Caught in, under or between a moving and a stationary object

A non-employee caught their fingers in the tailgate of their dump truck while cleaning it prior to getting loaded with stone. The local EMS was called to transport the non-employee to a local emergency facility for treatment. The driver works for Rittenbaugh Transport.

As the EE was helping to install a pin to hold the grapple onto the equipment another EE was using a sledge hammer to pound the pin into place. The hammer glanced off of the pin striking EE in the face causing a contusion on the R side of face.

Installing hose under loader above the belly pan, felt strain in left shoulder. This injury was not reported to me until November 5, 2009. I could not report this to MSHA until it was reported to me.

Employee was descending steps from truck after parking and twisted his right ankle upon stepping onto the ground.

Employee was repairing safety latch for crusher. The latch was positoned in a vice. When he loosened the vice the latch slipped out and caught his left middle finger between the latch and work bench.

EE WAS OPERATING A CAT 988B LOADER, DIGGING A NEW RAMP IN VERY ROUGH MATERIAL. EE FELT PAIN IN HIS LOWER BACK AFTER BOUNCING IN THE SEAT FOR SEVERAL HOURS.

WHILE INSTALLING A PULLEY FOR A BELT IDLER, THE WRENCH SLIPPED CAUSING THE EMPLOYEE TO HAVE HIS FINGER CAUGHT BETWEEN THE BELT AND THE IDLER PULLEY.
