An ignition occurred in 9 South Left mains miner section. They had mined through an abnormal rock intrusion. During a non-coal cutting period, the conveyor chain was started and caused an ignition at the miner.

The 1A Capsule at the 1A shaft was out of service due to a malfunction with Up Overtravel electrical switch.

Struck by flying object

While prying a rock off the miner, the drill steel shifted up when the rock came off the miner. They let go of the steel and it came down and hit the top of their hard hat, which pushed down on their safety glasses causing a scrape on their nose and a chipped front tooth.

Caught in, under or between running or meshing objects

Employees right knee was caught between the bucket being raised and the bucket on the bottom of the shaft.

Employee was cutting a plank for blocking rail when the bucksaw became stuck in a knot. Employee cut their hand after pulling the saw loose. Injury was not the result of a training or compliance issue.

Fall to the walkway or working surface

Employee was observing the removal of a joint of 3"" hose. When the staple was removed from one end of the hose, the hose blew out under pressure. The stream of water forced the employee to the ground, where EE injured EE's arm and hip.

Employee was observing the removal of a joint of 3"" waterline. When the staple was removed, the hose blew out under pressure. The hose struck the employee in the mouth and head.

Employee was observing the removal of a joint of 3"" waterline. When the staple was removed from one end of the hose, the hose blew out under pressure. The hose struck the employee on the wrist.

Caught in, under or between a moving and a stationary object

Employee was changing a cradle at 3 East 58 Wall, 4 Mother Belt on the tight side, while the employee was maneuvering the cradle it slipped pinching the right ring finger between the cradle and the bedrail. The event resulted in a Fracture to right ring finger and a laceration requiring sutures.

Struck by falling object

Employee was removing old belt structure from an area and stacking it on scoop. A piece slipped out and accidentally struck employee on the hand that was placed against the scoop bucket.

Accident type, without injuries

The elevator at Patterson Creek portal was stopped because of a dip in power. Power dipped briefly and no fan service was lost at the time. The elevator needed the rope gripper release / safety switch reset and then the elevator returned to normal operation.

Struck by rolling or sliding object

Employee was walking by miner after placing crib under the cat. Inby last rib strap, the rib rolled out and broke into pieces, striking them on the right side of the face. This caused a laceration to right ear/head and contusion to face/jaw. Employee received sutures and fractures to the jaw/head. There were rock rib conditions and miner cutting below btm levels unevenly.

Caught in, under or between a moving and a stationary object

Employee was attempting to set a jack in the roof to secure a tailpiece during a belt move. The jack needed lowered for adjustment. When they set the jack down, the employee's hand was caught between the jack and the tailpiece. Injury was not the result of a training or compliance issue.

The elevator at Patterson Creek portal was out of service due to the a set of the rope gripper pads becoming damaged. The pads where replaced. The VFD also faulted during this time period, requiring it to be replaced and reprogrammed. Once work completed the elevator returned to normal service.

Fall to the walkway or working surface

Employee was building a stopping, and was tackled to the ground by an employee of a different contracting company, resulting in a shoulder injury. EE continued to work unrestricted until 4/5/2025. Injury was not the result of a training or compliance issue.

Caught in, under or between a moving and a stationary object

Employee was swinging a sledgehammer to break up a rock on top of the miner in the 9 South Mains- Right section, while swinging the hammer EE's left ring finger was caught between the hammer handle and the miner frame. This event resulted in a cut requiring 16 sutures and a fracture to the tip of finger.

Struck against stationary object

Employee was throwing block into the scoop bucket. As EE was throwing a block, EE did not lift the block high enough. EE accidentally hit hand against the scoop bucket because EE's hand was wrapped around the block while in movement causing injury to finger.

Over-exertion in pulling or pushing objects

Employee was dragging / pulling a wash down hose in 1N TG when the hose snagged something on the mine floor. EE felt discomfort in EE's left bicep. Diagnosed with Torn Bicep. LTA on 2/10/25.

