The employee was operating a Genie S-60 Manlift. While navigating a downgrade, employee drove over a small pile of rocks causing the manlift to shift, jolting the operator. When the operator shifted, they planted their leg causing a hairline fracture.

Employee was removing a two piece v-belt guard. The top piece of the guard was removed, one of the bolts out of the underside of the lower piece of guard slipped down and hit his right arm just above the wrist causing a laceration. He received four sutures making this a reportable medical.

Employee was washing down the walkway around the #1 screen when the feed chute stopped up causing some rock to spill out. A small one hit him on the arm, then as he turned to move out of the way a bigger rock hit the back of his hard hat penetrating it and causing a small laceration on the back of his head that required three stitches.

FOREMAN WAS HELPING UNLOAD TWO JAW DIES WHEN THE CHAINS SLIPPED OFF ABOUT ONE FOOT OFF THE GROUND. THE FOREMAN PUT HIS HANDS UP TO BLOCK THE CHAINS AS THEY CAME TOWARDS HIM. THE CHAIN HIT HIS LEFT LITTLE FINGER LACERATING THE BOTTOM SIDE REQUIRING NINE STITCHES. NO FRACTURE WAS DETECTED.
EMPLOYEE WAS CHANGING OXYGEN & ACETELYNE BOTTLES WHEN HE PUSHED HIS RIGHT MIDDLE FINGERNAIL UP. HE HAD HIS RIGHT HAND AROUND THE BOTTLE SLIDING IT ON THE TRUCK WHEN HIS FINGER CAME INTO CONTACT WITH A PIECE OF METAL AT THE BACK OF THE BOTTLE RACK. STITCHES WERE USED MAKING THIS A REPORTABLE MEDICAL.

EMPLOYEE WAS PUNCHING A HOLE IN A CONVEYER BELT WITH A PUNCH AND HAMMER, WHEN HE HIT THE BACK OF HIS LEFT HAND WITH A HAMMER CAUSING INJURY WHICH REQUIRED 14 STITCHES.
