Dust was in the air after the use of a pneumatic air tool which caused irritation to the employee's right eye.

Employee's right finger was pinched between a come along and a metal plate causing a break to occur requiring a splint and restrictions.

Employee was washing down the scale area when employee slipped and fell onto the same level landing on knee. Employee used first aid until employee requested a clinic visit on 10/10/22 where PT was prescribed.

The employee was operating a haul truck when it caught fire. The employee exited quickly and received burns to their left arm and wrist.

Injured worker burned arm when IW reached directly in front of an acetylene cutting torch IW was holding. The IW received 2nd degree burns to right wrist.

Over-exertion in lifting objects

While lifting support stands to be placed under a haul truck, employee lost grip and then caught support stands which pulled EE down.

Flash burns (welding)

While pre-heating the weld on cutting edge employee was exposed to ark flash. Employee failed to wear appropriate PPE while performing this task.

Fall from machine

The injury employee was climbing out of a screen where he suspects he wrenched his lower back. The injured employee didn't have any pain until he was driving his haul truck approximately 1/2 hour later. The injured employee was treated and returned to modified.

While performing clean up in the finishing screens building at the tertiary plant employee had some material fall from above and hit him in the head and left shoulder.

The employee sustained injury to both shoulders that required treatment with cortisone injections and returned to modified duty. The injured employee was swinging a 20 lb. sledge hammer to remove a seized pin from the bushing in an arm of a Cat loader.

Slipped and fell hitting chin on conveyor while trying to remove a speed sensor.

Struck against a moving object

Employee was driving a tractor-trailer, loaded with fines, down into the pit, as he reached the intersection of the ""New Ramp"" and ""4th Level Ramp"" the tractor-trailer overturned into the bank. The employee was pinned in the overturned cab and had to be removed by emergency personnel. The employee sustained a fractured pelvis.

Flash burns (welding)

Employee was tack welding a piece of steel. He had the dark shield of his welding helmet and not protecting his eyes at the time he was tack welding. This caused him to get flashes in both eyes.

EE was climbing down from the grizzly belt to the screen below. He then stepped from the screen to the handrai. As he was climbing down his foot slipped and his shin hit the I-Beam.

Employee had finished his shift and parked his haul truck; when employee reached bottom step which is fastened to the truck by cables it swung in and his foot got caught under fender guard causing employee to fall backwards to the ground. Employee went to doctor who took x-rays and found three broken toes and a sprained ankle. Employee was put out of work until further notice.

Employee was working on removing a spindle on a pick up truck using an air chisel. A sliver of steel got in his left eye and cut his cornea. He went to an eye doctor who removed the steel and prescribed medication and told him to stay out until 10/24/07 due to chance of infection. He will go back on 10/23/07 for a check up.

Removing bolts from broken step on front end loader, one bolt was removed. While removing 2nd bolt with air wrench a back up plate that the bolts went through was seized to 2nd bolt. The plate turned with the bolt & came around & hit finger on hand that was holding it.

Employee was trying to dislodge a rock from a feeder pan with a bar. The bar slipped from the rock, and the employee's finger was pinched between the bar & chute wall. This resulted in three stitches on right hand, pinky. Employee was back to work the next shift with no restrictions.

EE WAS CLIMBING STEPS TO HAUL TRK CAB & LOST HIS FOOTING & FELL TO THE GROUND. THE INJURY TO EE'S LEFT ARM/SHOULDER, AS A RESULT OF THE EE'S HOLDING ON & TWISTING AS HE FELL TO THE GROUND. STEPS WERE WET WITH LIGHT RAIN. EE FELL APPROX 3 FEET.

EMPLOYEE WAS REMOVING A PAIL FROM UNDER THE ASPHALT PUMP WHEN SOME LIQUID ASPHALT SLASHED OUT AND BURNED HIS LEFT HAND.
