Employee was checking oil in hauler and was standing on steps. They slipped and struck diesel fill nozzle with right wrist. EE suffered undiagnosed non-specific wrist injury. EE was placed on restricted duty.

Employee was cutting a liner with hand saw and cut index finger on left hand.

Employee was trying to loosen a nut on an oil screen and lacerated hand.

Caught in, under or between a moving and a stationary object

Employee was placing PTO onto ground when left little finger got caught between PTO and nut that was on ground. Finger was pinched. Employee required 3 stitches. No loss time or work restrictions imposed.

Flash burns (welding)

Employee was welding at the primary crusher. A flash from another welder came behind his PPE and affected his R eye. He did not realize the injury to the eye until later in the evening.

Empl. helped to reline crusher feeder on Fri. Did not pursue medical help for strained back until 4/2/09. At time of appt. he received prescription meds but reported to Supv that he had not taken the meds. It was not until further questioning last week that we became aware that he had taken the meds which made the event MSHA reportable.
