While coming out for supplies man discovered a roof fall along # 1 travelway at third crosscut inby the drift opening

While building return wall went to put block on top row & felt pull in his shoulder. Did not miss any work until Aug. 5th. 2015. Then went off to have his shoulder fixed.

While installing mainline structure felt a strain in his lower back while putting stands on ropes. Came to work on 02/02/2015 and worked every day until 03/30/2015 then went off

While on return run discovered roof fall at intersection of crosscut 270 in the 2nd. NW. Mains. Fall was between 20-30 FT. long 18 FT. wide & 6 FT. high

While traveling outby in return to get low belt he ran over a rock and roofed out.

Before installing first row of bolts he examined the roof and saw a loose piece of rock and grabbed his rock bar. While scaling the loose piece a piece of rib/ rock above him came down and he was moving away from it when the piece he was scaling fell and pinched his hand between the rock and the floor.

While shoveling along low belt a thin piece of draw rock fell from the roof and cut his arm

While walking across parking lot slipped and felt knee pop Employee kept working until 01/02/2014 then went in to have knee operated on and is now off

While spotting dust out of a dust car he felt his back strain

While backing bolter out of a cut there was some rock laying on the mine floor the back of the bolter pushed a piece of the rock against the rib. When the rock hit the rib it came up and cut his ear with a sharp edge, he received stitches.

While doing preshift examination discovered roof fall out by in 4-5 entry at spad # 7961 mearsuring 30 ft.x 15 ft.x 10-0 ft. high

While hanging ventilation curtain a piece of draw rock measuring App.3FTx8 FT.x3-4 in. fell from roof striking his knee

While making weekly examination of intake seals discovered roof fall between crosscuts 119&120. Was inby side 15' long outby side 30' long 18' wide & 4 ft. high.

While making return examination discovered 2 roof falls. #1 along 5 SW Mains in x-cut 4-5 about 20' long but was not affecting travel or air.

While making return examination discovered 2 roof falls. Along B-Mains in x-cut 2-1 for about 40' long also not affecting travel or air. Told by MSHA Inspector, did not need to call hot line.

While on preshift employee went to check something out in return and found roof down at xcut 257 in #1 Entry and also at xcut 258 in 2-1 xcut.

While positioning his body to bolt felt pop in his knee continued to work until 1/23/13 had to have surgery on 1/24/13 making this a lost time accident.

At around 4:50 a.m. the intersection in A-2 fell in from 1 xcut inby D-1 three intersection.

While loading straps onto bolter felt a sharp pain in lower back.

While hooking up trailer to Johnson piece of draw rock meas. 2"" thick x 3' long struck him in the shoulder causing him to slip and drop trailer tongue on his hand.

While installing machine doors along #7 travelway, hook slipped off frame work letting door fall. Doors fell onto right leg cutting his leg resulting in 8-10 stitches.

After completing installation a 480V circuit from the main 1000KVAJ4940/480V PA approved substation approval #MM - S82-11. The junction mounting block inside the 480V cable trough exploded when the last of the 3 25amp 25,000V fused disconnects were engaged.

While training back drill pipe onto the rod truck, the EE lost his footing on the pipe laying on the truck. He fell onto the standard on the bed of the rod truck. He sustained bruising to his ribs on the right side.

While making return examination discovered roof fall in 5SW butt 4 crosscuts outby face all 5 entries and crosscuts unknown as to length of fall. Section was idle at the time.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED ALONG THE #4 ENTRY OF 5 SOUTH WEST BUTT WHEN THE MEN WERE TRAVELING TO THE WORKING SECTION

While mining a borehole pushed coal away from rib & poured water into section

While going to answer page phone slipped on piece of rock & fell & twisted his back.

While sitting in scoop giving instructions on what men were to do piece of scale fell from between bolt striking him across face & hand.

On April 11,2011 while moving rocks out of the way of the bolter EE twisted his back. EE went to chiropractor had an adjustment did not miss work. EE's back did not get better had to get a MRI then had to have surgery on his back on 5/9/11.

While installing 6"" yellow mine pipe, felt something pull in his back on March 25th, did not think anything was wrong. Did not miss any work, then on April 22 had to have emergency surgery for a ruptured disc.

