Employee was exiting loader and missed the bottom step injuring right elbow and shoulder. Employee worked regular duty up until 8/21/24. First day off work is 8/22/24.

Employee fell to ground while exiting loader landing on both feet. Several days later while exiting loader, ladder broke and employee fell to ground landing on buttock. Employee continued to work full duty until 09/13/24. 1st day of lost time is 9/14/24.

Employee was walking from the truck in the truck parking area to the mine office when EE tripped and fell on uneven ground.

Walking to the truck from the load out area when EE tripped over a mesh screen and fell.2

During repair to coal vessel, employees hand was pinched between flange & coal chute.

Exiting loader, missed last 2 steps of ladder, held on with Rt hand and feet sharp pain in Rt shoulder.

Fueling crusher - slipped climbing down ladder. 1st day of missing time 1/3/17

Lift parts into a pickup truck. Lifted large bearing and felt pain in back.

Welder shut off. Checked fluid levels to see the reason. Burned with the radiator fluid.

Removing rear axle from Mack Dump Truck - axle dropped crushing lt hand between it & truck frame.

Installing 14 inch plastic pipe. Pushed on pipe and felt pain/heard pop.

Employee was climbing ladder to get into truck. Bottom of ladder swung in at emp. Strained wrist/hand on grab rail.

On 2/4/2015 employee twisted knee getting out of loader. Did not seek medical treatment until 02/25/2015.

Employee was climbing down ladder of RR car. Fell and landed on shoulder. Worked until 8/29/13. First day off 8/30/13.

Employee was cleaning out RR car - slip & fell while inside car hitting back and side on i/s of car wall

Welder without on stangless with 7018 rod.

EE was hoisting 2 wet coal screws off groung & caught finger in between.

Rolling large rock off haul road and felt pop in right shoulder.

EE was shoveling from the portable plant and slipped and fell striking his chest against the steel frame.

Emp. was walking out of pump house & stepped in hole on uneven ground & twisted knee.

Employee drove through pot hole. Steering wheel spun and struck finger.

This employee was jack hammering ice & frozen material from a crusher when he was struck in the left eye with a piece of debris.

Several miners were assigned to clean the #6 belt line. When the conveyor was started one miner continued to clean around the head pulley with a 4' bar. The bar was pulled into the pulley & the end of the bar struck his right hand causing injury.

Emp. was walking on snow covered ground when he slipped & fell to the ground.

Employee was cutting rubber belting & utility knife slipped & cut rt thigh.

USING SLEDGE HAMMER TO DRIVE WEDGE OUT OF HOIST ROPE SOCKET.

EMPLOYEE WAS DEICING BELT LINES @ START OF SHIFT & SLIPPED ON ICY STEPS.

SIGNIFICANT THRESHOLD SHIFT OF 12 DB SINCE BASE LINE TEST ON 9-6-00.

EMPLOYEE WAS LOADING PARTS WHEN WING BLEW F/B INTO RT EYE CAUSING 2 SCRATCHES TO RETINA. THIS OCCURRED ON MINE PROPERTY @ A MACHINE SHOP WHERE THE PARTS WERE LOCATED.

EMP WAS ADJUSTING DRAG BRAKES ON 4600 WHICH CONSISTS OF TURNING LG TRAVER NUTS W / LG WRENCH. WHILE PUSHING & PULLING ON AIR WRENCH HE FELT SUDDEN PAIN IN ABS & UPON INVESTIGATION SAW THAT HE SUFFERED A HERNIA IN THE AREA OF HIS NAVEL.

WHILE CLEANING OUT RAIL CAR & PUSHING GATE CLOSED, LEFT KNEE CAP SHIFTED & MOVED TO THE LEFT.