Struck by falling object

Employee was installing a T3 strap against the roof from the miner in the 1B CM section. Employee was resting the strap on employee's shoulder during installation. The strap slipped off employee's shoulder and hit the top of employee's right hand causing a laceration behind the index finger. Employee received 5 stitches.

Fall onto or against objects

Employee was walking to the parts car in 9 South Mains, when EE tripped causing EE's left knee to strike against a loose rock. Diagnosed with a torn meniscus. LTA as of 11/20/24.

An ignition occurred in 1N Tailgate miner section while mining through an abnormal rock intrusion.

Over-exertion in lifting objects

Employee was offloading winder bars at the 2B Belt winder area. As EE was offloading the bar, EE felt tightness in EE's lower back. Diagnosed with a herniated disc. LTA as of 11/19/24.

Accident type, without injuries

An unintentional roof fall occurred in 3 East Mains #1 Entry 36 Wall. The fall is approximately 100' long / 16' wide / 20' high. There were no injuries as a result of this incident. Conditions contributing to the roof fall are adverse roof conditions and a shift of the strata causing the roof strata to separate above the anchorage.

Fall onto or against objects

Employee was traveling inby on the BMX Supply track. EE stopped at the shop switch and began to exit EE's mantrip. The mantrip following EE inby, hit EE's parked mantrip causing EE to fall back and hit EE's lower back and right elbow on the re-rail control box. EE was diagnosed with a fractured rib and transverse process fracture.

Caught in, under or between a moving and a stationary object

Employee was loading a bottom roller into the hoist car, when EE slipped and pinched right pinky finger between the roller and the hoist car. Employee received seventeen stitches.

Fall onto or against objects

Employee was walking along 3 Mother Belt when EE slipped and caught self before falling with EE's left arm grabbing the belt bed rail and felt a strain in EE's left bicep. On 9/4/24, MRI results determined employee has a torn labrum, and bicep tendon.

Over-exertion (Not Elsewhere Classified)

Employee grabbed handle on side of motor to help pull self into the jump seat. EE felt a pop and pain in shoulder. Injury was not the result of a training or compliance issue.

Employee was shoveling a rib roll and felt pain in shoulder. This employee did not miss work because of the incident until 1/29/2025.

Employee was cutting green dust hose for a sump using a knife. They had one hand on the hose to secure it and was cutting with the other hand. The knife accidentally slipped and struck finger causing injury.

Struck by flying object

While rib bolting in 1N TG section, employee swung the bolter towards the rib. The bolt wrench was still in the chuck. It hit a bundle of plates and knocked them off the miner. The bundle hit them in the right forearm resulting in 6 stitches.

Employee stepped on a small 3"" x 3"" piece of block and twisted left ankle when dismounting the mantrip. Diagnosed with a fracture in their left ankle.

Employees were offloading steel set beams from the supply car to the scoop. They were handling one of the legs (uprights) and employee accidentally got a finger caught between the end of the upright and the scoop on which it was being placed.

An employee was traveling inby on 3 East Mains track haulage. 109 wall switch was left for the turn and EE's mantrip traveled into the switch and struck a parked motor. EE hit their rib area in the operator's compartment. Diagnosed with three fractured ribs.

An unintentional roof fall occurred in 3 East Mains #5 Entry Inby 79 Wall intersection. There were no injuries as a result of this incident. Fall onto or against objects

Employee was walking in 9 South Mains, when EE's right foot tripped on the mine floor causing EE to fall against the rib. EE was diagnosed with a torn Achilles tendon and fracture to the fifth metatarsal in the right foot.

Caught in, under or between a moving and a stationary object

Employee was moving a rock off the ground next to the miner in 9 South Left Mains. EE pinched their left pinky finger between the rock and the rib while moving it. Resulting in a laceration and a fracture.

