Struck by flying object

A 600 psi air hose on the drill ruptured causing the window to shatter. The operator suffered a laceration on the top of their head requiring 3 staples to close.

Caught in, under or between a moving and a stationary object

Employee was replacing the idlers on the CAT 289D skid steer. As they pulled one bolt out and proceeded to the other bolt the idler fell. The employee was trying to stop the idler with their hand and their right index finger got pinched between the idler and the frame of the machine.

Employee had a personal medical condition causing loss consciousness and falling to the ground and fracturing orbital bone.

Customer truck driver mashed EE's right ring finger in the tailgate of EE's dump truck

Struck by falling object

On 3/29/21 an employee was helping to replace a return roller on a 30' belt. When the roller was popped out it fell down on EE's left pinky finger. The employee asked to seek medical treatment. EE was diagnosed with a shattered bone at the end of EE's finger and received eleven stitches.

EE was using a Box Cutter to cut cardboard tubes used to backfill drill holes when blade became stuck and the momentum caused EE's hand to continue upward across blade of box cutter resulting in laceration of right hand between thumb and forefinger.

Two employees were rebuilding an air swivel off of a Cubex drill. One was knocking the center shaft loose with a 1"" socket. The other was holding the center shaft with left hand. The employee placed right hand under the shaft to catch it. When the shaft fell, it pinched the employees thumb between the shaft and the table, causing a laceration to right thumb.

Employee was working a ratchet adjuster on the 5 1/2 crusher to relieve the belt tension. The ratchet slipped and the employee lunged forward and struck his forehead above the eyebrow on the edge of the I-beam resulting in a laceration.

Body was discovered in the pit at approximately 10:00am on September 3, 2008 by an employee. The body was not an employee or contractor so Greenville County Sheriff was notified along with MSHA. EMPLOYEE WAS HELPING OTHERS CHANGE LOADER BUCKET TEETH & SIDE LIP PROTECTORS. HE STRUCK THE SIDE LIP PROTECTOR WITH A SLEDGE HAMMER TO FREE IT. A PIECE OF METAL FLEW OFF OF THE LIP PROTECTOR & STRUCK HIS INNER THIGH.

EE HAD USED A CABLE PULLEY TO LIFT SCREENS FROM THE GROUND TO THE SCREEN DECK. HE WAS ATTEMPTING TO REMOVE THE PULLEY FROM THE CHAIN THAT WAS HOLDING IT. AS HE LIFTED THE PULLEY TO UNHOOK THE CHAIN, THE PULLEY SLIPPED AND THE CHAIN WHIPPED AROUND AND STRUCK HIS FINGER.

EMPLOYEE WAS CHANGING DRILL STEEL WHILE STEPPING BACKWARDS. HE STUMBLED ON A ROCK AND FELL BACKWARDS. THE EMPLOYEE PUT HIS RIGHT ARM DOWN IN AN ATTEMPT TO BREAK HIS FALL.
