Over-exertion in lifting objects

individual was pick up a piece of low belt structure when EE felt a pop in EE's low back. the individual went off work on 04/06/2026 as a lost time accident

ind was crawling when they felt a pop in their right knee

Caught in, under or between a moving and a stationary object

Individual was getting into the MBC when something contacted the man-in-position switch allowing the CM to start. The CM pulled forward moving the MBC and the kitchen contacted individual's foot fracturing two bones in the foot prior to shutting off.

Struck by falling object

Ind was grabbing a roof plate off the bolter tray when the plate caught another plate which came down striking the ind in the right arm causing a laceration which resulted in 5 stitches.

A roof fall above anchorage was found in cross cut 2-1 at wall four in 30 butt rooms at 5:37 am. The area was bolted with 6' super twists and 16' .7 cable bolts. There was a clay vein in the area.

Struck by flying object

the operator was pulling belt with a scoop when the belt slipped and came back into the operator's compartment striking the individual in the head and neck resulting in a pinched nerve.

ind was removing drill steels from the roof when EE's steels got stuck and EE tried to use the drill chuck on the bolter to remove the steels when EE's finger got smashed between the steels and the chuck which resulted in eight stitches

EE was operating a battery powered scoop and was traveling around a corner when EE put hand on the stop block of the scoop. The scoop pivoted going around the corner until the stop block hit against the frame of the scoop crushing EE's finger tips.

Ind was rolling mine belt when EE felt a pop in the right shoulder which resulted in a shoulder sprain.

Struck by flying object

ind bent a roof bolt over they were using a hack saw to cut the roof bolts off. while cutting the roof bolt they were pulling on the bolt and the bolt broke coming back and strike them on the chin which resulted in 5 stitches

ind was building a cement block wall when EE went to place EE's next block into place EE has EE's middle finger between the two blocks and smashed EE's finger which resulted in six stitches

Struck against stationary object

Individual was installing roof bolts. When EE went to reach for a roof plate, the plate cut through EE's glove and lacerated EE's hand which resulted in stitches.

Over-exertion in lifting objects

ind was laying out roof bolts alongside of EE's bolter. When EE felt a pop and burning in EE's shoulder. This resulted in a lost time injury.

Struck by powered moving object

individual pulled EE's johnston down in front of the highlift. EE assumed that there was no one in the highlift. the operator of the highlift did not know that the johnston was in front of the bucket EE began to move forward striking the johnston knocking the employee down resulting in fracture ribs.

Ind was helping to align to get the pan jack pin out. When the pin come out it struck ind's safety glasses causing laceration to left eye brow which resulted in 3 stitches.

Ind was putting cover back on scoop when the cover slid pinching EE's finger causing laceration which in turn resulted in stitches.

Ind was using a rock bar to pry a screen that was jammed when the screen freed itself the bar slipped out of the ind hand striking EE in the left facial area.

Ind was throwing a bundle of roof plates across bolter when the bundle of plates caught the side of the bolter and cut left hand middle finger which resulted in needing stitches.

Struck by falling object

the roof bolter was backing out of the entry winding up slack cable and pulled it down from the roof. striking the ind in the facial area resulting in a fractured nose.

EE was helping put in low belt structure when they went to pull a bottom stand under the belt they smashed a finger between two bottom stands which resulted in a laceration to finger and getting 4 stitches.

Struck by flying object

Employee was using a come-along to pull up main belt structure. When the come-long slipped out of the chain that was holding the come-long and it struck the employee in the facial area which resulted in the employee getting stitches.

The injured employee was going outby to get supplies and had to cross the system. EE communicated EE's intentions with the middle mbc operator and the injured employee went under the bridge before de-energizing. The operator reacted by stopping EE's machine. It is assumed that the front MBC continued tramming and the bridge tilted down as EE was crossing.

An unplanned roof fall occurred in #3 entry from cross cuts 5 thru 9 which one cross cut outby survey spad#3598 to one cross cut inby survey spad#3605. The cave was 320 feet long and the height of the cave is unknown. Adverse geological conditions were visible. Also the roof fall was found on pre-shift at 5:30 am

Fall to the walkway or working surface

IND GOT OFF THE JOHNSTON PERSONAL CARRIER AND BEGAN TO WALK TO EE'S MOBILE BRIDGE WHEN EE TRIPPED AND WENT TO FALL. EE STUCK EE'S ARM AND HAND OUT TO PREVENT SELF FROM FALLING WHEN EE FELT A PAIN IN EE'S LEFT SHOULDER WHICH RESULTED IN A SHOULDER STRAIN

An unplanned roof fall occurred out by survey spad #583 approximately 20' long and at least 6' high in the intersection of the number 2 entry. Adverse geological conditions were visible.

