Employee was removing accumulated metal from the magnet. During the removal of a large piece of metal, the angle iron shifted and struck the employee on the left side of the head below the hard hat brim. Employee sustained a laceration requiring a staple and a tetanus shot. The next morning, the Coworker reported dizziness and nausea and was diagnosed with a concussion.

EE was cleaning rock using pry bar. Pry bar slipped, pinched left thumb between bar and framing, resulting in left thumb contusion.

EE was trying to remove a tree that was stuck at the underside of a crusher, on top of a belt. While pulling on the tree, EE felt pain on the right side of abdomen, and the injury resulted in a strain. The injury did not meet reportable criteria until after EE's follow up visit on 01/22/2024 when ee was issued a work restriction of no shoveling.

The tailgate latch was stuck, and EE went to get it unstuck. In the process of doing so, the tailgate went shut and EE's finger got caught in it.

Employee was installing a new 20HP electric motor on the #3 conveyor. A 3/4 ton chain hoist was being used to lift the motor into place. The safety latch on the hook failed resulting in the motor striking the employees left forearm.

Injured was cutting tree's for firewood and a wind gust caused the striking tree to fall in a unexpected direction thus striking the tree cutter. Injured was not an employee of the mine.
The employee was assisting a contract welder who was working in a feeder area of the plant. The injured employee was acting as a spotter in the event that the welder needed help due to working in a confined space.

Employee fell thru a elevated walkway when the steel grating approx. 22"" wide and 36"" long he stepped on dislodged. the walkway was supported on both side with 3""x3"" angle iron and was adjacent to a crusher. It appears the weld broke as a result of vibration.
Installing replace ""shoes"" on VSI crusher and one fell from cart, used to transport them, onto his foot.

Employee was shoveling from under belt for several hours before pain & tingling in hands/fingers began. He did not report any problem until 7/5/06, we then sent him for a medical examination. *Possible carpal tunnel syndrome.