An unintentional roof fall occurred in 3 East Mains #1 Entry Inby 37 Wall. There were no injuries as a result of this incident. Conditions contributing to the roof fall are adverse roof conditions and a lateral shift of the strata causing the roof strata to separate above the anchorage.

Struck by falling object

While adjusting roof mesh on the miner, Employee had their right hand under the mesh to raise it over the bolter mast and their left hand on the bolter mast. A rock fell from the top and struck their left middle finger. Employee was diagnosed with a fracture to the left middle finger.

Absorption of radiations, caustics, toxic and noxious substances

Employee's calves were exposed to a modest amount of crib mixture while hooking up the ""y"" to the supply lines. Boots rubbed calves. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.

2023 · 17 incidents

Fall to the walkway or working surface

Employee was walking #4 Mother Belt conducting a pre-shift examination when they slipped on mud. From this event they fell to the ground and while falling EE attempted to brace themselves against the ground with right arm, which caused right shoulder to be injured from this action. Diagnosed with a torn rotator cuff on 12/5/23.

Struck by falling object

While side bolting on the left side of the miner in the 1NTG Section, a rock fell from the roof and broke apart on the rock deflector. A piece of the rock that broke apart fell outby and struck the right side of employees head / neck area. Employee was diagnosed with a concussion.

Struck by falling object

Employee was bolting on the left side of the miner in 2B section. After installing a new roll of roof mesh, the employee moved outby alongside the miner. Then the inby ATRS jacks were dropped, a rock fell from the roof hitting the pan and then hitting the employee on their left leg causing a laceration. They received sutures.

Struck by falling object

Employee was shutting off a one inch airline at the airline manifold, when an outby coupler on the four inch airline came apart. After separating, the inby pipe / manifold fell striking the employee's right forearm resulting in a laceration. Employee received four stitches.

Employee was rib bolting on the tube side of the 1B Miner when a piece of coal fell from the rib and struck the effected employees right knee. Employee finished EE's shift. On 11/17/23 MRI results determined the employee has a partial quad tendon tear.

Employee was side bolting on the miner in 2B section when a piece of rock fell from the roof and struck EE on the back of the hard hat causing the hat to fall off EE's head. Smaller debris fell from the roof and caused lacerations to EE's head and neck. Received five staples on the back of the head and three stitches on the neck.

Employee was in 1B Section loading a strap sled when EE's right middle finger was caught and pinched between the sled and the standard. Employee diagnosed with a finger tip fracture and received three stitches.

Caught in, under or between running or meshing objects

Employee was rib bolting when employee pinched left index finger between a bolt wrench and a drill steel. Diagnosed with a hairline fracture and received 5 stitches.

Struck against stationary object

Employee was driving a motor carrying an empty trip. EE slowed down suddenly to check a switch and the trailing motor slid into the empty trip causing EE to hit EE's right shoulder inside the operators compartment.

Absorption of radiations, caustics, toxic and noxious substances

Employee was working on a roadway and shoveling concrete material onto the ground. Some dust from the concrete accidentally got into EE's eye causing injury.

Caught in, under or between a moving and a stationary object

Employee was loading empty barrels in the bucket of a scoop when right foot was caught between the scoop bucket and the bottom. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.

Struck by falling object

While operating the tailgate side of the shear, a rock fell from the roof onto the tailgate drum. A small rock came off the drum and hit the employee on right thumb while holding the remote. They received 5 stitches.

Employee was rib bolting in 9 South Mains when a rock fell from the rib and glanced off EE's back. Employee felt pain in left shoulder after the incident.

Employee was holding up a pumpable crib bag while another employee was using a hilti gun to secure the bag to the top. When the injured employee slipped, they lost control of the bag and was struck in the nose by the swinging bag. Injury was not the result of a training or compliance issue.

Employee was rib bolting on the miner in the 3B section when they went outby to get a piece of wire mesh. As they pulled the mesh inby, they tripped on a rock and sprained their left ankle.

