The employee sustained a left forearm laceration while using a pair of scissors as a draw knife to cut scrap cardboard into smaller pieces for disposal. The employee was pulling the scissors toward themselves when the scissors slipped and cut into their forearm, 4"" above the wrist and glove, causing a laceration requiring 4 stitches.

Employee collapsed as a result of a personal medical condition, striking their jaw as they fell. The fall resulted in a chin laceration and temporal bone fracture.

Miner was kneeling and using an angle grinder to remove welding slag when the grinder caught and kicked back, striking the miner in the left thigh just above their knee resulting in sutures.

Miner was setting the brake on a rail car. Miner was reaching overhead and pulling to rotate the brake wheel when they felt a pull in groin area. Strain resulted in a hernia in the left groin area.

Cable for lower flexible step on Volvo A35D Water Truck broke when employee stepped up on it. Employee did not fall to the ground, but strained EE's lower back by holding on and lowering self.
Miner was using a sledge hammer to separate a bucket tooth from a shank which had broken off the bucket. On the fourth swing, a small shard of metal broke off the bucket tooth and struck the miner in the left leg below the knee. The metal embedded into the shin.

Miner was shoveling sand spillage from under tail of conveyor and felt a sharp pain in lower back radiating down left leg. This resulted in a muscle strain which induced the sciatic pain.
Miner alleges lower back pain from operation of a front end loader. Reported at 9:00 am 11/1/18...no definitive time of a specific incident.
Miner used a drift punch and chisel to remove the top of a bearing block so the bearing could be replaced. A small metal ""splinter"" from the block penetrated the miners glove and embedded in the 5th finger of the left hand.
Miner was cutting rusted steel with a torch. the miners pant leg had pulled up and was stuck on the tongue of EE's boot. Slag from the work piece entered the top of EE's work boot resulting in a burn.

Two maintenance persons were tossing a piece of irregular shaped scrap cast iron pipe into a scrap metal dumpster. Piece caught edge of dumpster and spun and pinched two fingers between pipe and dumpster
Employee slipped on wet, partially icy area and twisted the right knee. Did not fall. Was referred for physical therapy
Employee unlatched the left rear fender of the L-350 Loader to swing it open. When the employee grabbed the handle and pulled to swing the fender open, EE felt a pulling in right lower back above the pelvis and wrapping around the side to right groin area. Employee also felt a strain/pull in the front of the upper right thigh.

While exiting Cat 385 excavator, the operator noticed the egress ladder was full of heavy mud. The operator attempted to wipe the mud off the ladder by sweeping it with left foot. While doing this, EE felt a sharp stinging pain in left foot. Not reported to supervisor until following day.
While returning from the break room to EE's haul truck, the employee's right foot slipped on a small patch of snow covered ice and fell.
Employee was delivering a small sample bottle of sand to the lab and was descending the stairs at the end of the D5 conveyor when EE heard and felt a pop in EE's right knee.
Employee was removing feed distribution box from Rotex lid. While pulling on upper wrench to loosen the nut, the wrench slipped off and struck employee in lip resulting in laceration to lower lip and chipped top right front tooth.

Employee was using 385 excavator to pull down material from a 50 ft bench. A single 5-6 tn boulder which had been contained within the face of the material above, toppled off the top while the operator was swinging the excavator. The rock struck the tracks first, then upward into the front end of the cab. Case just became reportable when employee needed to seek physical therapy.
Employee was cutting steel angle using a torch. When oxygen was applied, the cut pocketed and returned slag and sparks towards employee. Slag bounced off of worker's cheek, onto his safety glasses and into his right eye.
Employee was moving seal pump from storage location on floor or storeroom onto a 2 wheel cart in order to transport to a chain hoist. Employee bent over and lifted the pump/motor assembly onto the cart and strained low back.
Two Dump trucks hauling on dirt road when 1st truck stopped, 2nd truck did not see due to the dust and struck the rear of 1st truck. Driver of 1st truck momentarily lost consciousness resulting in his truck going over small embankment. The injured employee was taken to hospital and received treatment for cuts on head, broken collar bone & possible concussion. Both trucks disabled.

Employee was preparing to lower a catwalk into place. Employee was under the catwalk holding it in place in the raised position, over center. Another employee removed the bolt which secured the catwalk. The catwalk started to fall unexpectedly before help could get there. Employee pushed the catwalk back over center to raised position which injured back.

