Roof Fall in # 4 entry of P butt, at the end of the shift Face boss called outside to report a fall in #4 entry onto the la bolter no injuries reported, called 800 to notify MSHA

While the maintenance foreman was entering the mine he noticed a fall in the belt line on A main just inside the porthole, he reported to the section and contacted the mine foreman

While pre-shift to the section was in the process, the examiner found a roof fall in #3 entry on the front MBC. area was dangered off and a report was phoned outside of the discovery. Fall was aprox. 30 foot long 25 foot wide and 4 foot thick, fall was located in #3 entry of M butt

While he was helping work on a belt drive he was struck by a man trip on his left hip

While employee was performing the preshift of the mine, he came across a roof fall in number 2 entry at spad # 5011 aprox 3 x cuts outby. The section was in the process of moving back away from this area the day before.

while employee was moving johnson duster by hand it shifted hitting his right hand against a steel bracket lacerating the right middle finger

while mining in number 4 entry in E-3 butt the continuous mine struck a well casing approx,.6-8 inch hole dia approx 9-10 inch no gases present water coming from pipe approx 6-10 gal a min federal inspector was onsite issued a K order at time

While bolting on the fletcher twin boom,he lowered the drill head onto his right knee

At 12 pm Dave Chambers found a roof fall in F mains return approx. 40 ft long , located 32-33 brk # 2 entry

EE was working on a LA bolter, EE decided to back the bolter into a cross cut to let a scoop pass,while backing into the xcut EE looked back to see how much room he had left when the la drifted and pinned his left arm between the rib and bolter, causing a laceration from the bottom of his bicep to his forearm he was able to free himself immediately.

while helping the mechanic ee was swinging a 5lb sledge when he slipped twisting his lower extremity, ee did not want to go to have checked until 3 days later. upon ee was diag.with a torn ligament in his back and l3,l4,l5 s1 ruptured

Roof fall on D-Belt in #5 entry between cross cuts 11-13, approx. 8' high. Fall was posted and dangered off.

While employee was roof bolting on the dbt twin boom, he was inserting his roof bolt when a piece of roof fell striking his left hand and side of head. Injuring his left hand, he was taken to the Heritage Valley Medical Center for treatment, where he was released and told to see an orthopedic surgeon for a crushing injury to hand.

Miner operator was attempting to remove a rock on the opposite side of miner (right side), miner was shutdown and a rock fell out between the bolt approx 4 ft x 4 ft 5 in thick striking him on the head pushing him to the ground. He was boarded and collared and brought outside to the ambulance who decided to fly him to UPMC Presby in Pittsburgh for precautionary measures.

While installing a safety cone on life line, employee was using a screw driver to pry cone apart. It slipped striking him in the hand causing a laceration on his left hand.

While working outside, Employee lifted with help from another employee a heavy part feeling a pull in is stomach. He refused treatment at that time. Since then, he saw a doctor and was told to have surgery on him.

While EE was tramming the miner, the EE reached next to him to remove a ventalation curtain and struck his left foot with the bull wheels of the miner, causing a fracture to his left ankle.

While greasing #2 belt drive the injured party ran into a roof mat which resulted in six sutures to the upper R/S lip. There was no lost time because od accident.

While installing a roof bolt, the employee's glove caught on the bolt when spinning. This caused a fracture, dislocation and lacerations on the left hand. Specifically, the third and fourth fingers.

WHILE EE WAS INSTALLING A ROOF BOLT A PIECE OF ROCK SLID DOWN THE BOLT, PINCHING EE'S FINGERTIP BETWEEN THE ROOF BOLT PLATE AND THE HEAD OF THE BOLT. THIS RESULTED IN AN AVULSION OF THE LEFT INDEX FINGER WHICH REQUIRED SEVERAL SUTURES.

WHILE DRIVING HIS JOHNSON FOUR WHEELER THE INJURED PARTY RAN INTO BELT STRUCTURE WHICH STRUCK HIM ON THE LEFT SIDE. THIS RESULTED IN TWO BROKEN RIBS. THERE WAS NO LOST TIME BECAUSE OF THE ACCIDENT.

while trimming a belt splice, the razor knife slipped and glanced off a belt rail and cut the employee. this resulted in a cut to the left hand that required five stitches. there was no lost time because of the accident.

