Over-exertion in lifting objects

The coworker was carrying 2 flat plates of steel weighing approx. 22lbs combined weight when they felt a pain in right shoulder. They stated that it felt like they were stung with a rubber band. Their shoulder began to bother them and their fingers were getting numb. Health line was contacted and was advised to seek medical attention.

EE was walking through the plant when EE stated that EE tripped over a pile of dirt and fell to the ground.

Employee was walking down steps at the plant, when EE stepped onto the ground from the last step EE rolled EE's left ankle. EE has a sprained ankle.

EE was pulling hand cart with a 55 gallon drum on it through a doorway. The cart became stuck on the door sill, as the employee pulled harder, employee felt pain in employee's back

Struck by flying object

Removing a PTO from a mobile piece of equipment using wrenches and screwdrivers. A piece of metal got behind the safety glasses and into EE's eyes.

Struck against a moving object

While operating the dozer in reverse, the EE had head turned and struck a rock which jarred their neck.

while helping to remove a bearing off of a screen shaft, an employee was using a hammer and struck the bearing causing a piece of metal to fly striking injured employee in the lip it was thought to be a laceration, but was later found to be imbedded in injured employee's lip

EMP'S LEFT HAND WAS INJURED WHILE DOING MAINTENANCE ON A ROCK BREAKER. THE EMP'S LEFT HAND WAS CRUSHED BETWEEN THE ROCK BREAKER AND HAND RAIL. EMP HAD 7 STITCHES.

Emp was removing snow and ice from scale area when he slipped on ice and fell. Emp's right hand hit a concrete barrier on the way to the ground and he received a laceration needing 2 sutures. Emp returned to work on 1/13 with limited use of right hand. Emp followed up with doctor on 1/14 and released to work with no restrictions, normal duty.

Emp. was pulling a cable and had his knee in place for support, when he began pulling, he felt a pain in his knee. Emp did not seek medical treat at the time of the accident, but later went to er for swelling and soreness. Emp missed 1 day of work, but is still on light duty.

Removing a piece of drill pipe that needed replaced and as employee was lifting the pipe he got a sharp pain in his left arm-bicep area.

Employee was walking on the catwalk of his euclid and slipped on oil. Employee caught himself and didn't fall, but hurt shoulder. After treating with doctor it was found he tore his rotator cuff.
