Employee was tightening a centerline cable for a dredge boat. Weather was clear temperature around 70 degrees Fahrenheit. As the employee was gaining tension on the centerline cable, the sideline cable the centerline was attached to was released due to a clamp failure. This caused the centerline cable and associated puller to strike the affected employee in the left leg.
Employee was mowing the grass and brush on berms. While moving the mower across the top of a berm towards the cab the blades struck a roof bolt that was lying horizontally under the brush. The roof bolt traveled end first through the windshield and struck ee in the tip of left thumb. It came to rest across lap.
EE STATED HE SLIPPED OFF OF D-8 DOZER STEP AND INJURED RT. ANKLE.
EE WAS DIANGNOSED WITH CARPAL TUNNEL SYNDROME ON6-26-2002 AND INFORMED MANAGEMENT ON 8-7-02 THATIT WAS WORK RELATED. BEGAN MISSING WORK ON 10-2-02.
EE HAD FUELED CRANE TRUCK AND LOST HIS BALANCE, FALLING AND STRIKING HIS BACK ON CONTAINMENT WALL.
EE WAS DRILLING A HOLE THROUGH BRACKET AND LOST BALANCE CAUSING LT WRIST TO BEND UNDER DUE TO THE LAPSE OF TIME BETWEEN THE ALLEGED INJURY AND DIAGNOSIS. FURTHER INVESTIGATION WAS NEEDED AND R EPORTABLILITY WAS DETERMINED ON 7-17-01.