Unknown. Woke up and rt. knee swelled up. Went to doctor. Said he had an infection in knee. Got medicine. Helped but had to be put in hospital to get IV to get it fixed up.

While crawling to return, piece of draw rock fell from roof striking him on the right hand breaking Rt. index finger.

While making weekly return run discovered roof fall in 8th S.E. butt. Fall blocking normal travel, no interruption of ventilation. Fall approx. 2700' from working section.

While making return air course weekly examination, discovered unintentional roof fall in the # 1 entry of 2nd N.W. Mains from SS # 3750 extending approx. 180 ft. inby in front of #2 & #3 seals of west butt.

While loading the rock duster a piece of rock fell and hit the EE's left knee.

While making a pre-shift examination discovered roof fall along #6 belt line at way # 246 and extending inby aprox 75'.

While installing lifeline along belt loop of line came in contact with roller. He was holding onto rest of line and it pulled his arm into roller.

Around 1:00pm men working in section heard rock falling upon investigation found a roof fall inby S.S. 5965 that was appro. 180' long x 20' wide and 5' to 6' high

While putting starter steel into drill head, piece of rock fell from roof, hitting him on left hand and broke his hand.

While mining a piece of rock fell out between bolts and hit injury in the back. Rock was approx. 3'x4' wide and 0""-5"" thick,

While tramming scoop to charging station, went thru wet area along travelway splashing mud & water into eye. Flushed out, started to bother him on 5-17-09. Went to Dr.

While taking rock out of coal chain had rock on edge ready to drop off coal chain. Started catching hand between roof and rock cutting end of thumb & index finger

Unknown shoulder strain did not require medical attention at the time but since then has had to have surgery on the shoulder on 8/21/09.

Roof fall occurred in the 5th south eat butt belt line starting 2 1/2 crosscuts inby ss# 5458 and extension approx. 100 feet.

Unintentional roof fall in belt line from SS# 3854 Inby, approx 200 feet.

While making weekly return examination discovered roof fall in return of 2nd north west mains 1 crosscut inby station #5297.

While retrieving structure in room set-up heard rock falling & found #6 room in 7th East butt had fallen in for 3 x-cuts. Built cribs & dangered off all access points to #6 room.

While changing steels got hand caught between steels.

While making preshift exam of the 6th east bott. EE found roof fall in the intersection or roundabout, dangered off, built cribs, set posts and installed some bolts.

While making a pre-shift examination of a old set of rooms employee found a roof fall around 4:30 am at intersection of #12 room and x cut 12-11 and part of x cut 12-13. Set posts and cribs and dangered area off.

When operator went to lower drill canopy, it stuck. Tried several times to lower it. When it came loose he was not completely under it. Struck him on brim of hard hat and nose - breaking his nose.

Roof fell for a crosscut 6-8' high in return. No impairment to ventilation. Set posts and cribs. Dangered off area.

While turning personal carrier around hit head in low area and was drug off machine.

While getting into mobile bridge EE twisted knee and heard something pop.

Roof fall.

Installing Cat pad pin with hammer when piece of pin came off striking man in left eye.

Injured was attempting to attach sling to dolly, slipped and fell against dolly, striking head on same.

Driving pin out of eyelet with a hammer. The hammer glanced off of the pin & hit the employee on his left hand. The EE did not seek medical treatment until 10/23/06. This is not a lost time accident to date.

Employee reported knee was sore on 7-17-2006 and was sore since 7-11-06. Did not miss work until 9-6-2006. First doctor visit for knee 9-1-06. Continuous crawling.

Man was lifting cables into conveyor trough; felt pain in right shoulder. No lost time.

Ee was tightening tail jack he let go of jack handle latch on jack was not latched and handle came back and struck man on head.

Injured man was operating continious mining machine when piece of draw rock fell from between roof bolts striking him on right ankle and foot.

MAN WAS MOVING SCOOP, TURNED, SCOOP CAUGHT LEFT HAND BETWEEN RUBBER STOP AND OUTSIDE OF OPERATOR'S COMPARTMENT.

EE WAS MOVING BOLTER CABLE. HIS RIGHT RING FINGER WAS SQUEEZED BETWEEN THE CANOPY POST AND THE CABLE, CUTTING HIS FINGER. EE RECEIVED 4 STITCHES.

