Over-exertion (Not Elsewhere Classified)

The roof of the MCC Room was leaking. Rainwater made contact with the main breaker causing the breaker to trip shutting down the plant. EE strained back while attempting to tarp the roof. First aid measures did not provide relief. Later in the week, ee received medical treatment from a workers compensation panel provider.

At the wash plant walking down the steps slipped on the steps which were wet and oily tried to catch self on hand rail and sprained right shoulder.

Employee was cleaning a box under a conveyor that was shoulder high. EE was using a 6-foot digging bar when EE felt a pain in EE's right shoulder requiring medical examination and lost time. Employee returned to work 9/23/2020

Changing dust screens at Wash Plant strained right knee in confined space. Employee submitted accident report on 9/16/16 but did not notify Superintendent that they were having surgery till 10/25/16. Employee is scheduled for surgery 11/3/16. Employee has been working normal job duties in the meantime.

Cleaning out silo with shovel, scraping and shoveling, strained lower right back.

Caught in, under or between a moving and a stationary object

Employees were flipping hammers at a horizontal crusher - STEDMAN CRUSHER. While flipping hammers, you have to move them to align them up. Employee caught middle finger under hammer with gloves on. Employee never looked till break time at approximately 9:00 to check out his finger. Employee reported right away.

Struck against stationary object

Breaking concrete slabs and stepped on a piece of heavy wire and punctured right foot. Foot swelled up next day and required hospitalization.

Fall from machine

EE jumped down from conveyor apprx. 3 ft to ground and strained back, he went to his personal chiropractor and continued to work. On 5/10 he was working at VSI and leaned back to put up angle iron and back tightened up, next a.m. he was unable to work and went to see a chiropractor and was restricted from work.

Fall to the walkway or working surface

EE was walking along cat walk and slipped on ice, he tried to catch himself with his left hand and dislocated his left ring finger in the process.

Employee was stacking bags of lime and twisted lower back, which caused a strain. He is currently being treated by a chiropractor daily.
