Loading plates onto Roof Bolter. When a piece of draw rock fell hitting their right hand. Resulting a laceration on the hand.

Bolting at the face and a piece of draw rock fell hitting EE's right finger.

Building a wall. Employee was pulling a block off the top layer of the cube with right hand. when the block fell smashing left pointer finger. that they had resting on the edge of the scoop bucket.

Faceboss was helping belt up. During the belt up, EE got EE's finger smashed between the lowtail and the rail. Resulting in stitches on EE's right pointer finger.

Employee was bolting at the face when EE went toward the back of the bolter to get a longer wrench when a brow fell and struck EE resulting in a dislocated hip.

Employee was measuring the area to cut and set posts when the rib rolled out and struck EE's leg resulting in a bruise leg. This resulted in a lost time accident.

Belting up at the face, putting structure underneath the belt. Felt pain in the right shoulder.

Individual was installing a roof bolt when EE strained their groin area resulting in a hernia. EE worked until EE's surgery on 12/30/2024.

Loading 16ft cable Bolts on bolter. They put a hand on top of scoop bucket and a piece of draw rock fell on top of their hand. Resulting a broken pinky finger on left hand.

EE reported right knee pain on 7/12/24. Due to excessive crawling. Worked full time until 8/12/24. Is when EE had surgery done on right knee.

Driving in beltline. Going through a low area and jammed neck.

A roof fall above anchorage occurred in the working section in the 3, 4, and 5 entries near station spad #7193. The area was bolted with 6' resin bolts and cable bolts.

A roof fall was found in the beltline entry in the E-15 butt approximately 80 feet long and at least 6' high. There was a localized clay vein in the area of the fall.

Prafulla Bursitis on Left Knee. From everyday crawling underground.

At the face, Belting up. EE was pulling low structure over. While doing that, EE felt a pain in the stomach area. Later to find out EE got a Hernia.

Changing out a roller on tight side of the belt and twisted right knee. Employee worked up to date of surgery on 6/29/2022.

Operator was trying to pull out a bent roof bolt and EE's right finger got caught between the bolt and plate. Resulting laceration on right finger that end up receiving stitches.

The operator was traveling back the return on the scoop and ran over a piece of omega block causing EE to roof out and cut EE's right ear. This resulted in sutures.

Operator knuckled scoop down on skid. Causing the scoop sideways and roofed the operator up to the roof.

Throwing low belt structure along the rib. Twisted to grab another piece of low belt structure and felt a sharp pain in lower back. EE worked underground until 8/9/2021.

Swollen Knee from crawling in low coal seam. The employer work underground until the day for surgery on 7/14/21.

Shoveling coal under belt. When Shoveling felt a pain in lower back. Diagnosed with lower back sprain.

Two surveyors were traveling up the slope out of the pit when the personnel carrier lost power and began drifting backwards. The ride impacted an embankment causing it to rollover and pin the passenger, causing fractured ribs, and head trauma. This resulted in a lost time injury.

Employee was rolling belt up onto low structure, when employee felt a strain in right shoulder. Employee worked at position until 10/2/2019, is when employee got surgery.

Everyday crawling. Employee's knee has been swollen up through out this time. Getting it drained and numerous doctors. They decided to do surgery on 5/28/2019.

At 1/16/2019 1:00 pm. Employee right knee was hurting while crawling. Went to the doctor on 1/24/2019 and found out it was a meniscus tear. Employee worked until 2/15/2019. The day of surgery.

An over sized rock was stuck in the bridge of the MBC. EE tried to remove the rock out of the bridge. When pulling the rock out EE's left hand slipped off causing it to fall back into the bridge and smashing EE's finger. EE worked until 11/23/2018. Is when got surgery in finger.

At 9:00 am on 7/9/18. Employees knee was bothering EE while crawling. Went to the doctor on 8/2/18 MRI showed a medial meniscus tear on right knee. Employee worked until 8/23/18 is now getting surgery today.

Conditions was bad. Already bolted with 6' bolts and cable bolts. It appears that a change in the roof strata, (transition area), above the anchorage zone occurred, Allowing bed separation, Resulting in the roof fall.

Employee was picking up a belt bottom stand when he was struck by a rail causing a laceration to his right middle finger requiring 5 stitches.

While crawling employee twisted his left knee causing a tear in his meniscus went off work 8-21-15 having surgery on 9-21 15.

While crawling over time left knee started to swell and get sore.

While building a brattice wall employee was using a pole ax to split wedges. He hit the roof causing the ax to hit his right knee with the blunt end causing a contusion. Employee went off work on 5-27-2015.

