Caught in, under or between a moving and a stationary object

The employee was adjusting the crusher when the toggle block didn't seat, ee turned the fly wheel with the v-belt and fingers got caught in between the v-belt and sheave causing a fracture to the right hand.

Employee heard a pop and felt a pain in his right shoulder while tightening bolts on a loader.

Employee was digging a ramp shot out using a front end loader. He reported the digging to be rough and it caused him to bounce around inside the cab. He said he felt soreness in his neck and shoulder all week and on Monday he thought he needed to see a doctor.

EE WAS CHANGING SCREEN CLOTH IN BOTTOM DECK OF WASH SCREEN. AFTER CHANGING SCREEN CLOTH, EE WAS LYING ON HIS BACK PULLING HIMSELF OUT OF SCREEN. AS EE PULLED HIS UPPER BODY OUT OF SCREEN, HE FELT PAIN IN RT. SHOULDER.
