Fall from scaffolds, walkways, platforms

Employee was helping three other employees install a pump and motor onto a new pump platform that had been installed on the settling pond. The eyes that attached the walkway to the pump had been installed wrong. When the pin was removed the walkway twisted to the left, broke loose and caused the walkway to overturn. Handrail struck employee and knocked him into the pond.

Changing out a damaged roller on the conveyor when the roller fell and caught his finger between the roller and the concrete floor.

Employee was getting down from working on loader bucket and suffered a laceration to his left hand.

Over-exertion (Not Elsewhere Classified)

While dismounting the machine the mechanic turned his body while his foot was planted and twisted his left knee. He could not bear weight on his left leg and the knee began to swell immediately. He was taken to the medical facility for examination and tests are being performed at this time to determine the extent of the injury.

Struck by falling object

Boom truck was lifting pieces of zinc from inside the 54 crusher stat.and placing them into metal containers for recycling.The injured EE was guiding the pieces into the metal contain.when it bumped the side of the cont. and several pieces fell out striking the EE's left hand. EE suffered a small laceration on his left palm req. 4 stitches.

Fall to the walkway or working surface

EE was going to perform a LA abrasion test on a stone sample. He was cleaning out the machine from recent repairs. He dumped welding slag from inside the machine into a catch pan under the machines. He emptied the pan, was replacing it under machine, steel sphere was missing. EE bend down to get the sphere and lost his balance and fell face 1st onto the concrete floor.

Struck by flying object

Employee was removing a piece of tramp metal from the Nordberg 7' standard crusher. He was inside the crusher using a burning bar to remove metal. When metal gave way, a piece flew out and struck employee on the right hand and under his chin.

Struck against stationary object

Employee was apraying round-up on weeds in the storage area; he stepped up onto the weather head of electric motor & when he stepped onto a piece of angle iron & twisted his ankle. He was diagnosed with a fracture of a small bone on the top of his left foot.

Employee was applying kool-seal to the roof of the shop bldg., he was carrying a 5 gallon bucket to fill up a roller pan when he stepped onto the skylight & fell 22' to the floor below. EE was wearing a full body harness but was not tied off. Fracture to his right heel & ankle and he dislocated his left shoulder & hand.

EE WAS DISMOUNTING FROM THE HAUL TRUCK FACING AWAY FROM THE VEHICLE. WHEN HE STEPPED OFF THE LAST STEP HIS LEAD FOOT ROLLED OVER RESULTING IN A SPRAIN TO HIS RT. ANKLE.

WHILE TRAVELING IN A LOADER FROM ONE LOADING AREA TO ANOTHER LOADER WENT THROUGH A DIP IN THEROAD & BACK STARTED HURTING.

EE WAS CLEANING THE SHORT CONVEYOR IN THE MANUFACTURED SAND TUNNEL. EE WAS CLEANING SAND FROM BETWEEN THE TROUGHING IDLERS WITH HIS HAND. HIS SLEEVE WAS CAUGHT BETWEEN ROLLER ""CONVEYOR BELT"" HIS ARM WAS PULLED ACROSS THE ROLLER.

EMPLOYEE WAS REPAIRING A PIECE OF SKIRT BOARD RUBBER ON THE SCREEN. HE BENT DOWN TO GO UNDER AN OVERHEAD BEAM AND WHEN HE STOOD UP HE FELT A SHARP PAIN IN HIS LOWER BACK.

EE WAS CUTTING A 24"" WIDE PIECE OF CONVEYOR BELTLOCATED INSIDE THE CHUTE FROM THE 6'X 16' SCREENMATERIAL HAD BUILT UP ON TOP OF THE BELT & WAS BLOCKING THE CHUTE, AS THE BELT WAS CUT THE MATE RIAL SHIFTED & THE EE FELL & STRUCK HIS LEFT EL BOW ON THE SCREEN DOOR.

EE WAS INSTALLING SERVICE HORNS. HE STEPPED ON A FENDER AND TWISTED HIS ANKLE.

EE WAS WELDING OVERHEAD. SLAG FELL ON HIS HARD HAT AND WENT UNDER WELDING SHIELD INTO HIS LEFT EYE.

EE WAS ATTEMPTING TO STRAIGHTEN A BENT TRACK PAD ON THE CAT 330B ROCK HAMMER. THE PAD WAS HEELED AND EE WAS PUMPING THE HYDRAULIC JACK HANDLE. THE HEADOF THE JACK SPRANG OFF STRIKING HIS RIGHT HAND.

EMPLOYEE HAD CLEAN A SPRAY NOZZLE AND WAS TRYINGTO GET THE PLANT OPERATOR'S ATTENTION TO TURN ON THE WATER PUMP. THE EMPLOYEE WAS STANDING ON A 4' HIGH PILE OF SCREENINGS. WHEN HE STEPPED DOWN TO GET OFF THE PILE THE PILE WAS FROZEN AND DIDNOT GIVE AS HE EXPECTED. THIS CAUSED HIM TO STRAIN HIS CALF.

EMPLOYEE TOOK A SHORT CUT AND JUMPED DOWN OFF A 3' HIGH CONCRETE FOUNDATION TO GET A WRENCH. EMPLOYEE JUMPED TOO FAR AND LANDED ON HIS HEELS. HE FRACTURED AN ANKLE AND THEN HIS FEET CAME OUT F ROM UNDER HIM AND THEN SLID DOWN A BANK. HE HIT HIS TOE IN HIS BOOT WHEN HIS FEET CAME OUT FROM UNDER HIM AND FRACTURED A TOE.

EE WAS FILLING OIL TANK. OIL HAD SPILLED ONTO FLOOR ABOVE TANK. EE WAS HOLDING HOSE AT TANK. OILDRIPPED ONTO EE FROM ABOVE. OIL WENT INTO HIS EYE WHEN HE WIPED EYE WITH DIRTY HAND.

EE WAS BEING LOADED WITH BALL ROCK. WHEN LOADER DUMPED A BUCKET OF ROCK IN THE TRUCK, IT CAUSED THE TRUCK TO ROCK FROM SIDE TO SIDE.

EE WAS USING A AMPROBE BOLT METER TOP TEST THE FUSES, WHEN HE TOUCHED ONE LEAD TO THE CENTER SECTION A FLASH OCCURRED AND BURNED HIS RIGHT FOREARM.

EMPLOYEE WAS ASSISTING ANOTHER EMPLOYEE IN RE-INSTALLING A TAIL PULLEY GUARD, INJURED FELT A PAIN IN HIS BACK WHILE LIFTING THE GUARD.

EMPLOYEE WAS USING A HAMMER AND SCREWDRIVER TO CHIP ICE OFF TROUGHING IDLERS WHILE THE CONVEYERBELT WAS RUNNING. THE EMPLOYEE'S LEFT HAND AND WRIST WAS PULLED BETWEEN THE CONVEYER BELT AND THE IDLER.
