Employee was tightening bolts on the rock hammer located at the primary crusher. He was using a 16 lbs hammer and a knock wrench to tighten bolts strained right forearm and elbow. He was placed on restrictive duty on 5/28/08 no lifting over 5 lbs.

REMOVING A CHUTE THAT FEEDS STONE TO THE CRUSHER. CRANE WAS HOOKED TO CHUTE AND BOLTS WERE CUT WITH A TORCH. FRONT SECTION OF CHUTE DROPPED DOWN APPROX. 2' AND BACK SECTION BECAME LODGED. CHU TE BROKE FREE AND FRONT SECTION SHOT UP APPROX. 2' CATCHING EE'S FINGER BETWEEN THE CHUTE AND A 12""I-BEAM.