The BMX Portal Elevator hoist was out of service due to wires being loose / disconnected from the controller for the power pack. No persons were injured or entrapped due to this event.

Employee was side bolting on the miner in the 2B section when a small piece of rock (4""x4""x2"") fell from the mine roof and struck employee on the left ear, causing a laceration requiring 7 stitches.

Caught in, under or between a moving and a stationary object

Employee was changing out a roller on the beltline and it slipped out of EE's hand. EE went to catch it and got finger caught between the roller and a crib causing injury.

3B continuous miner section hit an uncharted gas well in the #2 entry just outby 45.5 wall.

An unintentional roof fall occurred in 3 North Mains 12 Wall #1 Entry Intersection. There were no injuries as a result of this incident. Conditions contributing to the roof fall are adverse roof conditions and a lateral shift of the strata causing the 8' bolts to fail at their couplers.

Employee was flushing lines and left leg and foot came into contact with silicate material. Injury was not the result of a training or compliance issue.

Employee pinched right thumb between a slate bar and a belt crossover while installing an arch on #5 mother belt. Employee received ten stitches to the thumb.

The elevator at BMX portal was out of service due to the control transformer malfunctioning. The transformer was replaced and the elevator returned to service.

Struck against stationary object

Employee was cutting off old belt in 4B storage unit. EE was in the process of changing blades on EE's razor knife. EE left the razor knife on the ground with the blade out. After getting a new blade out from bag, EE put hand on the ground and the razor knife to get up and suffered a laceration to right index finger which resulted in eight sutures.

Employee was walking around the tail of the slope and EE stepped wrong and rolled ankle causing injury.

Struck by falling object

Employee was transporting supplies through the mine on a motor, when the supplies caught an air hose hanging against the mine roof. The hose fell and hit the employee in the face. Surgery for a deviated septum on 6/7/22.

Employee was using a come-along to lift an ATRS jack over the rib door on the miner when EE pinched EE's left hand between the jack and door. EE received 15 stitches.

Employee was operating the head gate drum of the shear in 5B LW when a rock came off the drum and hit the employee on left hand. Employee received 3 stitches to left index finger.

Struck by falling object

Employee was rib bolting in the 4B miner section when EE swung the rib bolter towards the rib, the bolter hit a loose bolt knocking over a stack of roof plates that fell from above the rib bolter controls striking the employee on the left forearm causing a laceration. EE received fifteen sutures.

Struck by powered moving object

Employee was standing next to the can setter controls on the scoop with leg bent and foot against the rib causing a knee to stick out. Employee was injured when scoop bucket swung towards the rib causing injury to the knee.

The elevator at Patterson Creek portal was out of service due to a short power outage which caused the safety relay bar/switch on top of the elevator to activate. The breaker and switch were reset and the elevator returned to service. The miner onboard the elevator exited safely once it returned to service.

Struck by falling object

Employee was rib bolting when a piece of rock fell from the rib and hit right leg / ankle. EE was diagnosed with a fracture to lower right leg.

Employee was changing a rack wheel when the wheel slipped and EE tried to catch it and pinched finger between the rack wheel and the relay bar fracturing the tip of EE's left index finger.

Employee was using a claw hammer while putting together hose on the monorail. EE hit the hammer with a pick hammer to line up holes for the staple and a sliver of metal came off causing injury to EE's abdomen area.

Employee was setting a k-door frame and caught right index finger between the horizontal cross member and the vertical support beam causing a fractured finger tip.

Employee was setting up the can setter. It shifted and pinched employee's finger causing injury.

Employee was shoveling the tail of the slope and felt pain in the shoulder when EE raised the shovel to dump onto the belt. Note: Employee worked until Aug 23, 2021 when EE was diagnosed with a tear and had to have surgery. EE has been off since that date.