WHILE WALKING BY HIS BOLTER, EE STRUCK HIS RIGHT ARM ON A ROOF BOLT PLATE THAT WAS ON THE TRAY OF THE BOLTER. THIS RESULTED IN A CUT THAT REQUIRED FOUR STITCHES. THERE WAS NO LOST TIME BECAUSE OF THE ACCIDENT.

WHILE OPERATING A FOUR WHEELER OUT OF THE MINE, THE INJURED PARTY HIT A ROCK WITH THE WHEEL CAUSING THE STEERING KNOB TO STRIKE HIM ON TOP OF THE RIGHT HAND. THIS RESULTED IN VENIAL BLEEDING & 4 SUTURES.

WHILE INSTALLING FLYBOARD HE WAS HOLDING BOTH STEELS AND MOVING POT BACK AND FORTH WHEN THE POT HIT A ROCK CAUSING THE TWO DRILL STEELS TO PINCH HIS FINGERS.

WHILE OPERATING MANTRIP, EE HIT BUMP RESULTING IN RAISING HIM UP & STRIKING MINE ROOF.

PROLONGED CRAWLING OVER A PERIOD OF TIME CAUSED AN OCCUPATIONAL ILLNESS CALLED BURCITIS.

WHILE NACKING MANTRIP UP, HE STRUCK A ROOF BOLT RESULTING IN BROKEN COLLAR BONE.

INJURED RIGHT WRIST WHILE RESETING SCOOP BREAKER.

WHILE MINING COAL IN THE #5 ENTRY WE ENCOUNTERED A BORE HOLE. IT TURNED OUT TO BE AN 11 INCH ABANDONED GAS WELL. THE INCIDENT OCCURRED IN OUR INTAKE OR #5 ENTRY, 35 FEET INBY SPAD #2536. THE S ECTION IS REFERRED TO AS 3 SOUTH BUTT.

WHILE BRINGING SCOOP OUTSIDE, THE BATTERY LEAD SHORTED RESULTING IN BATTERIES SMOKING.

HE DEVELOPED SORE LEFT KNEE FROM CRAWLING.

WHILE CRAWLING HE REACTIVATED PREVIOUS INJURY.

WHILE LOADING ROCK, HE FELT SHOULDER AND BACK PAIN.

WHILE LOADING ROCK DUST, EE SLIPPED.

WHILE BACKING UP 3 WHEELER, EE CAME IN CONTACT WITH AN 8"" I BEAM THAT PUNCTURED HIS LEFT LEG.

EE WORKED ALL DAY 9-10-2002. HE CALLED IN A 2255 ON 9-10-02 AND STATED HE WOULD BE OFF WORK UNTIL 9-16-02. NO IMMEDIATE ACCIDENT REPORT WAS COMPLETED.

WHILE EE WAS LOADING CRIB BLOCK HE TWISTED HIS BACK CAUSING INJURY.

WHILE PUTTING COVER ON SCOOP, HE LOST HIS FOOTING AND SLIPPED STRIKING HIS LEG.

EMPLOYEE CAME IN CONTACT WITH THE MINE ROOF WITH HIS HEAD, CAUSING HIS INJURY.

WHILE TRAMMING THE FLETCHER ROOF BOLTER, HE RAN OVER SOME UNEVEN BOTTOM CAUSING THE DECK TO RAISE AND CAUSING HIS HEAD TO BE ABLE TO COME IN CONTACT WITH THE MINE ROOF.

EMPLOYEE WAS UNHOOKING A CHAIN FROM SCOOP WHEN IT WAS ENERGIZED AND RAM WAS PULLED IN CAUSING CHAIN TO TIGHTEN ON HIS MIDDLE AND INDEX FINGERS.

EE WAS OPERATING SCOOP WHEN HE RAN OVER A 6X6X42 CRIB CAUSING THE SCOOP TO IMPACT UPON HITTING THE GROUND RESULTING IN AN DISC INJURY IN EE'S LOWER BACK.

INJURED AND OTHER EE WERE REMOVING ROCK FROM THE TOP OF THE FAIRCHILD MINER. OTHER EE HAD A SLATE AND INJURED WAS PULLING WITH HANDS. THE ROCK BROKE, PINCHING AND CRUSHING INJURED'S FINGER.

BACK PAIN AND NUMBNESS FROM WORKING CONDITIONS.

WHILE OPERATING MANTRIP, EE CAME IN CONTACT WITH ROOF BOLTER TRS.

