Struck against stationary object

While traveling along C2 drive on his Johnson, the employee struck his left arm on a wiper bar. This resulted in a dislocated elbow and fractured arm.

A roof fall occurred on the H-2 belt line at #35 crosscut. The fall was approximately three foot high and twelve foot by twelve foot in length located towards the tight side of the belt.

Accident type, without injuries

While examining the return, the mine foreman found a roof fall. The fall was at an intersection where spade 2329 is located. The area was bolted with six foot super twist bolts. The fall is approximately six foot high. The men were withdrawn out-by. The area was timbered and danger off.

Struck against stationary object

While traveling in the belt line on a Johnson stinger, the employee ran into a roof jack which caused a laceration above the left eye. This resulted in seven sutures.

An assistant mine foreman found a fall in the return at E-mains station spad 2333. The fall was at an intersection measuring 18' x 55'. The height of the fall was 9'. The area was dangered off and supported.

While installing a six foot bolt and plate, the employee pinched his thumb between the roof bolter head and roof bolt plate. This resulted in sutures to the left thumb.

While examining the intake, the mine examiner found a cave twenty-five foot outby spad 19893 between four to five entry. The cave measured sixty foot long and six foot high. The cave area was supported with four foot fully grouted bolts. The area was timbered off and abandoned. All work was done according to the approved plan.

Struck by falling object

While bolting, a piece of scale fell and rolled off the canopy striking the employee on the left foot. This resulted in a hair line fracture to the left foot.

While examining the return, the employee slipped on smooth bottom covered in red boy. This resulted in a labral tear to the left shoulder which will require surgery to repair.

While shoveling coal spillage at M2 tail, the employee felt a sharp pain in his lower back. The employee continued to work until 3/26/2013. He was diagnosed with herniated disks. This resulted in a lost time accident.

While shoveling coal spillage, the employee felt a pain in his groin area. The employee continued to work until 2/25/13. He had surgery on 2/26/13, this resulted in lost time accident.

Fall onto or against objects

While preparing to make a splice on B-1 belt, the employee tripped and fell onto the belt square that was in his left hand. This resulted in a puncture wound and one suture to the webbing on his left hand.

Caught in, under or between a moving and a stationary object

The employee pinched his right index finger between the nut and the short arm on the Fletcher bolter. This resulted in nine sutures.

Struck by falling object

While crawling to the MBC, the employee was struck on the right shoulder and arm by a piece of roof scale that fell between the rib and the adjoining bolt. The employee continued to work until July 14, 2015, at which time he underwent surgery.

While trying to loosen the bridge pin on the miner, the ratchet slipped and the employee fell against the conveyor striking his right shoulder causing a shoulder strain. The employee continued to work until 2/11/13. He had surgery on 2/12/13. This resulted in a lost time accident.

While adjusting the head on the Fletched bolter. The employee pinched his left index finger. This resulted in four sutures to the left index finger.

While traveling from three to four entry, the employee struck a low hanging roof plate on his right neck. This resulted in five sutures from a laceration.

A roof fall occurred in # 3 entry, 8xc of J Mains and meas. 25' in L. 40' W X 7' H. The fall was found on a preshift exam. Two pre-shifters were U.G. when cave was found. Entrances to cave were timbered and dangered off. Ventilation was not interrupted. Men were instructed on alt. escape way. Roof was bolted with 4' fully grouted resin bolts and 12' cable bolts.

While using a four way wrench to loosen a tire nut on his Johnson Mantrip, the EE felt a pain in his upper left arm which resulted in a torn bicep.

A roof fall occurred in the J1 butt section in the #3 entry 75ft. inby survey station 2682. The fall measured 20ft in width and 95ft in length, it was approx 7ft in height. Fall was found on a pre shift exam. No men underground when cave was found. Entrance to cave were timbered and dangered off. Moved to location 300ft outby cave area to resume mining in J1 butt rooms.

While tightening belt structure, the wrench that the employee was using slipped, causing him to fall forward and strike his chin on the belt structure.

While come-a-longing belt structure together, a chain broke causing the employee to pinch his left index finger. This resulted in a laceration and sutures.

While walking bent over in the intake, the employee struck a cable bolt plate, which resulted in three sutures to his right lower eye lid.

The section foreman noticed that the roof began working approx. 55' inby station #2336 located 1 1/2 cross cut outby the face of #2 entry. Additional roof support was installed, however, it caved on a battery scoop for approx 60' and 12' high. Additional support was added and accesses to the fall were dangered off. Work is being done according to the approved plan.

While moving the DBT twin bolter, a piece of rock fell from between the rib and the first bolt striking the employee on the shoulder. This then pushed his head into the bolter which resulted in a forehead laceration with sutures.

While traveling to start a belt that was down, the section foreman noticed a fall had occured. This was in the 1st southwest mains belt entry, two crosscuts inby spad 1836. Approx 20' wide x 30' long x 9"" high. The area was cribbed, timbered and bolted. All accesses to the fall were dangered off. Vent tubing was humg to divert gas to return.

While examining the mining machine for water leaks, the employee dropped the pan of the miner on to this left leg. This resulted in a fracture to the left femur and torn muscles.

While installing a roof bolt, a piece of roof scale fell onto employee's right hand causing a laceration to the end of the 3rd digit. This resulted in seven sutures. There was no lost time.

During a daily exam, a fall was found in the northwest main belt line (section of the mine is idle) adjacent to spad 325. All accesses to the fall were timbered and dangered off. The fall was about 20'wide, 40' long and 6' high.

Employee had finished one row of bolts. and was examining roof preparing for second row. When a piece of scale measuring 15""x13"". 0-3"" thick fell on employee causing a laceration to the back r/s on head. This resulted in 16 staples to close the wound, and neck. back soreness.

During a weekly aircourse exam a fall was found in the North/West main return, adjucent to spad #430. All access to the fall were timbered and danger signs hung. The fall measured 20'wide, 20'long and 6'high. Does not affect the quality of air.

Plastic 2""NPT nipple on chem stream tank (chemical for treating mine water) broke. Chemical splashed on face and legs, washed face w/mine water, then took shower. Treated in trama unit (eyes flushed). Chemical burns of face, legs and mouth, left scarring. All stainless steel hardware has been installed on holding tank.
