Arsonist suspected of setting fire that expanded onto quarry property. 911 called. Fire was confirmed to be on quarry property and 800 number was called. Local responding agencies contained and extinguished the fire. They confirmed they were seeking an arson suspect.

Arsonist suspected of setting fire that expanded onto quarry property. Quarry employees did not know about fire until fire department arrived and asked to access back of property. Once fire was discovered on the property, 800 number called. Local responding agencies contained and extinguished the fire. They confirmed they were seeking an arson suspect.

Employee was remounting the wheel chocks on the 980G Loader when it slipped smashing employee's finger.

Employee was walking back to personal vehicle to get a flashlight to perform pre shift inspection on mobile equipment and tripped over a rock that was submerged.

On December 10, 2019, employee slipped and struck EE's shin on a toe board resulting in an abrasion. EE received first aid treatment by a physician and released to return to work with no restrictions. On January 6, 2020, EE went back to doctor for a follow up visit where EE received medical treatment for an infection and placed on restricted duty.

Employee was loading a hydraulic hammer in the bed of the service truck. The cable came off the sheave near the hook and employee used pry bar to put the cable back on. Employee suffered a back strain.

The injured employee was walking across the railroad tracks towards the employee building and began walking up a small hill when they stepped on uneven ground and rolled their ankle. The employee felt a pop in their left ankle and had swelling.

Employee was assisting in the removal of the spider cover plate from the gyrator crusher. When the plate was lifted, it began to swing and the employee attempted to stop it from hitting the bulk oil tank. His hand was pinched against the oil tank.

AFTER COMPLETING THE PRE-SHIFT INSPECTION OF HAUL TRUCK, EE REMOVED THE WHEEL CHOCK AND WAS THROWING IT UP ONTO THE CAB WALKWAY OF THE TRUCK WHEN EE FELT A PAIN IN LOWER BACK. EE WAS DIAGNOSED WITH A MUSCLE STRAIN AND RETURNED TO WORK

EE WAS OPERATING THE WATER TRUCK IN THE PIT. EE CLAIMED THAT THE WATER VALVE CONTROL BOX BROKE LOOSE & FELL ON THE FLOOR. AS HE ATTEMPTED TO MOVE THE BOX, HE TOOK HIS EYES OFF THE ROAD. THE TRUCK VEERED SHARPLY TO THE LEFT, AND STRUCK THE BERM. THE TRUCK PROCEEDED THROUGH THE BERM AND OVER THE WALL, APPROX. 20', COMING TO REST UPSIDE DOWN.

EMPLOYEE WAS USING A CHAIN HOIST IN THE PLANT, AS HE WAS ATTEMPTING TO SET THE HOIST IN PLACE, IT SLIPPED OUT OF HIS HAND AND FELL ON HIS RIGHT FOOT. IT STRUCK THE FOOT JUST BEHIND THE STEEL TOE, CAUSING A HAIRLINE FRACTURE OF THE 5TH TOE.

TWO EMPLOYEES WERE ATTEMPTING TO ADJUST THE FORKS ON THE FORKLIFT. AS THEY WERE POSITIONING ONE OF THE FORKS, INJURED'S FINGER GOT CAUGHT BETWEEN THE FORK AND THE SUPPORT FRAME.

EE WAS WALKING FROM THE EMPLOYEE BUILDING TO THESHOP. AS HE PASSED THE LOADOUT CONTROL BUILDING HE SAID HE STEPPED IN A HOLE AND FELT A PAIN IN HIS RIGHT LEG. PHYSICIAN'S EXAM REVEALED AN INJU RY TO THE TENDON IN HIS FOOT.

THIS DRIVER WAS ATTACHING THE FUNNEL TUBE TO THEDISCHARGE BOOM FOR LOADING. THE FUNNEL TUBE FELLAND JERKED HIS ARM.

WHILE ASSISTING OTHER EMPLOYEES MOVING A PUMP IN THE PIT, HE WAS OVERCOME BY HEAT. AIR TEMPS AT THE TIME OF THE INCIDENT WERE IN THE HIGH 90'S. AT THE ONSET OF SYMPTOMS, HE WA SREMOVED TO A COOL LOCATION AMD FIRST AID WAS PROVIDED.
