Accident type, without injuries

The Plant Manager was contacted by the Cross VFD stating that a fire had started in the ditch on County Line Rd in front of the quarry. The fire spread onto mine property before it was extinguished.

Struck against a moving object

The operator was sitting in an Off Road Truck being loaded when Excavator swung around and struck the catwalk with the counterweight, shaking the operator on the inside of the cab.

Struck against stationary object

Employee was loosening bolts on top of the Davie FS2-E Drill mud pump to troubleshoot a noise. Upon using an adjustable wrench, it slipped causing EE to strike EE's left hand near the pinkie knuckle. Employee was wearing Hexarmor impact/cut resistant gloves at the time of the incident. The injury occurred between the cut protection and knuckle protection area on the glove.

Struck by flying object

The EE was installing a pin on a crane. Using a hammer the EE made contact with the pin. During impact a piece of the hammer came and struck the EE in the left forearm. The piece went into the forearm.

Struck by falling object

EE was removing a section of wear plate steel with a torch. As the final cut was made the plate fell and landed on the EE's left hand dislocating three fingers and lacerating the palm.

EE was using a side grinder when it kicked back and up striking him in the forehead causing a laceration above his Rt. eye that required stitches to close.

Technician was attempting to place a portable hydraulic press, weight approx. 100 lbs., onto wood crib blocks. Hydraulic press fell off of wood crib blocks and fell. Techician tried to hold onto press and sprained left shoulder.

EE WAS USING HANDTOOLS, PERFORMING MAINTENANCE IN PLANT ALL DAY. HE NOTICED SOME PAIN THAT NIGHT, AND TREATED IT AT HOME. A FEW DAYS LATER, HE REPORTED THE INCIDENT, AND WAS SENT TO THE DR. XR AYS REVEALED A FRACTURE OF ONE OF THE BONES IN THE WRIST, BUT IT COULD NOT BE DETERMINED WHEN THE FRACTURE HAD OCCURRED. THE EE SAID HE HAD FRACTURED HIS WRIST IN A MOTORCYCLE ACCIDENT YEARS

EMPLOYEE WAS HELPING STEM BLAST HOLES, MOVING ROCK IN 5 GALLON BUCKETS. AS HE LIFTED A BUCKET, HE FELT A PAIN IN HIS LOWER BACK. HE WAS SENT TO A DOCTOR AND DIAGNOSED WITH BACK STRAIN. HE CONT INUED HIS NORMAL DUTIES. IN MARCH 2002 HE RETURNED TO THE DOCTOR AND WAS DIAGNOSED WITH A HERNIATED DISC.

INJURED AND OTHER EMPLOYEE WERE LOOKING THROUGH THE SCRAP YARD FOR SPARE CONVEYER PARTS. INJURED STEPPED ON A NAIL PROTRUDING FORM A PIECE OF WOOD, PUNCTURING HIS FOOT. FIRST AID TREATMENT WA S PROVIDED AT THAT TIME. ON OCT 8 HE WAS ADMITTED TO THE HOSPITAL WITH AN INFECTION IN HIS FOOT.

STEPPED OFF BACKHOE FOR AN ONSITE MEETING. ABOUT A HOUR LATER, RETURNED TO EQUIPMENT AND KNEE STARTED HURTING.
