Fall to the walkway or working surface

Employee had help prying an unbolted and stuck point rail, with a slate bar, and as the slate bar became dislodged out of point rail the employee's left foot stayed planted on ground as EE was falling backwards. Afterwards the employee heard something snap in EE's left ankle.

Employee stated EE was trimming a piece of belt when EE's knife slipped out and made a puncture/laceration to the to the right upper kneecap. Employee sought medical attention and received 4 sutures.

Employee stated they were in the process of building a crib, when they felt pain in their back. Employee sought medical attention on 3/1/2028 and was taken off work.

Struck against stationary object

Employee was in the process of putting the bedrail into the structure and accidentally got EE's finger caught on a burr in the structure causing injury (stitches).

Fall to the walkway or working surface

Employee stated EE was walking to the shop and slipped and fell in mud located at #9 bottom. Employee was taken to the hospital and was diagnosed with a broken ankle. This incident is being reported as an LTA.

Over-exertion NEC

The employee stated EE went to step off a diesel motor and felt pain in EE's back. Employee sought medical attention and was taken off work.

Caught in, under or between a moving and a stationary object

Employee stated they were loading rollers onto a flat car when they lost grip and the roller smashed their ring finger between the roller and the mine floor. Employee sought medical attention on 11.3.25 and was taken off work. This incident is being reported as an LTA.

Fall onto or against objects

Employee stated they slipped on the roof bolter platform, struck the canopy with left eyebrow. The employee sought medical attention and was given 3 sutures. The incident is being reported as medical.

Inhalation of radiations, caustics, toxic and noxious substances

A miner was diagnosed with pneumoconiosis.

Accident type, without injuries

On 10/8/2025, a D10 Dozer inadvertently entered an underground feeder opening on our raw coal stockpile, trapping the employee inside the dozer.

Over-exertion in pulling or pushing objects

The employee stated EE was in the process of sliding a tube back to install a roof strap when EE felt something pop in EE's arm. Employee sought medical attention and was taken off work on 10/25/2025. This incident is being reported as an LTA.

Over-exertion in lifting objects

The employee stated EE was in the process of hand loading a 6-foot bolts in a scoop bucket and felt pain in EE's left arm. The employee sought medical attention and was taken off work. This is being reported as an LTA

NEC

Employee was found unresponsive. Emergency services were immediately notified. live saving measure were taken on site. Employee was taken to medical center via ambulance. EE was pronounced dead at 5:23AM at the hospital.

Struck by... NEC

Employee was cutting up old belt into small pieces to be able to remove. EE's knife blade broke while cutting towards self and EE struck EE's right leg just above EE's knee. EE received six stitches.

The employee stated that they were pushing a bolt in a hole when a small piece of rock fell, striking the employee's tooth. The employee sought dental attention and had the tooth repaired.

Over-exertion NEC

The employee stated that EE was walking down the entry to tighten cribs when EE stepped into a hole, twisting EE's knee. This injury became reportable on 9/26/2025 when the doctor took EE off work.

Rubbed or abraded

Laborer was torquing bolts on a check valve to spec. Their hand slipped off wrench scraping finger on a bolt.. possibly leaving a piece of metal in the wound. Laborer not realizing. The piece of metal was in their finger. Let it go over the weekend, causing an infection.

The Employee stated EE went to pick up a solid block, and as EE went to step, EE felt pain in EE's knee. The employee sought medical attention and was taken off work.

Accident type, without injuries

The slope hoist car was out of service due to a damaged brake control cable. The cable has since been repaired, and the hoist is now operational.

Struck against stationary object

Employee stated EE went to turn and talk to a fellow employee, and in that process, EE hit the side of EE's foot on a pontoon and fractured a bone in EE's foot. The patient sought medical attention and was diagnosed with a fracture and taken off work. This incident is being reported as an LTA.

The employee stated that while crossing the North Mains belt, foot slipped on the crossover, causing them to fall and pinch their knee. The employee sought medical attention and was taken off work on 8/6/2025. This is being reported as a LTA.

The employee stated EE was dragging a green discharge line when EE felt a pop in EE's back. Employee sought medical attention and was taken off work on 5/21/2025. This incident is being reported as LTA.

