While lifting a gear with the crane on truck, chain jumped & hook on chain hit right wrist & thumb

EE was using an air tool with a flapper wheel installed on it. He was working on a hoist cylinder for a haul truck. He was cleaning the packing seats to replace the packing. While cleaning the seat a particle of foreign material got in his left eye. He was wearing safety glasses, dust mask and ear muffs but admitted he should have worn a face shield or goggles.

Employee was greasing inside the house of the dragline. Employee had to squat down to grease a fitting when he felt something pull in his right leg (calf area). First day off work 1/27/09.

Over-exertion in lifting objects

Employee was loading engine onto truck. He needed blocking. EE and another employee were lifting a 12x12x48 wooden block onto the truck. At mid lift EE's back snapped.

EE WAS CHANGING CUTTING EDGE ON LOADER. HE CUT HIS LEFT MIDDLE FINGER ON A SHARP BOLT HEAD.

EE WAS TRIMMING BELTING TO FIT BACK OF SCREEN WITH A UTILITY KNIFE. EE WAS KNEELING AND KNIFE WENT THRU BELTING AND CUT HIS INNER THIGH ON HIS LEFT LEG.
