Struck by falling object

Loading plates onto Roof Bolter. When a piece of draw rock fell hitting their right hand. Resulting a laceration on the hand.

Bolting at the face and a piece of draw rock fell hitting EE's right finger.

Building a wall. Employee was pulling a block off the top layer of the cube with right hand. when the block fell smashing left pointer finger. that they had resting on the edge of the scoop bucket.

Faceboss was helping belt up. During the belt up, EE got EE's finger smashed between the lowtail and the rail. Resulting in stitches on EE's right pointer finger.

Employee was bolting at the face when EE went toward the back of the bolter to get a longer wrench when a brow fell and struck EE resulting in a dislocated hip.

Employee was measuring the area to cut and set posts when the rib rolled out and struck EE's leg resulting in a bruise leg. This resulted in a lost time accident.

Individual was installing a roof bolt when EE strained their groin area resulting in a hernia. EE worked until EE's surgery on 12/30/2024.

Loading 16ft cable Bolts on bolter. They put a hand on top of scoop bucket and a piece of draw rock fell on top of their hand. Resulting a broken pinky finger on left hand.

EE reported right knee pain on 7/12/24. Due to excessive crawling. Worked full time until 8/12/24. Is when EE had surgery done on right knee.

Driving in beltline. Going through a low area and jammed neck.

A roof fall above anchorage occurred in the working section in the 3, 4, and 5 entries near station spad #7193. The area was bolted with 6' resin bolts and cable bolts.

A roof fall was found in the beltline entry in the E-15 butt approximately 80 feet long and at least 6' high. There was a localized clay vein in the area of the fall.

At the face, Belting up. EE was pulling low structure over. While doing that, EE felt a pain in the stomach area. Later to find out EE got a Hernia.

Operator was trying to pull out a bent roof bolt and EE's right finger got caught between the bolt and plate. Resulting laceration on right finger that end up receiving stitches.

The operator was traveling back the return on the scoop and ran over a piece of omega block causing EE to roof out and cut EE's right ear. This resulted in sutures.

Operator knuckled scoop down on skid. Causing the scoop sideways and roofed the operator up to the roof.

Throwing low belt structure along the rib. Twisted to grab another piece of low belt structure and felt a sharp pain in lower back. EE worked underground until 8/9/2021.

Swollen Knee from crawling in low coal seam. The employer work underground until the day for surgery on 7/14/21.

Shoveling coal under belt. When Shoveling felt a pain in lower back. Diagnosed with lower back sprain.

Struck against a moving object

Two surveyors were traveling up the slope out of the pit when the personnel carrier lost power and began drifting backwards. The ride impacted an embankment causing it to rollover and pin the passenger, causing fractured ribs, and head trauma. This resulted in a lost time injury.

Employee was rolling belt up onto low structure, when employee felt a strain in right shoulder. Employee worked at position until 10/2/2019, is when employee got surgery.

Everyday crawling. Employee's knee has been swollen up through out this time. Getting it drained and numerous doctors. They decided to do surgery on 5/28/2019.

At 1/16/2019 1:00 pm. Employee right knee was hurting while crawling. Went to the doctor on 1/24/2019 and found out it was a meniscus tear. Employee worked until 2/15/2019. The day of surgery.

An over sized rock was stuck in the bridge of the MBC. EE tried to remove the rock out of the bridge. When pulling the rock out EE's left hand slipped off causing it to fall back into the bridge and smashing EE's finger. EE worked until 11/23/2018. Is when got surgery in finger.

At 9:00 am on 7/9/18. Employees knee was bothering EE while crawling. Went to the doctor on 8/2/18 MRI showed a medial meniscus tear on right knee. Employee worked until 8/23/18 is now getting surgery today.

Conditions was bad. Already bolted with 6' bolts and cable bolts. It appears that a change in the roof strata, (transition area), above the anchorage zone occurred, Allowing bed separation, Resulting in the roof fall.

Employee was picking up a belt bottom stand when he was struck by a rail causing a laceration to his right middle finger requiring 5 stitches.

While building a brattice wall employee was using a pole ax to split wedges. He hit the roof causing the ax to hit his right knee with the blunt end causing a contusion. Employee went off work on 5-27-2015.