The #9 elevator door switch failed

Struck against stationary object

The employee was shoveling along the 79 Mains North belt when EE's left thumb came in contact with a bottom roller, causing a laceration. The employee sought medical attention and was given eight sutures and taken off work. This is being reported as an LTA

Employee stated they were operating a diesel motor when a drop of water came into contact with their left eye behind safety glasses. The employee sought medical attention after their shift and was prescribed medical eye drops.

2 employees were ascending in a man-lift bucket to change rollers on the slope belt. The operator had to tram forward to clear a catwalk, and one wheel of the man-lift sunk in soft material, which caused the lift to tilt. The lift shut off automatically, and the employees were stuck until lowered by the FD.

The employee was in the bolter station when the miner advanced a piece of rock that got caught between the back support and the rib, causing the jack to yield due to the bolter back stop not fully extended, causing the employee to sprain their left ankle. The employee was taken off work on 4/30/2025. This is being reported as an LTA.

The employee stated employee was walking down the CW3 belt when employee slipped and fell and, in the process, heard something pop in knee. The employee later sought medical attention and was taken off work on 4/10/2025. This incident is being reported as an LTA.

Employee stated EE was trying to line up a strap in a strap jack when the employee on the other side pushed the strap, this caused the strap to cut a small end of EE's thumb off.

Struck by rolling or sliding object

Employee stated employee was bolting off the miner on 78MS when a piece of rib rolled out and struck the employee on top of the foot. Employee sought medical attention and was diagnosed with a fractured foot.

The employee stated was operating a motor down 6W haulage when some rock dust fell from the top and made contact with employee's eye. Employee sought medical attention and was prescribed medicated eye drops.

The employee unhooked a car from the hoist car, turned to step down from the ramp, and slipped on a piece of rail, falling on their back and twisting their knee. The employee was taken off work.

Employee was assisting a roof bolter operator backing down the entry when the cable fell off the tray. The employee repositioned the cable and, while walking back to the front, was struck by the ATRS pad in the shoulder.

The employee was removing an electric motor from an EMU in the Underground shop and felt pain in EE's right shoulder. Employee sought medical attention and was taken off work on 5/1/2025

Employee was tightening allthread on steel set and had hand on wrench. The impact gun tightened on bolt and the wrench accidentally swung around and caught finger between wrench and steel set causing injury.

At 3:50 am a roof fall was discovered on Cumberland West 1 belt at break 35.5. The fall was approximately 16ft wide x 15 ft high x 25ft long.

The IE felt a piece of coal/stone hit L.hand from an elevation while hoisting a new siding panel with a rope & pulley. Prior to removing the old siding, the floors were cleaned near the panels to be removed. Not certain how/from where it fell. The whole bldg vibrates and there is a lot of area. Non-displaced transverse fracture of the 1st metacarpal. Restricted duty 4-6 weeks.

The employee reported that while operating a motor, a piece of rock dust fell from above and got behind their safety glasses, causing irritation. The employee received medical treatment and was taken off work. This incident is being reported as an LTA.

The employee stepped on a piece of coal and twisted EE's ankle and knee. The employee sought medical attention and was taken off work due to a fracture of the ankle on 8/2/24.

The employee stated: was sitting when a piece of rock fell and struck the employee in the head. The employee sought medical attention and received 5 staples. This incident is being reported as an LTA.

The employee stated was attempting to dismount the crossover at the stage loader crossover and slipped, falling 2 1/2 feet to the ground. As EE was falling, EE reached out to catch self and strained their arm. The employee sought medical treatment and was taken off work. This incident is being reported as an LTA.

The employee stated they were in the process of throwing cribs from a pile when a co-worker threw a crib, smashing their right finger. The employee sought medical attention on 5/29/2024 and was taken off work. This incident is being reported as a LTA.

The employee stated they were in the process of hanging vent tubes when the rope slid off, causing the tube to strike the employee. The employee sought a medical specialist on 5/21/2024 and was taken off work on 6/3/2024. This incident is being reported as a LTA.

Employee stated they were carrying a bag of rock dust when they stepped into a rut and they felt a pain in their left knee.

Number 6 shaft elevator had multiple error codes and management began trouble shooting the elevator and discovered water had entered into the ESTOP switch on the bottom landing of elevator.

The employee was operating a motor when the plastic windshield flew up and struck the employee in the face. This incident became reportable on 3/5/2024 when the employee sought medical attention (dentist) and was taken off work.

Employee stated: stepped off the 650 duckbill and twisted rolled left ankle. Employee had MRI on the ankle on 3/4/2024 and EE has a partial tear of the ligaments.

