installing roof bolt with plate, bending bolt over to bolter head when hand slipped and it sprang back causing a laceration on top of right thumb requiring stitches

Tightening a bolt on the joy stick when the boom came down on the wrench in EE's hand causing a laceration in EE's palm

Individual was assisting a move from room to room when the individual was tearing low belt apart and felt a strain in groin. Individual worked full duty until surgery on 12/12/2022

Normal working section, helping the mechanic tighten blower belt on a Twin-Boom bolter, using a pry bar to pull tension on the belt when it slipped hitting EE's right hand index finger causing a cut which required stitches

Working installing bolts in 38"" high section, twisted knee and felt a pop and pain resulting in a torn meniscus, worked until surgery on 3-6-19.

Getting out of MBC hit elbow on side of machine.

Hit his hand off side of MBC while getting out at the end of the shift. Sought treatment of injury on 6/30/14.

Loading structure into scoop bucket smashed thumb between structure and bucket. Became lost time on 3/6/14

Drilling hole for roof bolt, drill steel broke and swung around hitting bolter behind ear causing a cut.

Installing roof bolt, aligning bolt head to chuck, spun motor to align and bolt head caught glove and tore end of small finger on left hand off to first knuckle.

On going knee problem from crawling on knee. Accident happened on 6/14/12, employee continued to work, 2/13/13 he had surgery on knee Prepatellar bursitis

EE was crawling in the section when a piece of rock fell from roof hitting him on his foot.

Normal conditions

Was bending 6 foot roof bolt.

EE was driving a johnson personal carrier out of the mine, when driving past a parked scoop he came in contact with the scoop and his left arm was injured between the two machines.

He was handling a piece of corrigated sheet metal in the muddy conditions when it cut him.

Was throwing cribs under miner to get unstuck when the crib block hit side of machine with his finger between the block and machine.

Hooking up a bolt trailer to the scoop, he pinched his hand between scoop & trailer tongue.

Miner's knee. I was not told of any injury date. I was just told knee was swollen and he was seeing his family physician, and not a panel doctor until 1/4/08 for one visit then back to his family physician. Didn't consider it to be work-related at this time

Handling low belt structure when thumb got smashed between a rail and bottom structure.

Gradual oncoming knee problem from crawling on knee surgery 11-14-2007.

A piece of rock fell out between bolts.

Backing miner out of right X-cut, stopped and was swinging miner over when left tram lever on remote stuck, causing miner to swing downhill. The auger of the miner rolled over EE's leg, causing a break.