IND WAS PULLING SLACK BOLTER CABLE BACK SO THAT THE TWIN BOOM BOLTER DIDN'T RUN THE CABLE OVER AS IT COME OUT OF THE CUT. WHEN THE REEL WINDER COME ON IT TIGHTENED UP THE CABLE STRIKING THE IND IN THE FACIAL AREA.

Ins was lifting a roll of mine belt up when the employee felt something pull in left shoulder. The employee continued to work regular job until 07/25/2018. Went in for shoulder surgery on 07/26/2018.

Caught in, under or between a moving and a stationary object

Men were advancing a skid of high voltage for a power move and the scoop was pushing the skid into a cross cut. The employee thought that the scoop was done moving the skid and EE went up to get the nips off the skid and the scoop operator was told to move the skid up two more feet. The scoop operator didn't see employee and caught employee foot between skid and bucket.

While mining a cut of coal in cross cut of room 26 to room 27 the rib blew out and there was an open hole approx. 2 1/2 feet in the rib line.

Individual was helping re-bolt an area of the mine that was already bolted when the helper on the bolter drill steels was getting stuck in the roof. The individual used a wrench to hold pressure on the strap so the helper could get the steels out and got a finger pinched between the steels and the roof.

Individual was taking a pump off of a pallet. when the pump went to fall off the pallet the individual tried to prevent the pump from falling off the pallet which in turn fell on hand and fracturing middle and index finger

IE was bent over the drill head of roof bolter attempting to take suction hose off to get it unplugged. His knee bumped the joy stick causing the drill head assembly to move pinching his left hand (little and ring fingers) in a pinch point on the boom causing injuries to his fingers.

IE was lifting rub rail on Joy 14CM-15 when he felt a severe pain in his lower back. IE was transported by ambulance to Indiana Hospital and was released.

Roof fall found by mine examiner on return run at #2 Entry Return Run at Station 2045. Size of roof fall 70' Long X 18' Wide X 6' High. Ventilation was not disrupted. No equipment involved.

Rubbed or abraded

Employee is experiencing pain in his left knee from repetitive kneeling and crawling. Employee has been a miner for 21 years. This is not a result of any one single accident. Employee has worked full duty up to and including 10/28/2014. Last day worked was 10/28/2014.

IE was putting structure in scoop bucket when he pinched his left ring finger between structure and scoop bucket.

When doing preshift roof started to work and fall occurred at inby dump point from #6(ss2053)to & including #3(ss2051). Size of fall 245'x19'x10'. Ventilation not disrupted. Roof bolter covered and miner, haulage and section scoop are inby the cave.

After roof bolting machine was pulled up to install next row of bolts & after ATRS was set, IE swung his head out & placed drill steel into head in preparation to drill outside hole, when a piece of roof rock(18""x12""x1""-2"" thick) fell striking canopy then striking his left hand causing laceration.

IE was bending a roof bolt when he twisted his right knee. IE has worked full duty up until surgery on 07/31/14.

IE was checking torque on bolter, he had gauge wrench in chuck and put pressure on it, it checked ok, when he went to release pressure, he pulled lever too far and spun wrench into his right wrist, hand and hip area pushing him against the bolter.

IE was installing outside bolt in #4 Entry of 1 Butt, when IE went to straighten the bolt, the bolt spun around hitting his left hand on the end of T-5 Channel resulting in a laceration of his left index finger.

IE was driving mantrip out at end of shift, did not notice uneven bottom, front wheel dropped in, steering wheel kicked and jammed right hand causing injury to right middle finger.

IE assisting in drilling & installing rib bolts using a Post Hole Drill. As IE was pushing 4' Torque Tension roof bolt into hole that he had just drilled, jack on drill that tightens against mine roof/floor to hold drill stable slipped out of position allowing Post Hole Drill to kick out pinching his hand between coal rib & drill assembly causing crush injuries to his left hand.

Employee was building a wall with 6"" block. The block (wet) slipped out of hand and landed on left hand catching small little finger.

While cutting sandrock bottom outby in #5 Entry to cut a ditch for water to run to a sump, a piece of sandrock flew up hitting employee in the right eye. The sandrock broke safety glasses of employee and cut his right eye.