While pulling rock off miner employee stepped on uneven bottom twisting knee. Employee worked up until the day of his surgery on 4-28-15

While drilling the roof a kettle pot fell striking employee in right thigh causing a laceration and bruise.

While going to restart belts employee tripped on a wire striking right knee on a rock. Continued to work until day of surgery.12-3-2014

While crawling in beltline employee hit a hanging roof bolt plate cutting his right ear.

Employee was putting drill steels together when a piece of draw rock fell scaring him. He had his hand on the joy stick and hit the thrust causing drill steels to go thru his hand.

While crawling employee felt something pop in his left knee. Keep on working until 4-7-14 when he went off.

While crawling out of the scoop he twisted his back

WHILE GOING OVER THE BELT CROSS-OVER HE SLIPPED AND HIS LEFT ARM GOT CAUGHT ON THE RAILING, CAUSING A FRACTURE OF HIS LEFT ELBOW.

He was on the stacker knocking frozen mud down. The bar he was using slipped and hit his left knee. Contusion and possible medial meniscus tear

While putting together 4"" waterline a pounding device slipped out of a miner's hands and struck EE's left hand, cracking a bone in his left wrist.

Normal crawling on knees has led to right knee problems - prepatellar bursitis right knee.

Found intersection caved/cross cut inby spad #u18 in #4 of main return air course cribbed off. All side air going over top of cavs and around all sides.

While installing a roof bolt, left index finger got caught between roof bolt plate and drill head of roof bolter.

While pushing up roof bolt employee placed hand on top of coupler and pushed wrong leaver causing bolt to cut left hand index finger.

While pulling bottom belt stand out from under belt EE smashed finger between another stand cutting finger and breaking bone on finger tip.

Lifting a bundle of roof bolt plates and placing them on the machine, employee hurt his hand, fracturing a bone behind the right index finger.

CAVE AT FACE NO #4 ENTRY IN A7 BUTT 75' INBY SPAD NO. 1534 FELL FROM FACE BACK ABOUT 25' ALSO FELL ABOVE THE BOLTS USING 5' FULLY GROUTED BOLTS AND ROOF MATS. CRIBBED AND TIMBERED OFF ALL SIDES OF CAVE AND DANGERED OFF SAME.

Found cave in #4 entry 300' outby face of a9 butt fall is about 50' long above the bolts. cribbed off all sides and dangered off same moved to another section.

WHILE PUSHING BOLT & PLATE UP TO THE ROOF BOLT, IT KICKED OUT & STRUCK BACK OF LT. HAND CUTTING RING FINGER & MIDDLE FINGER, REQUIRING STITCHES.

SEVERE BILATERAL CARPAL TUNNEL COMPRESSION DUE TO OCCUPATIONAL TRAMA. THIS PROGRESSIVE CONDITION NOW REQUIRES SURGICAL CORRECTION.

EMPLOYEE WAS HOOKING A SLING AT BACK OF LOW BELT DOLLY. HE SLIPPED AND FELL, CATCHING THE LOW BELT STRUCTURE WITH HIS RIGHT HAND. THE DOLLY BACKED OVER HIS RIGHT HAND.

ROOF FALL HAPPENED IN SUMP ENTRY #2 OFF A-MAINS AT STATION SPAD #U828. FALL IS APPROX. 150'LONG.FALL IS NOT IN THE MAIN ENTRIES. THEY ARE ALL OPEN CRIBBED OFF ALL SIDES OF CAVE AND REBOLTED W/ 12""CABLE BOLTS. FALL IS OUTBY FACE AREA ABOUT 2000'.

CREW OF MEN WERE ADDING BELT & STRUCTURE TO LOW LOW BELT. AFTER PULLING BELT UP TIGHT WITH CHAIN AND SCOOP TRACTOR EE WENT TO UNHOOK CHAIN. SCOOP DRIFTED BACK ABOUT THREE INCHES AND PINCHED FI NGER BETWEEN TAIL AND CHAIN. INVESTIGATION FOUND NOTHING WRONG WITH BRAKE SYSTEM ON EQUIPMENT.

ROOF FALL HAPPENED IN THE #4 ENTRY OF THE A8 SECTION 5 HOLES INBY 51 SPAD # OU966 AT PLUS 20+79. WE THINK POSSIBLY GOING UNDER A STREAM INFLUENCE. CRIBBED OFF ALL SIDES OF CAVE AND DANGERED OF F TO MAKE SAFE TRAVEL ROUTE AROUND CAVE.