The BMX slope hoist car was taken out of service when the Main Breaker malfunctioned. The main breaker was replaced along with applying dielectric grease to the power plug connection points.

Employee was loading T-3 rib straps onto the miner unit when EE cut their left index finger on the edge of a T-3 strap. This event resulted in 5 stitches to the left index finger.

Employee was recovering vent tubes in the 5B HG section from 4 Left entry. While pulling a tube up off the mine floor, EE lost balance and fell towards the rib. While falling forward EE stuck right hand out to brace self, when EE's hand struck against a rib strap. Diagnosed with a fracture to the right 5th finger on 2/10/21.

Accident type, without injuries

The elevator at Patterson Creek portal was out of service due to a power fault causing a comm loss to the encoder reader which caused a fault malfunction in the computer processor. The computer processor had the faults cleared in the programming and the elevator returned to service.

Fall from machine

Employee was standing on the toes of a shield to advance the face, lost balance and fell backwards and right knee twisted causing pain. Diagnosed with a tibial plateau fracture and torn ACL / meniscus on 11/9/20.

Struck by falling object

While operating the miner, a rock came down between the mesh and struck EE on the left middle finger. EE received ten sutures to the finger.

Employee was assisting loading a scoop battery and as EE stepped back, EE slipped causing EE to fall down. EE felt pain in EE's left knee. Diagnosed with a tibial plateau fracture on 10/22/20.

Employee was walking towards bolter station when they stepped on a rock and felt a pain in right knee. Employee was diagnosed with a torn ACL on 9/30/20.

The 7 North #2 Capsule at the 7N2 shaft was out of service due to a malfunction with the VFD. The VFD was replaced and the capsule returned to service.

Coworker was removing conveyor belt roller from frame when roller broke and struck the conveyor belting material that was pulled up to provide slack for conveyor belt splicing. The impact caused the slack conveyor belting material to strike the injured employee causing a laceration to the left elbow and a contusion with swelling in the lower left thorax area.

Employee was sumping the miner in when a 12"" cc bolt sprung loose off of the top of the miner, striking the employee in the mouth chipping EE's front two teeth.

Employee was operating a gopher drill on the 5A Section belt when a piece of rock came from the mine roof and struck employee's right hand causing a laceration to the right middle finger. Employee received 13 stitches.

The 1A capsule at the 1A Shaft was out of service due to a malfunction with the PLC processor. The PLC processor had internal switches that became non-operational, the PLC processor was replaced and the capsule returned to service.

Employee was exiting a Brookeville mantrip when employee stepped on the rail causing employee to slip and fall. The employee's face area struck the side of the mantrip during the fall causing a laceration requiring 3 sutures.

Employee was rib bolting on the left side of the miner in 5B TG. EE grabbed a roof strap from the top of the miner and flipped it so EE could slide it over to the right side of the miner. EE's right pinky finger got stuck in the tip-out bolt hole when EE flipped the strap. Diagnosed with a fracture to EE's right pinky finger.

CB Mining employee was assisting with the assembly of an Armored Face Conveyor Chain on the head gate. While in a prone position a rock approx. 24"" X 18"" X 3"" dislodged from the roof and fell approximately 4 feet striking the back of the right knee. Employee experienced sharp pain and knee was immobilized.

Fall to the walkway or working surface

Employee was using a come-along to move the outby end of the 5B HG tail piece. The Come-Along was hooked to a 5/16 chain that was secured in a belt clip. The chain broke causing the employee to fall backward therefore hitting their head, causing injury. Received four sutures to the left cheek and a fracture to the occipital bone.

The BMX slope hoist was taken out of service during an MSHA inspection for a broken center left brake assembly's inby guide spool. This condition is in conjunction with MSHA citation #9078996. The MSHA District Manager was on-site and promptly investigated the accident.

Employee was hanging communication cable along 7 North Track, when EE tripped on a track tie and fell on EE's left hand. The incident resulted in a fracture to the left pinky finger.

