emp was operating a haul truck when he felt pinching in his right leg. emp reported the incident on 10/24/2011. emp missed 5 days of work for muscle spasms

EMP WAS PUTTING OIL IN PL090 WHEN OIL HOSE POPPED OUT AND STRUCK EMP IN THE HEAD CAUSING HIM TO FALL BACKWARD ONTO SHOP FLOOR. EMP BROKE BACK RIBS

Employee hurt his knee while he was walking through mud, when he slipped and fell.

Emp was stung by a bee. Stated he did not feel well and went home. He called his doctor who told him to take over the counter medicine. No doctor visit made. Emp returned to work on 7/10/07 at normal time. Emp quit the job at 8 am on 7/10/07. Emp never showed anyone where he was stung.

EE was welding and hot slag got in under his helmet and into his right eye

Employee was making a lift of 16-50ac feed tub and shell. Employee pried load loose and when it gave way, employee's leg slipped to the outside of crusher. The load the came back and pinned leg between shell and crusher (deep muscle bruise - right leg).

EE WAS REPLACING BELTING ON CONVEYOR WHEN IT (BELTING) CAME LOOSE AND HIT EE ON HEAD, KNCOKING HARDHAT OFF THEN GIVING HIM STITCHES.
