EE was working on a LA bolter, EE decided to back the bolter into a cross cut to let a scoop pass,while backing into the xcut EE looked back to see how much room he had left when the la drifted and pinned his left arm between the rib and bolter, causing a laceration from the bottom of his bicep to his forearm he was able to free himself immediately.

while helping the mechanic ee was swinging a 5lb sledge when he slipped twisting his lower extremity, ee did not want to go to have checked until 3 days later. upon ee was diag.with a torn ligament in his back and l3,l4,l5 s1 ruptured

While employee was roof bolting on the dbt twin boom, he was inserting his roof bolt when a piece of roof fell striking his left hand and side of head. Injuring his left hand, he was taken to the Heritage Valley Medical Center for treatment, where he was released and told to see an orthopedic surgeon for a crushing injury to hand.

Struck by falling object

Miner operator was attempting to remove a rock on the opposite side of miner (right side), miner was shutdown and a rock fell out between the bolt approx 4 ft x 4 ft 5 in thick striking him on the head pushing him to the ground. He was boarded and collared and brought outside to the ambulance who decided to fly him to UPMC Presby in Pittsburgh for precautionary measures.

While installing a safety cone on life line, employee was using a screw driver to pry cone apart. It slipped striking him in the hand causing a laceration on his left hand.

While working outside, Employee lifted with help from another employee a heavy part feeling a pull in is stomach. He refused treatment at that time. Since then, he saw a doctor and was told to have surgery on him.

While EE was tramming the miner, the EE reached next to him to remove a ventalation curtain and struck his left foot with the bull wheels of the miner, causing a fracture to his left ankle.

While greasing #2 belt drive the injured party ran into a roof mat which resulted in six sutures to the upper R/S lip. There was no lost time because od accident.

While installing a roof bolt, the employee's glove caught on the bolt when spinning. This caused a fracture, dislocation and lacerations on the left hand. Specifically, the third and fourth fingers.

WHILE EE WAS INSTALLING A ROOF BOLT A PIECE OF ROCK SLID DOWN THE BOLT, PINCHING EE'S FINGERTIP BETWEEN THE ROOF BOLT PLATE AND THE HEAD OF THE BOLT. THIS RESULTED IN AN AVULSION OF THE LEFT INDEX FINGER WHICH REQUIRED SEVERAL SUTURES.

WHILE DRIVING HIS JOHNSON FOUR WHEELER THE INJURED PARTY RAN INTO BELT STRUCTURE WHICH STRUCK HIM ON THE LEFT SIDE. THIS RESULTED IN TWO BROKEN RIBS. THERE WAS NO LOST TIME BECAUSE OF THE ACCIDENT.

while trimming a belt splice, the razor knife slipped and glanced off a belt rail and cut the employee. this resulted in a cut to the left hand that required five stitches. there was no lost time because of the accident.

WHILE WALKING BY HIS BOLTER, EE STRUCK HIS RIGHT ARM ON A ROOF BOLT PLATE THAT WAS ON THE TRAY OF THE BOLTER. THIS RESULTED IN A CUT THAT REQUIRED FOUR STITCHES. THERE WAS NO LOST TIME BECAUSE OF THE ACCIDENT.
