Struck against a moving object

The Euclid R-Haul truck Comp. No. 671 was hauling a load of stemming material to the drill bench in the Wadesville Pit. The truck went over an embankment, upset, coming to rest on the driver's side causing fatal blunt force trauma to the driver located inside the cab. The cause of this accident is yet to be determined pending further investigation.

Employee was riding in the rear of a service truck and had hand resting on the crane perch. The truck leaned to one side causing EE's hand to be caught between the crane boom and perch.

This miner was found unresponsive in the back of a lube truck. 911 and MSHA were immediately notified. Upon further investigation it turns out this was the result of a non work related medical condition.

Employee was climbing on the machine to start shift when they slipped. The cause may have been some mud and frost on the machine. Employee did state they cleaned off their boots as best as possible.

Accident type, without injuries

A non injury accident occurred when a 785B truck came in contact with a 69KVA power line owned by a utility company. The operator was hauling rock for road maintenance. The operator dumped the load and drove away with the box still raised underestimating the distance to the power line.

Employee was climbing ladder of a WA800 wheel loader while carrying a fire extinguisher. Employee felt a pull in right arm resulting in a strain and then banged right leg against the loader resulting in a contusion.

Carpal tunnel due to repetitive motion operating mobile equipment and hand tools. Accident occurred 1/31/2020 but EE has been working constantly up until 9/23/2020 and was told by Dr. that EE has to stop.

Employee climbed down ladder of truck to the ground. Proceeded to walk away and slipped on ice. Grabbed ladder with left hand to break fall resulting in strained left shoulder.

While walking across uneven ground, employee stepped on a rock and twisted right knee. Employee has been working all this time till 9/30/2020, employee gets surgery today.

While dismounting the truck employee climbed down the ladder to the bumper. EE did not realize that the under bumper ladder was missing. When EE stepped over the bumper from that missing ladder EE fell to the ground.

Employee was driving service truck thru pit on haul road. EE drove into a large water formed pothole and jarred back.

Employees were in process of removing a pump from a mine shaft. When one employee started to cut a drive shaft coupling with a torch, a gas ignition occurred causing burns to another employee.

Flash burns (electric)

Overhead wires knocked down by dump truck. Lines on ground were de-energized. One line on pole appeared energized and was not knocked down. De-energized line came in contact with energized line. Flash occurred causing a burn.

Employee was doing pre-shift inspection of PC 4000. Tripped in engine compartment and struck their ribs on firewall.

This employee, along with 4 others, was hoisting a pump. The wire rope connected to the pump got wrapped around a rock on the ground. When it came loose it snapped and struck this employee across the upper chest. He was then lifted up and fell backwards down over the edge of the mine pit the pump was being lifted out of.

Deceased was a trespasser who was electrocuted while stealing copper from a sub-station.