Skidsteer operator was picking up rods with the skidsteer and the skidsteer had rolled forward from the pressure of him lifting and when it broke free, some of the rods came off the forks with one striking right foot.
EE claims he has Carpal Tunnel based on activities as a mechanic at work.
Employee was changing cutting edge on skid loader bucket with impact wrench and caught his glove and left middle finger on rotating bolt head lacerating finger.

Employee was opening 2nd lid on rail car. He lifted the lid with his left hand and felt a sharp pain in his left bicep.
Employee was moving bucket teeth from truck to scrap dumpster. Felt sharp pain in upper back and right arm.
Operator was trying to close the discharge gate on a rail car using a spike puller. The spike puller slipped out of employees hand and fell on his right foot above steel toe.

Employee was driving haul truck on new uneven haul road. Employee woke up the following morning with back pain.
Employee was pulling on a wrench to loosen hydraulic tank drain plug and felt a pull in right elbow. Injury was reported on 2/4/11 but first medical treatment was 3/1/11.
On 1/5/11, employee was exiting the CAT 385 and upon stepping from the top step to the second step, it was pushed into the top step. Right leg went to the ground and left leg was stuck on top step, twisting left knee. Injury was reported on 2/23/11. Received Medical Treatment on 3/2/11.

Employee was filling sample jars and needed to replace the jars. The box of jars was on top shelf. When employee pulled the box down, it fell, causing the weight of the box to pull on EE's shoulder. *Note: Jars are filled with sand.
Was guiding a wheel assembly on the spindle shaft of haul truck and assembly shifted. Pinched left index finger between planetary and hub.
Seat is a low-back bucket seat & does not support part of the torso. Over the years of operating on uneven surfaces, a numbness in the thumb, forefinger & index finger of both hands has occurred. (Injury has just now became a medical as employee is now receiving medical treatment.)
While charging mill, one rod did not go in all the way. Operators did not look before starting the next rod. Once the next rod was moving, employee tried to stop it before it hit the rod that did not go in, causing his left knee to twist.

Unloading 1 1/2"" round cold rolled steel rods from a truck individually. Each rod was being blocked on one end only. When they went to unload 2nd bar, the load shifted. When this happened, the steel bar struck employees left hand, thus causing his left thumb to be injured.

EE was reaching down underneath belt to lift roller out of hanger and while pulling on roller it did not move but shoulder was pulled. He has been treating with a physician but has continued to work per doctor's instructions. Doctor decided to do surgery on 5/1/2007 to repair a torn rotator cuff.
Employee was removing burnt slag from a piece of metal. While doing this task, a piece of slag entered into employee's right eye.

Before starting work, EE decided to clean SUV/Explorer of material from day before. When opening rear hatch door, He did not notice motor frame from previous day slid and was resting against hatch door. Once he opened hatch door, the motor fell from vehicle onto his foot. He had not changed into safety shoes. He wanted to clean vehicle before he changed into his ppe.
The employee was working in a confined area. He was pulling down on the adjustment handle and the upper man was sliding a tool over the adjustment handle. The tool slipped off the adjustment hitting him in the two front teeth.
EMPLOYEE WAS LIFTING PLATES FOR MILL END LINERS WHICH RESULTED IN PAIN IN LOWER BACK.

EE WAS COMING TO WORK AND WENT TO ENTER LAB DOOR. WHEN HE REACHED FOR DOOR HANDLE HE MISSED STEP. HE THEN LANDED ON NEXT STEP WHILE STILL HOLDING ONTO HANDLE. HE THEN FELT PAIN IN HIS LOWER BACK AND HIP AREA. (IMPROPER LANDING AND STEPS CONTRIBUTED TO ACCIDENT.)
EMPLOYEE WAS PUSHING OVERBURDEN TO EXPOSE SAND PRODUCTION. IN DOING SO, HE FELT AND HAD PAIN IN SPINE AREA. THE DOZER IS ONE OF THE WORST MACHINES RE: JERKING, THUMPING, HARD ON OPERATOR. LOADER CAN BE ROUGH ALSO.

EMPLOYEE WAS GRINDING EDGES SMOOTH WHERE THEY HAD TORCHED PIECES OUT OF A 2"" PIECE OF FLAT STEEL. THE GRINDER GOT CAUGHT IN THE HOLE CAUSING IT TO BE THROWN FROM HIS HANDS THUS CUTTING HIS RIG HT FOREARM.
