Employee was putting hydraulic oil in dozer tank. Oiler was handing hose to employee and valve was accidently hit which opened ball valve. Oil sprayed out under high pressure cutting glove and hand.

Rock wedged between oval wheels; putting chain around rock to pull it out, a tire blew up and broke pinkey finger.

Drove truck down into fuel area for unknown reason.

Working on bearing (was spring loaded), cut lock nut off, hit impact wrench, spring came loose, impact wrench sprung back and pinched his thumb between wrench and motor.
