Struck against stationary object

Individual stepped off a main tail onto a crib block, the crib block rolled on its side causing EE's ankle to roll and breaking EE's tibia.

Struck by falling object

Individual was removing the canopy from the twin boom while performing maintenance when it slid off striking EE's wrist resulting in a fracture.

While unbolting a ram cylinder, the wrench EE was using to hold the nut pinched EE's right pinky finger causing a laceration resulting in stitches.

While standing up, stooped over, getting off mantrip when EE stepped forward felt a pop in EE's foot. This resulted in a fractured 5th metatarsal bone.

When the miner operator activated the cutter heads on the miner, a foreign object struck the individual in the left side of face resulting in a laceration and fracture.

Individual was on top of bolter while the operator of bolter lowered boom the individual's left foot was pinched between the long and short arms of boom causing a partial amputation of toes on left foot.

While lifting up low belt to remove low structure, felt a burning pain in lower back. Individual went off work 3/18/2025, making this a lost time accident.

While bolting, a steel came out of roof landing on right thigh while lowering drill the head caught the steel puncturing right thigh resulting in sutures.

While cleaning a tracking beacon with utility knife, EE's knife fell. When EE went to pick it up, the blade contacted EE's left lower thumb causing a laceration needing 2 sutures.

While running supplies with diesel scoop over time developed pain in lower back. Individual had to receive surgery on 11/05/2024 for Lumbar Foraminal Stenosis resulting in a lost time accident.

Individual was crawling through brattice and crawled into the scoop bucket that was parked on other side resulting in 5 sutures on their nose.

While pulling drill steels out of roof, a stuck drill steel became dislodged and struck individual in upper lip resulting in stitches.

While placing a roller in structure, the roller slid and pinched left index finger against the structure causing a laceration needing 7 sutures.

While loading roof bolt plates on helper side of bolter, elbow came in contact with a pack of roof bolt plates already on the bolter causing a small laceration needing 3 stitches.

While swinging bolter head, employee tripped and fell forward and put right handout to catch their fall and left hand grabbed joystick which dropped bolter head onto their right hand causing 2 fingertip amputations.

While recovering room set up, individuals right hand got caught on a cable support hanger resulting in a laceration needing 9 sutures.

While pulling a spreader out of mainline structure, the spreader got caught between rail and belt causing spreader to stop motion and individual's back to twist causing lower back pain.

While changing park brake pad on #5 scoop, hydraulic pressure bled off caliper pinching left index and middle fingers between pad and caliper causing laceration to left index and middle finger.

The helper snubbed the cable of twin boom as operator was pulling into the number 5 entry. The helper went toward the off-operator side without notifying the operator. As the operator turned the machine into the cut, the back of the bolter swung towards the solid rib pinning the helper against the rib causing a fracture.

#4 entry, wet bottom. While employee was throwing a crib block out of the way, left index finger got pinched between crib block and bolter resulting in 5 stiches.

While trimming a piece of rubber on EE's bolter, the employees knife slipped and stuck EE's left lower forearm. This required surgery to repair EE's ulnar nerve.

While unhooking a cart from the Damascus ride the tongue of the cart fell to the ground smashing EE's right ring finger, thus requiring stitches.

The employee was traveling down the intake slope on a personnel carrier when the employee experienced service brake failed. Employee's dynamic brake was functioning but was gaining speed, employee steered the vehicle in to the right rib to reduce speed. After contacting the right rib the vehicle over turned.

While picking up structure the employee felt pain in lower back, coal seam was 48"" high. EE worked for the next 15 days before going off work. The first day off was 05/01/2019.

Fall from machine

While throwing posts off the back of the Cat IT Forklift the employee slipped and fell off of the counterweight. Breaking 3 ribs.

The employee was helping belt up when the employee was struck on the right thumb with a rail that was being installed by another ee. This injury result ed in 4 stitches.

While installing a cable bolt employee felt a pop in the Right knee. First day off work was 4/4/2018. EE was working on knees in low coal height as usual, conditions were dry and the ee was installing a cable bolt which ee had weight on knees to insert the cable bolt and felt a pop in ee right knee.

Knee swelling from crawling. EE went in for surgery 9/10/2018 to repair the left knee as it continually got worse.

While tramming a scoop, ee ran over a piece of rock and hit head on the roof. EE needed 6 staples.

While driving EE's Damascus EE hit a cable bolt with EE's head knocking EE's hard hat off and cutting EE's head. EE required 9 stitches.

Repetitive crawling. Went to doctor 11/1/2017 determined bursitis in both knees. Didn't miss any work until 1/2/2018 when they operated on them.

While sliding a blade of a drag onto the Damascus, the employee experienced discomfort in left shoulder. A MRI revealed bursitis.

While building a wall the employee lifted a cement block and twisted feeling pain in lower back. The employee reported to us 7/31/17 that the employee had Doctors excuse that the employee was unable to work. This was the first time the employee notified us of the injury.

While bolting the employee reached for a roof bolt and was cut by a burr on another bolt, causing a laceration on right forearm.

While changing dolly wheels on an MBC EE felt something in the right shoulder. While swinging a sledge. Had surgery on 12/16/16 to repair shoulder.

A slicken-sided piece of rock 78"" long by 20"" wide by 0-11"" thick fell from the mine roof in the #2 entry of the E-mains 001 working section. This rock struck employee on the left hand causing a compound fracture to the employee's fifth finger (little). The area was bolted according to the roof control plan with 6' fully grouted resin bolts and oversized bearing plates.

While washing a scoop with the pressure washer something flew into left eye. Thus causing a large corneal abrasion.

While removing a belt trainer from the dolly the employee smashed left ring finger when EE put it in the tray of the bridge. This action broke the bone on the end of this finger. there was no missed days.

While pulling on the miner cable EE twisted right knee. EE worked up until 6/20/16 when EE had it operated on.

The underground miner notice a piece of rock hanging out by the face, employee went to get a rock bar and heard a pop. Employee went to spin away and the rock hit foot.

While putting main line in EE slipped and fell striking EE's right elbow on the ground. EE's elbow swelled up and EE went to hospital. EE received shot.

While the employee was running across the coal yard to his hi-lift, he stepped in a hole rolling his right foot breaking both bones in his lower leg.

While getting dressed he sat on a chain of a basket, and while pulling his first boot on he must have leaned back and unhooked the chain. Then when he leaned ahead to pull the other boot on, the chain he sat on became free dropping the empty basket down hitting him on the top of his head. He received 10 stitches.
