The scoop guy was pushing a skid in to the belt line. The skid caught a roll of belt pushing it sideways into the employee who was in a crosscut, bruising EE's hip.

EE was exiting a scoop and twisted knee. EE was excused from work on 3/11/2025, and will be getting meniscus repaired in right knee.

EE slipped on ice getting out of EE's vehicle and hit EE's head. This resulted in a concussion. EE was excused from work 12/10/2024.

While installing a bolt, EE pinched pinky finger between the plate and drill head. Which resulted in stitches.

EE was trying to remove pin from the low-tail, when the tail moved back. EE's finger got pinched between the pin and the top surface of the tail. Which resulted in stitches.

EE was spotting low structure and strained shoulder, resulting in a lost time accident as of 12/28/2023.

Individual was trying to straighten a top slide when it slid off of the structure and the individual struck hand on the rail. This resulted in a fractured hand.

Pinched their hand between drill head and roof, which resulted in stitches

Employee was backing up the mobile, and pulled themselves up in the seat, at the same time the mbc went over a hump, pushing their head down. Which caused their chin to hit off the guard above the levers. This resulted in stitches.

EE was removing a rock from the conveyor, when it dislodged and pinched EE's finger between the rock and coal chain, causing a fracture.

Was backing up miner for the day, the employee shut the machine off. Crawled under the bridge and it teetered and pinched their hip, causing a dislocation.

While lifting low belt to remove a bottom stand, felt a strain in back, which became a lost time accident 02/20/2020.

Steep grades in the area.

Employee was installing roof bolt when the employee pinched hand between the bolter chuck and head of roof bolt resulting in laceration which required stitches.

Scoop operator was tramming around a corner when EE came in contact with a low hanging corner.

Found a roof fall in the 1st west mains section at 2:55 PM in the #2 entry 8 X cuts outby section. Fall measured approx. 19' wide - 80' to 100' long 10' high. Area was bolted with 4' bolts.

Crawling into the MBC and hit knee off the side of it. Got medical attention 10-2-14.

Bolting the roof twisted to get a bolt.

The individual got pinched between low belt structure and scoop.

Found a roof fall during a pre shift in the 1st East butt. It was in the last open crosscut from #3 entry into both left & right cross cuts. It measures approx. 80' long 19' wide 5' to 6' high

Changing pump on miner

Miner was installing bolt when glove got caught on roof bolt head & he started to rotate bolt at the same time.

A piece of loose rock fell from between roof bolts & hit the man on the hand cutting his palm on his left hand.

Found a roof fall during air course run it was in 1st SW Mains #7 intake roof it measured approx 50' long & 18' wide 8'-10' high.

EE was going through double doors in belt line slipped and fell on his left knee.

A roof fall occurred in the intersection of 2nd NW mains at spad #1012 measured approx 40'long - 19'wide - 10'high area was bolted with 6"" fully grouted belts and 10' cable bolts.

Employee slipped on ice while walking between office and shower house at Lowry Mine. This is a reoccurence of this injury. It was not reportable before. Employee woke up 8/11/10 in pain with a swelled knee, continued to work rest of week, then went on light duty 8/11/10. Then went off work 8/19/10.

Crawling on bottom with knee pads and sprained knee.

The individual cut his right wrist on a roof plate while attempting to remove a bent roof bolt.

The individual caught his right hand between the tounge of the duster and the belt chain that was connected to the duster

Employee was installing a canopy on a s&s scoop. The canopy slid around pinching his finger.

Employee was splitting a wedge with a pick hammer when he struck himself with the pick end.
