Employee was striking metal with a hammer when a fragment of steel broke off and struck them in the forehead. The fragment was removed with forceps and the employee returned to work. The employee later felt irritation, so upon re-examination, another fragment of steel was found in their forehead, so surgery was performed on 11-4-25 to remove the second fragment.

While operating the shuttle car the driver had to travel back past the cable snub to dump on the feeder. When doing this the cable came up and hit them on the side of the head. This occurred 3 different times throughout the shift. As the shift went on, they developed a headache.
Two Micon technicians while traveling down the slope entrance into the mine, lost their brakes, as well as the regen braking was not functioning properly. The operator of the cart was able to make it to the bottom of the slope without rolling over or throwing either one of them off the ride.

While shoveling around the feeder, EE felt pain in right shoulder. EE worked EE's regular job until 7/1/24.

While helping the miner operator to put the rub rail back onto the side of the miner, the rail swung out and struck EE on the top of EE's left foot.
While removing an Omega Block wall, a block fell striking EE in the head, back and leg. No medical treatment was pursued at that time. EE has been working since the accident and did not seek any medical treatment until 06/26/2023. 06/26/2023 was EE's first missed work day.
While backing up a cart, the steering knob spun and hit them in the right hand.
While removing a K wall EE used EE's shoulder to knock over a panel. When EE hit into the panel it didn't move and EE jammed EE's arm into EE's ribs. When EE returned to bolting on Monday 1/30/23 second shift EE's ribs started to bother EE. That is when EE reported it to EE's boss.

While driving the mantrip out of the mine at the end of the shift, the driver hit their head on the roof. EE did not report this incident to the section foreman or any management personnel. On 12/05/2022 EE called and said that wouldn't be able to work due to a concussion. On 12/09/2022 EE reported it as a work related accident to mine management.
While recovering High Voltage cable, onto a skid, EE's hand slipped and a loop of cable hit EE in the mouth. On 11/12/2022 EE went to the hospital and found that 3 of EE's teeth had been cracked and need removed.
While getting a bucket of raw coal from the pile, a small rock bounced in the open window, striking the operator in the head by EE's right eyebrow.
While replacing rollers on the overland belt, one of the bottom rollers slid down the catwalk, striking EE on the right ankle.

While installing new tracks on the CAT miner, the mechanic was using a chain come-a-long to pull the slack so that EE could install the pins. With tension on the come-a-long the hook slipped off and struck EE in the right thigh, cutting EE's pants and leg.
While reaching for a metal roof strap, the bolter operator cut the knuckle of right index finger on the sharp edge of the strap.
While pushing drill steel to the roof to begin drilling, a piece of rock fell from the roof and struck EE in the back of the upper right arm. The rock measured approximately 18 x 24 x 1 and had a sharp outer edge.

While walking toward the rear of the bolter, the operator tripped and caught self, with both hands, on the bottom. When this happened, EE felt a sharp pain in the left wrist.
While roof bolting the operator was straightening the bolt in the hole when operator lost grip allowing the bolt to rotate around away from the operator. This caused operator to overextend self and pull a muscle in the left side of stomach and leg.

While hanging empty 1-1/2"" miner waterline to the roof, the victim felt a pain in the mid to lower left back.

Employee was operating shuttle car and hit a bump causing the hard hat to fall off and EE hit head inside the shuttle car canopy.
While walking behind the roof bolting machine as it was traveling across the section, the bolter helper got their head hit from the top of the canopy up to the roof. Due to the height of the section, the helper was walking bent over where helper's head was out in front of helper, unaware of how close helper was to the bolting machine.

While moving miner cable and miner waterline to the rib, EE felt a burning sensation in the upper left arm.
While cutting the straw off a waterline coupler with a utility knife, the victim cut the back of left thumb knuckle.

While carrying a bundle of roof bolt resin from the rear of the bolter to work station, the bolter operator tripped on a rock and fell on right hand. This caused a fracture in right thumb.
While sealing an Intake stopping, the employee developed chemical burns on right forearm and both thighs from B-Bond mine sealant.
While moving a cart in reverse the operator turned around to see and was steering with left hand. EE got too close to the rib and the front tire caught, spinning the steering wheel out of hand. This caused the ball on the steering wheel to strike ee in the back of hand.

The Section Mechanic was prying on miner waterline that was wrapped around the feeder crusher drum when the Mechanic noticed a soreness in left arm. Continued to work regular job at full duty until 12/06/2017.
While bolting at the face of the number 3 slope, EE was straightening a roof bolt to place it in the drill head. EE was pushing with one hand on the bolt and the other on the plate. EE's hand on the plate slipped and cut EE's right thumb through the glove.
While bolting, EE twisted at the waist and felt something pop in EE's lower back. EE then had pain down EE's leg and was unable to straighten EE's back. EE continued to work EE's regular job duty without restriction until 01/29/2018.

Miner was lifting a drill head when he felt a strain in his middle back.
The roof bolter operator was in the pit loading supplies onto his bolter. When he cut the metal band off of his supplies, the band sprung back and cut his right upper arm. He received 3 stitches.
