While traveling outby in return to get low belt he ran over a rock and roofed out.

Before installing first row of bolts he examined the roof and saw a loose piece of rock and grabbed his rock bar. While scaling the loose piece a piece of rib/ rock above him came down and he was moving away from it when the piece he was scaling fell and pinched his hand between the rock and the floor.

While backing bolter out of a cut there was some rock laying on the mine floor the back of the bolter pushed a piece of the rock against the rib. When the rock hit the rib it came up and cut his ear with a sharp edge, he received stitches.

At around 4:50 a.m. the intersection in A-2 fell in from 1 xcut inby D-1 three intersection.

While hooking up trailer to Johnson piece of draw rock meas. 2"" thick x 3' long struck him in the shoulder causing him to slip and drop trailer tongue on his hand.

While installing machine doors along #7 travelway, hook slipped off frame work letting door fall. Doors fell onto right leg cutting his leg resulting in 8-10 stitches.

After completing installation a 480V circuit from the main 1000KVAJ4940/480V PA approved substation approval #MM - S82-11. The junction mounting block inside the 480V cable trough exploded when the last of the 3 25amp 25,000V fused disconnects were engaged.

While training back drill pipe onto the rod truck, the EE lost his footing on the pipe laying on the truck. He fell onto the standard on the bed of the rod truck. He sustained bruising to his ribs on the right side.

On April 11,2011 while moving rocks out of the way of the bolter EE twisted his back. EE went to chiropractor had an adjustment did not miss work. EE's back did not get better had to get a MRI then had to have surgery on his back on 5/9/11.

While installing 6"" yellow mine pipe, felt something pull in his back on March 25th, did not think anything was wrong. Did not miss any work, then on April 22 had to have emergency surgery for a ruptured disc.

Unknown. Woke up and rt. knee swelled up. Went to doctor. Said he had an infection in knee. Got medicine. Helped but had to be put in hospital to get IV to get it fixed up.

Driving pin out of eyelet with a hammer. The hammer glanced off of the pin & hit the employee on his left hand. The EE did not seek medical treatment until 10/23/06. This is not a lost time accident to date.

Employee reported knee was sore on 7-17-2006 and was sore since 7-11-06. Did not miss work until 9-6-2006. First doctor visit for knee 9-1-06. Continuous crawling.