MAN WAS INSTALLING ROOF BOLT IN HIGH SPOT, FELT PAIN IN LOWER BACK. 1ST DAY OF MODIFIED DUTY AND PRESCRIBED MEDICATION WAS 4/26/05.

Injured went home. After sleeping right knee swollen.

EE WAS ADDING PULLER STEEL. HE CAUGHT HIS HAND BETWEEN STARTER STEEL AND FINISHER STEEL, PUNCTURING HIS LEFT HAND. NO LOST DAYS.

INJURED WAS IN MUD AND TWISTED AS HE MOVED SPRAINING KNEE. WHEN MAN CRAWLED KNEE WOULD SWELL. INJURED'S FIRST DAY OFF OF WORK WAS 5-6-04.

MAN WAS INSTALLING ROOF BOLTS. LOOSE PIECE OF ROCK FELL HITTING MAN ON LEFT SIDE. FIRST DAY OFF WORK 1-30-04.

INJURED WAS REMOVING ROCK FROM THE MOBILE BRIDGE. AS HE ATTEMPTED TO FLIP ROCK OUT HE SLIPPED TWISTING RT ANKLE AND FELT PAIN IN LEFT SHOULDER AND HIS RIGHT ELBOW STRUCK MOBILE BRIDGE.

EE WAS RISING TO FEET AFTER CROSSING HAULAGE UNIIT, FELT SHARP PAIN IN LOWER BACK.

REOCCURENCE FROM INJURY DATED 1-29-02, 10-26-2000, AND 9-29-2000. MAN WAS WALKING TWISTED KNEE.

WHILE SERVICING MINER, EE PICKED UP A BUCKET OF MINER BITS AND A CAN OF OIL, CAUSING HIS BACK TO TWIST AND SNAP OUT.

MINER WAS CHIPPING ROCK WITH HIS HAMMER. A PIECE FLEW BEHIND HIS SAFETY GLASSES STRIKING HIM IN HIS EYE.

HIS KNEE SWELLED UP ON THE 15TH. MINER FIGURED THE SWELLING WOULD GO AWAY ON ITS OWN. SWELLING DID NOT GO DOWN SO HE WENT TO THE DOCTORS ON THE 24TH THATS WHEN I WAS TOLD.

EE WAS USING A CUTTING TORCH AND A SPARK ENTERED HIS EAR. ALSO EE WAS PULLING ON A BOLT CAR AND SPRAINED HIS FINGER.

THE HEAD OF THE BOLTER WAS LOWERED IN AN ATTEMPTTO REMOVE THE BOLTER CHOCK, EE'S KNEE WAS UNDER THE HEAD.

ROOF CONDITION IN THIS AREA ROCK STRATA. LITTLE DAMAGE TO THE CONTINUOUS MINER AND BELT LINE. NO WORKERS IN THE MINE AT TIME OF INCIDENT.

TWISTED HIS KNEE CRAWLING TO A PIECE OF EQUIPT.

LIFTING SMALL PIECE OF EQUIPMENT WHEN IT SLIPPEDPINCHING 2 FINGERS.

ACCIDENTLY HIT HIS BACK ON AN OVERHANG

BOLTING, CRAWLING, FLUID ON KNEE.

LOADING STRUCTURE ONTO SKID UNLOADING ROCKDUST.

SUBJECT WAS CUTTING A SHAFT.

NECK WENT OUT OF PLACE WHILE SHOVELING.

THE MAN TWISTED AROUND TO LOOK AT HIS MACHINE, WHEN THE PAIN IN HIS BACK OCCURRED.

THE INJURED PERSON WAS WORKING ON CONTINUOUS HAULAGE SYSTEM WHEN HE TWISTED HIS RIGHT LEG AND TEARING LATERAL LIGAMENT IN HIS RIGHT KNEE.

WAS INSTALLING LOW BELT STRUCTURE, WHILE BELTING UP. THE TOW CHAIN BEING USED TO PULL TAILPIECE UP, BROKE AND CRUSHED INJURED EMPLOYEE'S FINGERSBETWEEN TAIL PIECE AND STRUCTURE, THAT WAS BEING INSTALLED BEHIND TAILPIECE, RESULTING IN 2 BROKEN FINGERS AND 1 BADLY BRUISED.
