Over-exertion in pulling or pushing objects

EE was working on connecting two pieces of 12""x8' pipeline. EE attempted to move a retaining collar to one end of the piece for connection, the pipe was elevated. EE was in a kneeling position while attempting to move the ring. As EE pulled on the ring, the ring became wedged on the rubber stopping it. The force of the stop caused EE to sprain lower back.

Drowning

A 19 year old man died while swimming in the quarry. Police report states he went underwater and never resurfaced. The quarry is a restricted area with no trespassing signs posted. Fire department reported this incident to MSHA but did not notify Eastern Industries. This person was not an employee of the company.

Drowning

An unknown trespasser was swimming in the closed, abandoned quarry section of property with several friends when he entered the water at approximately 2:45 pm and did not resurface. His friends called 911 and his body was recovered at approximately 4:00 pm by local water rescue team divers. State police were on scene but did not contact the mine owners.

Employee was disconnecting a hose from the water truck when he struck the bumper with his left arm, causing a laceration.