The #9 shaft elevator had a brake fault and initially we thought it was the encoder but found it to be the Red Lion cable.

Employee stated was removing the monorail when the chain unhooked and fell striking EE in the right shoulder. Employee started missing work on 10/25/2023.

Employee stated: had pulled through the #2 intersection and stopped to get cable and a shuttle car hit the back of deck.

A roof fall was discovered at 15 crosscut on North Mains belt in our old B haulage entry. The size of the fall was estimated to be 120 ft long x 16 ft wide x 10ft thick.

Employee stated had been carrying pumpable crib bags through water when EE's legs got wet creating a rash on their legs. Employee reported this to management the following day.

Employee stated was changing belt rollers when EE pushed down with bar. EE's finger was smashed between the belt rail and bar.

Employee stated was lifting a concrete block when EE felt a pain in left arm above the elbow.

Employee stated EE was in the process of hooking up a winch to pull belt. When EE raised up a highline hook caught EE's ear cutting EE's ear. Employee received 8 stitches in EE's left ear.

Employee stated they were uncoupling a dust and the hose still had pressure on it and came off the end of pipe causing them to stumble backwards and strain their back. Employee started missing work on 4/20/23

Employee stated employee was hosing and pumping muck when employee stepped off bridge and foot got stuck. Employee pulled leg out of muck and felt a pop in right knee.

Employee stated they were throwing belt structure under the bottom belt and pinched their pinky finger on belt stand. Employee received stiches making this a reportable injury.

Employee stated EE was picking up a roll of cable and the plastic wrap was under EE's left foot causing the roll of cable to be hard to lift straining in EE's back.

Employee stated they were scaling down a piece of rock and it hit the canopy and slid off the canopy striking them in the upper part of their body.

The employee was standing beside the miner when a piece of rock fell striking employee on the head. Employee sought medical attention and was taken off work on 1/18/2023.

Working on diesel master cylinder and went to grab a hose being fed to them a piece of metal punctured their left hand between thumb and index finger. Employee received 3 stitches.

Working on control power switch and could not get car back running. Car was down 158 minutes.

Fall onto or against objects

Employee slipped and fell in an area leading to a sampling point. When they fell they hit their head on a metal beam discarded in the area. The impact of the fall caused a cut on their ear. There was a mat in the area that caused the employee to slip when they stepped on it. The area has been cleared and the owner has put down gravel making for a safer path of travel to the sample

Employee stated they were lifting a box of glue and it started coming and they attempted to grasp the sticks of glue and felt a pop in the middle of their back.

Employee was performing normal work tasks on a surface crib site, when employee got a small amount of bulk pre-mix dust in their left eye. This incident did not involve any aspect of compliance with rules & regulations; mine equipment or mining system; job skills & miner proficiency, training & attitude; or protective items relating to clothing, or protective devices on equipment.

Employee stated a piece of rock fell from the top striking them in the left shoulder and head. Employee started missing work on 12/28/2022.

Was mining in the #1 Face on 76TG and mined into a methane degas hole on right side of entry and methane became over range in the face.

Employee stated was stepping off a D9 dozer, slipped and fell on dozer blade. Employee sought medical treatment and was taken off work on 10/27/2022. This incident is being reported as a LTA.

The employee stated: got off of motor to throw a switch, after the switch was thrown, employee went to get back on the motor and as the employee went to set down, employee sat down on butt wrong causing a contusion. The employee stated was picking up a plank and felt a pop in their shoulder. The incident was not reported to mine management until 5/6/2022. This incident is being report as a LTA.

The employee stated they were in the process of throwing a super post under the pontoon of a shield when, in the swinging motion, right middle finger got smashed. Employee sought medical and suffered a fracture and required sutures. Incident is being report as a LTA.

The employee stated was walking toward longwall face when stepped in a rut and rolled ankle. Employee sought medical attention and was taken off work. This incident is being report as a LTA.

Employee stated tripped on a burred plastic band at the end of the track. Employee sought medical attention and was diagnosed with a fractured knee cap. This incident is being reported as a LTA.

The employee was changing a hose when the hose blew apart and emulsion fluid got into employee eye. The employee sought medical attention and was taken off work. This incident is being report as a LTA.

The employee swung an ax and missed the crib and hit their shin, causing a laceration. This incident is being reported as medical due to having to get three sutures.