AN UNPLANNED ROOF FALL HAPPENED IN THE #4 ENTRY OF THE A8 SECTION ONE HOLE INBY #5 MAIN RETURN AT SS STAD #834. I FOUND THE CAVE ON MY WEEKLY AIR RUN. CLEANED SAME UP AND REBOLTED AND CRIBBED THE AREA TO MAKE AREA SAFE AND TRAVELABLE.

WHILE HAMMERING ON SPLICER ROD EMPLOYEE MISSED AND HIT 5TH FINGER ON LEFT HAND CUTTING OPEN SAME.

EE WAS CRAWLING OUTBY FACE AREA A PIECE OF ROCK FELL FROM ROOF FALLING ON RIGHT FOOT. AREA WAS BOLT ON ROCK FELL BETWEEN BOLTS.

WHILE LIFTING DISCHARGE ROLLER, ROLLER SLIPPED AND LANDED ON FINGER CUTTING INDEX FINGER OF RIGHT HAND.

WHILE BACKING MINOR OUT OF X CUT PINCHED FOOT BETWEEN MINER SEAT AND RIB.

EE WAS WALKING ACROSS COAL YARD, TRIPPED OVER A PIECE OF COAL & FELL ONTO HIS RT SIDE INJURING RT HIP & RT SIDE OF RIB CAGE.

NORMAL CRAWLING ON KNEES & OTHER SPORTS RELATED INJURIES HAS LED TO RIGHT KNEE PROBLEMS, BEING #1- CHRONIC PREPATELLAR BURSITIS RIGHT KNEE, #2- TEAR IN THE POSTERIOR TORN MEDIAL SCRAPED SAME D ID NOT REMOVE. OCCUPATIONAL ILLNESS DUE TO REPEATED TRAUMA.

EMPLOYEE HAS A RECURRING PROBLEM IN HIS LOWER BACK FROM ACCIDENT ON 6-14-00 DOES NOT KNOW WHAT HAPPENED TO HIS BACK OR WHEN IT HAPPENED.

EE WAS PUSHING A BOLT UP WITH A BOLTER HEAD AND GOT FINGER BETWEEN HEAD OF BOLTER AND BOLT. CUT FINGER.

ON 7-24-01 EE CRAWED ON A PIECE OF ROCK BRUISING RIGHT KNEE CAME BACK TO WORK LOST NO TIME. AS A RESULT FROM NORMAL CRAWLING AND REPEATED TRAMA EE HAS DEVOPOPED CHRONIC PREPATALLAR BURSITS OF RIGHT KNEE.

NORMAL CRAWLING ON KNEES HAS LED TO RIGHT KNEE PROBLEMS. PROBLEMS BEING #1 - CHRONIC PREPATELLAR BURSITIS RIGHT KNEE. #2 - TEAR IN THE POSTERIOR HORN MEDIAL MENISCUS RIGHT KNEE. OCCUPATIONAL I LLNES DUE TO REPEATED TRAUMA.

KNEE SWELLED FROM NORMAL CRAWLING ON IT.

ON 7/24/00 EE CRAWLED ON A PIECE OF ROCK BRUISING RIGHT KNEE. CAME BACK TO WORK LOST NO TIME. AS A RESULT FROM NORMAL CRAWLING AND REPEATED TRAUMA EE HAS DEVELOPED CHRONIC PREPATELLAR BURSITIS OF RIGHT KNEE. (OCCUPATIONAL ILLNESS DUE TO REPEATED TRAUMA.)

OPERATOR SHUT MACHINE OFF AND WHILE EXITING STRUCK LEAVER WITH LIGHT CORD MAKING MACHINE TURN FINGER AT PINCH POINT AND GET SMASHED.

VICTIM LIFTED 80LB BAG OF CEMENT AND TWISTED WITH IT. SPRAINED LOWER BACK.

WHILE CRAWLING AND SHOVELING KNEE GOT SORE AND SWELLED UP.

WHILE ROOF BOLTING KNEE SWELLED UP AND GOT SORE.

VICTIM WAS INSTALLING ROOF BOLTS. PINCHED FINGER BETWEEN BOLT PLATE & BOOM OF ROOF BOLTER.

EMPLOYEE WAS ROOF BOLTING AND GOT FINGER BETWEEN BOLT PLATE AND DRILL HEAD OF MACHINE.

MAN GOT OUT OF SCOOP AND BUMPED KNEE ON BOTTOM . IT WAS AT NO.2 ENTRY IN THE LAST OPEN CROSS CUT.

LAYED HAND DOWN ON METAL STRAP CUT BACK OF LITTLE FINGER.
