While working on a pump in a sump, employees foot got stuck in the mud. EE tried to twist out but fell over breaking ee leg.

While pulling a rock out of bridge conveyor chain. a sharp edge of the rock cut his hand.

Employee crossed over small supply car tongue and tripped, twisting his knee.

While setting 5X5 posts, one fell down striking him down the back.

While putting bottom rollers into hanging brackets, the bottom roller slipped out and caused the bracket to fly up and strike him in the cheek. 3 stitches.

Employee was building a brattice wall with 4"" tongue & groove cement blocks. He slammed his thumb between two cement blocks cutting his thumb.

While unloading rock dust off of a supply car, the car ran over his foot.

While getting on and off MBC seat EE twisted his knee.

Knee swelled up from excessive crawling. Knee was drained on 1/7/13

Employee was climbing down ladder of hi-lift. His hand slipped off and he fell to the ground.

EE was putting pressure to a roofbolt after spinning when a piece of rock fell cutting his arm.

Jumped on top structure to install top, chipping a bone and caused swelling

Roof fall old room set up in #4 entry one crosscut outby spad #415. Roof fall for about 10' long and fell out from nothing to about 5 or 6' high. The area was double rowed posted and dangered off all access.

While setting 5 X 5 posts one came down and struck him in the wrist.

EE had his knee drained on 4/20/12 after his shift.

EE was crawling in mud and twisted his knee.

While removing a plate on the joy miner the plate slipped and a sharp edge cut his hand.