EE STOPPED TO PICK UP PLATES, BENT OVER AND PICKED UP PLATES, RESULTING IN PULLED MUSCLE.

PART OF THE RIB ROLLED ONTO EE'S HAND AS HE AND CO-WORKER WERE MOVING ROCK OUT OF WAY OF BOLTER CABLER.

EE WAS LIFTING A 50 POUND BAG OF ROCK DUST WHEN HE FELT A PAIN IN HIS BACK.

WHILW EMPLOYEE WAS OPERATING SCOOP, HE RAN OVER A ROCK AND THE IMPACT INJURED HIS UPPER BACK.

WHILE PULLING ROCK OUT OF CONVEYOR HE FELT SHOULDER PAIN.

HE WAS SHOVELING UNDER BELT, LIFTED SHOVEL INTO BELT AND BELT CAUGHT SHOVEL CAUSING SHOVEL TO BE PUSHED DOWN AND HANDLE PUSHED UP SMASHING HAND BETWEEN SHOVEL AND RAIL.

WHILE CLEANING #5 FACE, HE INHALED BATTERY FUMES.

WHILE EE WAS PULLING ROCK OUT OF BRIDGE HE FELT HIS SHOULDER POP.

WHILE EE WAS CROSSING OVER BELT NEAR #4 TAIL, HE LOST HIS BALANCE AND FELL, STRIKING HIS HAND ON THE RAPID HAUL.

WHILE SHOVELING RIB HE FELT AND HEARD HIS SHOULDER POP.

WHILE HE WAS INSTALLING ROOF BOLT, HIS GLOVE GOTCAUGHT ON BOLT RESULTING IN SPRAIN & STRAIN OF HAND & FINGERS.

HE WAS PULLING HIGH VOLTAGE CABLE FROM ONE POWERBOX TO ANOTHER.

EE WAS UNLOADING TRASH FROM THE FRONT LIFT LOADER WHEN HE SLIPPED STRIKING PIECE OF STRUCTURE CUTTING HIS LEFT ARM.

DUE TO FREEZING AND THAWING, HIGH WALL FELL AT THE FAN HOUSE APPROX. 10 FT HIGH AND 16 FT IN DEPTH, HITTING BACK OF FAN HOUSE AND AMAGING THE 4160 VOLT UNDERGROUND POWER CABLE.

EE FELL WHILE EXITING THE 482 SCOOP, FALLING ON HIS LEFT SIDE BRUISING HIS HIP, RIBS, AND LEG.

A PIECE OF ROCK WAS STUCK IN TAIL OF BRIDGE. HE STATED THAT HE THOUGHT THE OPERATOR OF THAT BRIDGE KNEW HE WAS GOING TO REMOVE THE ROCK. AS HE ATTEMPTED TO DO THIS THE OPERATOR STARTED HIS BRI DGE CAUSING A ROCK TO STRIKE EE ON THE LEFT RINGFINGER.

EE WAS INSTALLING A #5 REBAR 42"" ROOFBOLT. WHILESTRAIGHTENING THE BOLT IN AN AWKWARD POSITION HEFELT SOMETHING IN HIS RIGHT SHOULDER POP.

EMPLOYEE WAS TRAMMING THE #2 BRIDGE AROUND CORNER AND HIS BRIDGE WAS PULLED TOWARDS THE RIB BY THE #3 BRIDGE OPERATOR, #2 BRIDGE JAMMED EMPLOYEE'S LEFT LEG INTO RIB.

EMPLOYEE WAS CLEANING #4 ENTRY WITH SCOOP. HIS LIGHT CORD CAUGHT ON A BENT ROOF BOLT TWISTING HIS HEAD WHICH RESULTED IN PULLED MUSCLES IN HISNECK.

EE WAS RIDING MANTRIP OUT OF MINES. HE RAISED UP AND STRUCK A CONTROL BOLT WITH HIS HEAD. IMPACT RESULTED IN A CUT TO HIS FOREHEAD AND SCALP TAKING APPROX 30 STAPLES AND MULTIPLE STITCHES TO C LOSE.

DRAW ROCK FELL WHEN EMPLOYEE SET HIS ROOF SUPPORT JACK HITTING HIM ON THE RIGHT SHOULDER.

KETTLE BOTTOM/DRAW ROCK FELL BETWEEN T.R.S. AND CONTROL STATION WHILE HE WAS DRILLING HITTING EE ON HEAD, ARM AND HAND.