Employee was trying to remove a cable bolt that was stuck in the roof, while the employee tried to pull the bolt from the roof while reversing the head of the drill the bolt head struck EE in the left arm causing a laceration and shoulder pain. EE received three sutures on left arm.

The employee was in the 3A longwall in the process of changing bits on the tailgate drum of the shearer. While the employee and a co-worker were spinning the drum down, the injured employee pinched right ring finger between a newly installed bit and the LW face. The result of this incident was 14 sutures to the right ring finger.

Struck by falling object

The employee was standing behind the rib pinner on the intake side of the 5A Joy ED miner when a bundle of 4"" x 6"" roof plates unexpectedly fell off the side of the miner. The plates then struck the employee's left middle finger causing a laceration that required 5 sutures to repair.

Employee was cleaning beltline and injured back while throwing rock dust with a shovel.

The safety switch for the buffer jack was out of alignment. The elevator hoist was out of service on the bottom landing until the safety switch mounting bracket was adjusted. There were no injuries or entrapments as a result of this occurrence.

Employee was handling belt cradle and smashed finger causing injury.

Struck against a moving object

The employee was scooping an entry with a 580 scoop, and was attempting to remove a rock with the bucket by going underneath it. While in this process the front scoop tires where lifted off the ground, then the bucket came off the rock causing the scoop to bounce down. During this motion the employee bounced in the cab and bit down on teeth, resulting in a chipped tooth.

The employee was bolting on the intake side of the continuous miner, while retrieving a roof strap from the strap rack on the miner EE got EE's left hand caught between an upright standard on the strap rack and an upright standard on the bolt rack causing a laceration/avulsion to the back of EE's hand. The injury required 12 sutures to repair.

The fall originated just inby a permanent ventilation stopping located in the entry and traveled approximately 19 1/2 long and 7 1/2 feet wide. There were no intersections affected by the fall. No persons were injured or equipment damaged as a result of the fall.

The jack rope gripper faulted due to an oil leak. The elevator hoist was out of service at the surface landing until the jack was filled up with oil. There were no injuries or entrapments as a result of this occurrence.

Employee was helping to remove a rock that was stuck in the LW face chain/panline. While lifting the rock using a come-a-long the rock unexpectedly pivoted and caught employee's left foot between the rock and the face side of the panline handrail. On 3/29/17 employee was diagnosed with an avulsion fracture of the navicular bone in left foot.

Over-exertion in wielding or throwing objects

Employee was carrying a de-rail to be installed on the track when the de-rail unexpectedly opened up causing employee to lose grip with the left hand allowing all the weight of the de-rail to shift to the right arm injuring their shoulder. Employee was diagnosed with an torn rotator cuff and began missing work on 5/24/17 to have the right rotator cuff surgically repaired.

Employee was examining 3 belt around 48 wall when employee took a step and strained employee's left knee. The walkway in the area was level and had no obstructions. Employee was diagnosed with a torn ACL and began missing work on 5/5/17 to have employee's knee surgically repaired.

Employee was adjusting the roof mesh above the continuous miner when left hand slipped and struck the edge of a previously installed roof strap. Caused a laceration to the knuckle of middle finger on left hand that required 2 sutures to repair.

During the weekly inspection of the 7N #1 capsule it was determined to be non-operational. The cause of the capsule being non-operational was determined to be from the guide rails in which it travels in the shaft being bent and distorted which would not allow it to travel the entire length of the shaft. There were no entrapments or injuries due to this occurrence.

Employee was installing Air Lock Doors outby the Longwall Headgate. Injured employee and another employee went to set the leg of frame upright and the top jack slid down striking injured employee on left thumb.

Employee was rib bolting off of the Continuous Miner, while drilling a hole a piece of rock fell from the rib striking the employee on the right elbow causing a contusion. On 9-22-15 the employee began missing work due to this occurrence.
