FUEL A LOADER. ONE WAS DISMOUNTING THE LOADER BY MEANS OF THE LADDER. WHEN HE STEPPED FROM THE BOTTOM STEP TO THE GROUND LEVEL HE LOST HIS FOOTING AND FELL TO THE GROUND.

EE WAS HELPING CHANGE WEAR PLATES & TEETH ON THE LOADER BUCKET. AN IMPACT WRENCH WAS BEING USED TO LOOSEN THE NUTS. ONE BOLT WAS LOOSE & DROPPED DOWN. WITH HIS GLOVES ON HE PUSHED THE BOLT BAC K INTO ITS CRADLE TO CONTINUE TO LOOSEN. AS THE WRENCH WAS APPLIED THE BOLT DROPPED DOWN & THE HEAD SPUN AROUND LACERATING THE FINGERS ON HIS RIGHT HAND.

EE HAD UNCLOGGED THE CHUTE ABOVE THE #1 SCREEN AT THE #3 CONVEYOR. HE WAS CLIMBING OUT OF THE CHUTE & STRUCK HIS RIGHT ANKLE ON THE ANKLE ON THE CORNER OF THE V-BELT DRIVE GUARD, FOR #3 CONVEYOR.

INJURED EE & ANOTHER EE WERE REPLACING THE GUARD ON #19 CONVEYOR. INJURED EE WAS USING A CRESCENT WRENCH TO TIGHTEN THE NUT THAT HELD THE GUARD IN PLACE. THE WRENCH SLIPPED & STRUCK HIS HAND A GAINST THE EDGE OF THE GUARD RESULTING IN LACERATION TO TOP OF 2 FINGERS RIGHT HAND.

TWO EMPLOYEES WERE INSTALLING A NEW OXYGEN CYLINDER ONTO THE WELDING TRUCK. WHILE ON OPPOSITE SIDES THEY PICKED UP THE CYLINDER IN AN UPRIGHT POSITION WITH THE TOP TILTED BACK. AS THEY PLACED THE BOTTOM OF THE CYLINDER INTO THE STORAGE RACKVICTIM'S FINGER STRUCK AGAINST THE FRAME OF THE TRUCK CAUSING THE LACERATION. THE STORAGE RACK IS 22"" OFF THE GROUND AND IS 10"" WIDE.
